Brian Ducre was arrested by DPS Capitol Detail and booked into EBRPP on May 5, 2016 at 7:58 a.m. on charges of Disturbing the Peace and Obscenity. Ducre was released on May 15, 2016 at 11:52 a.m. after he was pronounced dead. Ducre signed an Intake form on 05/05/2016 acknowledging receipt of the inmate handbook (which includes inmate medical services and the inmate grievance procedure. Ducre signed the Committing Jailer Reads to Prisoner form on 05/05/2016 indicating he was not taking prescribed medication, was not under the supervision of a doctor and did not need to see a doctor at that time. Trey Cavalier was assigned to M&N control for the night. That morning (which was May 14, 2016 because he was working nights) about 30 minutes to an hour (he could not give exact times because log books were being picked up for the investigation) before 4:00 a.m. Dy. Cavalier was notified by Dy. W. Benson that Brian Ducre (who was a new move that was escorted to M&N on a wheel chair) was acting strange and that medical needed to see him. Dy. Cavalier called the medical department and spoke with Nurse Minor and advised him of the situation. Cavalier’s shift ended he was held over and assigned to N Wing on B-Shift. During that time, Nurse Bates came to M&N to conduct Pill call. While Nurse Bates and Dy. Cavalier were conducting pill call on N01, Nurse Bates attempted to give inmate Ducre his medicine but Ducre did not respond and just looked at them. Nurse Bates then advised that she did not know why Nurse Minor moved Ducre to lockdown. She said he was not supposed to be there because of his injuries. That was the last time Dy. Cavalier heard anything about the situation before he was relieved at 8:00 a.m. Once Dy. Cavalier returned to work on Saturday May 14, 2016, he was again assigned to M&N control. Half way through his shift he was advised by Cpl. LaMotte that he needed to contact medical and asked them if they could come and check on Ducre because he was acting strange. Dy. Cavalier contacted medical and spoke with nurse Sterling and she advised him that she was eating and that she would let the others know and someone would come. For the remainder of the night Dy. On Thursday 11/12/2015 at 1905 hours, during shift briefing, Officers Jason Peterman and William Miller pass down information concerning what was believed to have been an elopement from CDU by a male Baker Act patient. He was reported missing by Registered Nurse Ophie to Doctor Gomez at approximately 1530 hours. Doctor Gomez who was concerned for the patient’s safety and wellbeing then contacted Primerose Vernet from the Florida Hospital Risk Management Department assigned to our facility and she in turn contacted the Orange County Sheriff's Office and reported the issue.
OFC Montejano made contact with the Hegle at Primary Care Desk B lobby. Where the she gave an omission and stated she had an appointment and because the Johnson was not at her desk she missed it. Further, she claims Dr. Little told her she could get the Tens Unit right away. I did however confirm that Hegle did not have and appointment for the use of the Tens Unit. On Thursday, August 27,2015, @ 5:29 pm ,Tyneice Daishae' Key( DJS Youth ) was redirected by Dennis Thomas ( Senior Residential Staff) to return to Tours Unit. Tyneice Daishae' Key( DJS Youth ) became verbally aggressive towards Telethia Menefee ( Residential Staff). Tyneice Daishae' Key ( DJS Youth) also began to curse and threaten to hit April Smith ( Supervisor of Tour Unit). Tyneice Daishae' Key( DJS Youth ) was redirected to her room to take time away. On Thursday 12/24/2015 at approximately 2307 hours. Security Officer Omar Alonso (420) was contacted by E.D. Charge Nurse Sharey Selover about an uncooperative intoxicated male patient, Jose D. Gonzalez (DOB: 03/30/1977; FIN# 85006354), come in through the EMS Offload area. Officers Alonso and Ayuso reported to the call and observed an intoxicated male being wheeled into the Special Care Unit (SCU) E.D. room # 39. According to his assigned Nurse Sara Lopez, the patient had been involved in a physical altercation and had been kicked hard in the groin area. Patient did not behave badly or disruptive once he saw that Security were present and his Nurse was able to get his vitals, blood work, and urine without having any issues. Security staff
I spoke with Versie Crossland the cook at operation on 11/16/15. Ms. Crossland stated she was in charge the day of the incident while D'Andrea Franklin and Brittany Shavers were on bus route. Versie stated she believed Nelda Shaver the director was at her other operation in Queen City that day. Versie stated she was cleaning the kitchen up and preparing snack for the after school children when BreAnn Henderson came in the kitchen and asked for an ice pack. Versie stated she asked BreAnn what happened, BreAnn replied that Daniel had fell and hurt his forehead. BreAnn also told Versie to call his mom. Versie said she was walking to the office to get the phone when she saw D'Andrea Franklin pull up so she went out and told her what had happened.

