Smalls (MHP), Destiny and Ms. Beaton (MHS) review Destiny’s Individual Plan of Care (IPOC) and discuss Destiny’s behaviors. Intervention: MHP provide Destiny and MHS a copy of the draft IPOC. MHP, Destiny and MHS review and discuss each section of the IPOC. MHP answered Destiny’s questions after redirecting Destiny’s display of verbal aggression. Action: MHP and MHS discuss Donovan’s behaviors in the home and community. MHP and MHS review monthly paperwork that is needs to be submitted. Focus: Ms. Smalls (MHP) administered the Casey Life skills assessment, the Circle of support and the Child and Adolescent Needs and Strengths assessment with the assistance of Ms. Givens (MHS) and Antazia.
Intervention: Ms. Smalls (MHP), Santonias and Ms. Givens (MHS) discussed Santonias complying with the rules of the home. MHP and Santonias had a discussing about his NFL football team as a way to build rapport. MHP requests feedback on possible goals from Santonias and MHS for Individual Plan of Care. MHP, Santonias and MHS discuss and process an incident that happened at the school’s gym.
Focus: Santonias and family will learn and develop firm, consistent limits and maintain appropriate boundaries. Ms. Smalls (MHP), Santonias and Ms. Givens (MHS) debrief recent incidents in the home and community.
Behavior: Reyna behavior has improved from the initial date of placement. Youth became very proud when achieving her goals. Reyna completed his goals of complying with rules of the home and school on a daily basis, express her frustrations in an appropriate manner and have bi-weekly visits with bio-family. Youth learned various ways to cope and express her feelings at individual therapy and SAFY clinicians.
Goal 1: Kayla will improve her social functioning. The director was forthcoming with providing Information about Kayla's numerous behaviors. The director reported, "Kayla started throwing chairs and getting physical with her peers we called both foster parents and was going to call urgent care."
Intervention: MHP, MHS and Skylar discuss steps Skylar take before deciding daily. MHP review the outcomes of each decision Skylar made in the past month. MHP, MHS and Skylar role-play Skylar deciding to gain insight. MHP challenge Skylar’s thoughts on thinking through each step before deciding for the best outcome. MHP, MHS and Skylar discuss some outcomes from recent decisions. MHP praise Skylar for processing decisions.
Intervention: CSP, MHS and the youth discussed weekly improvement in the home and community. CSP and Miasia review the youth’s feelings and thoughts for the past week in the home and school. CSP, MHS and Miasia discussed ways to improve mornings and keeping on task. CSP and MHS praised Miasia for her progress on this goal. The youth had zero incidents of disrespecting others belongings. CSP provided reinforcement interventions for respecting boundaries in diverse settings. CSP discussed medication management concerns with MHS.
CM spoke to Destiny (youth) regarding a follow-up on her court date. Destiny noted her upcoming court date is scheduled for Wednesday, 4/26/17 at 9:00am in regards to receiving custody and visitation rights for her daughter. Destiny reported she has a job interview scheduled for Tuesday, 4/25/17.
Service: CM will reauthorize IIC services when IIC update PNs and treatment plan. IIC will work with youth for 2 hrs/wk in order to continue assisting youth with effective coping strategies when angry, developing positive communication skills, improving good judgment and to help youth listen and follow caregiver’s rules. The family will continue to work with Maria Flores (FSO partner) as needed in order to be linked to community resources and receive peer to peer support services. The family is Moderate level with FSO.
Intervention: MHP request for a report of Samantha’s recent displayed behaviors. MHP, MHS and Samantha process displayed behaviors in the home and the outcomes. MHP and MHS listen to Samantha verbally express her emotions. MHP validate Samantha’s emotions about being in foster care and wanting to be with biological family.
Intervention: MHP asks Taliana and MHS for a summary of Taliana’s behaviors for the past two weeks. MHP, Taliana and MHP discuss reasons for redirection and display of verbal aggression. MHP challenge Taliana’s thought about communicating with biological father. MHP and Taliana process triggers to verbal aggression. MHP and Taliana discuss using a disrespectful. MHP, Taliana and MHS discuss using coping skills used in the past two weeks. MHP demonstrated active listening skills and requested Taliana to practice the active listening skill. MHP encouraged Taliana to acknowledge the negative behaviors and process the behaviors for better outcomes.
Health Care Integrator (HCI) met with Shawn at the park for the first visit of the month to provide him with coordinate services, evaluate satisfaction with services, and to track Shawn’s progress in the program and towards his goals. Shawn was at the park with biological mom’s (BM) male friend. Shawn was sitting on the mat playing with grass. HCI approached Shawn and asked him can they sit on the benches so they are able to talk. HCI inquired to BM male friend about her whereabouts he stated that BM was at the home and she was painting one of the bedroom. HCI will continue to assess in meeting Shawn’s needs and preferences.
Ms. Smalls (MHP) provides MHS and Destiny support and encouragement at bi-weekly face to face visits. MHP, MHS and Destiny is establishing a meaningful relationship. MHP, MHS and Destiny practice managing emotions and communication in the bi-weekly face to face visits through positive interventions, role-play, and practicing of coping skills. MHP modeled the coping skills and interventions for Destiny and MHS at the bi-weekly visits. Destiny receives Behavior Modification services to alter reported negative behaviors.
QP provided Quadir with a CBT activity geared towards ending IIH services. QP Reminded Quadir and his mom that there a few more session left before he is discharged. QP explained to Quadir that the purpose of today’s session is to review goals and techniques learned to far in services. QP reviewed with Quadir anger management skills. QP asked Quadir, how he thinks his anger management skills has change since starting service over 6monthas ago. QP encouraged Quadir to discuss some of his low point of when he reacted poorly to anger situation in the home and at school.
