On February 23, 2017, the Mini-Mental State Examination and the Role Checklist was administered to Mrs. Cecile Sullivan. Results from both assessments along with the information obtained from interview process were then used to analyze her cognitive capacity and identify her assets and limitations. Moreover, this information were also used to determine and understand client’s concerns and difficulties in adjusting to the aging process and enormous life changes.
Cecile is a 95-year-old Caucasian-American female residing at Landmark Monastery Heights in West Springfield, Massachusetts. Mrs. Sullivan stated that she lost her spouse many years ago and has not remarried since then. During the interview process, she expressed profound sadness (crying) and disappointments towards her family, as she felt forgotten and abandoned. Sullivan resided and received her high school diploma from a secondary school in South, Holyoke, MA. Prior to her admission to the facility, she raised horses and ran a horse business, which was owned by her family for many generations. While it was her main source of income, she is also very passionate about horses and considered it a hobby.
Unfortunately, a full medical history was not obtained because client was not oriented and also exhibited lack of awareness. Client identified being a parent, a friend, wife, and horse owner as her main life roles. In the middle of the interview process, the client showed frustrations of her inability to recall. Interview was stopped since client could not answer further questions as she became verbally disorganized and was unable to control her
