Our customers download the app onto their device. They will need to create a mini profile so the app will be customized to their medical needs when it comes to controlling and improving their diabetes. The app will make recommendations that will aide in improving our customers’ health. There will be links to diabetes related resources such as to articles, websites, doctors’ contact information, tips of the day, and workout plans if needed. The plan is to have doctors receive notifications when a patient is showing abnormal result from their monitor. Also have it where a weekly download will be available to the health care provider to see if any concerns arise. This will be an additional fee that will be picked up by the insurance company.

Unrecognized pre diabetes is also a growing concern that is increasing dramatically. Diabetes is not diagnosed for most homeless people, because they do not do have a yearly physical check-up. Published guidelines are useful to patients and practitioners because they focus on the improvement of care. Clinical pathways are also important, because they focus on the outcome and assessment of their achievement.
Portals can help in manage poly-pharmacy, provide patient education, engage patient and family in the care of managing chronic illness (Fant, 2015). When patients have access to their personal health records, they feel empower and want to do something about what is not normal. For example, a patient who is newly diagnosed with chronic kidney disease will have series of labs depending on which stage the kidney disease is in (“Understanding your lab,” n.d.). If the patient has access to portal, than the patient can easily access the lab results and share with his/her neprologist, dietitian, dialysis center, and other provides that the patient sees. There is no delay in retrieving the information or faxing or mailing the labs to the patient. Portal is very convenient as patient can be sitting at the comfort of the home to view labs, order medication refill, or even schedule an appointment. My research project is primarily focused on examining the uses of Nightscout and evaluating changes in health behaviors and health outcomes associated with its use on patients in managing their diabetes. Nightscout is a do-it-yourself (DIY) mobile technology system developed by patients and caregivers to monitor blood glucose levels remotely using Cloud technology. We are interested in studying Nightscout because it is a new patient-created innovation, and is the first successful DIY system to allow patients and caregivers remote access to data, such as blood glucose levels, through mobile technology. Since Nightscout is still in the earliest stages of development, we focused on studying the community of Nightscout users for issues such as

Mobile health or mHealth, provides an entire new aspect of the relationship between patients and their doctors and other medical providers. Mobile devices place important and critical information into a medical professional’s hands in real-time. Doctors can monitor a patient’s condition more frequently, allowing them to make better and more informed decisions and diagnoses.
Project management takes the form of many job positions and roles within a job force. Project management to some, means a person who manages a project. This is a very broad title. Project managers can manage a project that covers a new program release, an implementation of a new method, or the creation of a physical product for a customer. For me, project management is taking a task from start to finish, with the goals of the customer in mind. Three roles that I would be interested in accepting would be a Retail Construction Project manager, Construction Project Manager, or Project Manager for Space planning.