He was brought in by his friend Gavriel Martinez. At 2338 hours Orange County Sheriff's Deputies Kenneth Jorgensen (Badge# 8180) and Deputy Michael Fairley (Badge# 6210) arrived and interviewed the GSW patient, both deputies were already on site for an unrelated event. Security Manager Richard King was informed by text of the incident at 2340 hours as well as Nursing Supervisor Debra Reilly. At 2350 hours OCSO Deputy Kristine Helms (Badge# 5021) arrived and requested to speak to the patient’s friends who was waiting in the E.D. lobby. At 0010 hours Deputy Michael Fairley contacted SO Alonso and advised that it was OK to open the Emergency Department. The lockdown was lifted and Security Manager and Nurse Supervisor were informed of the incident. The patient, Lopez was airlifted to Orlando Regional Medical Center (ORMC) at 0030 hours at which time OCOS Deputy Kristine Helms informed me that the Gang Unit task force has been called for further investigation. On 11/07/17, I D/O B. Bailey #D1200 was assigned to the Medical Clinic as a floor officer. At approximately 0230 hours. Inmate Bowman, Torry BKN #1720190 refused medical treatment. As a result, he was given a refusal to sign so he could be returned to his floor. Inmate Bowman was given a pen so that he could sign the refusal. However, Inmate Bowman did not return the pen. Inmate Bowman was given several verbal commands to return the pen; however, he failed to comply. Lt. Steele was notified. Consequently, 8 male inmates in 3LB 23 were searched also a strip search was conducted. No pen was recovered.
On 8/25/2015 S/O EMT Perez was dispatched to PV-509 regarding severe leg pain. S/O EMT Perez knocked and announced his presence at the door and was verbally invited in by the resident a Mrs. Margaret Folkins. Mrs. Folkins stated that she had been in severe leg pain the entire day and she was having difficulty resting. Mrs. Folkins also stated that she was at the hospital for it the day prior because of it but the cause of the pain couldn’t be diagnosed. According to former employee Stephone Else, medication aide, alleges she heard Ms. Lyles tell Phillip Yovonnie, LVN to “Take her to China” with Amara Anele, RN, and Lilian Foster, Medication aide present at 6:20-6:30am. On Thursday 07/21/2016 at approximately 2223 hours, Security Supervisor Steven Evans was contacted by Assistant Nurse Manager Robbie Philips via landline and asked to conduct a (44V) Enforcement Escort Visitor Off Property for the discharged female patient in E.D. #48. The patient, Susan Harris (DOB: 03/22/1952), had been quarrelsome and refusing to leave. SOs Christopher Paz and Ariel Weiland responded to the scene. Upon arrival, we observed the patient laying down on her bed, we approached the discharged patient and spoke with her. Mrs. Harris agreed to leave without further incident. On 02/06/2017, at approximately 1715 hours, while I was conducting meal distribution and ID Headcount in Durango Jail, 3225 W. Gibson Lane, Phoenix, AZ 85009, Housing Unit D-3, Pod D. When Inmate Peterson, Jordan T324870, cell 57-2 received his meal tray, I noticed he had dried blood stains on his shirt, his left eye, left cheek and left corner of his lip were bruised and swollen. At that time I asked the inmate what happened and he replied, "It’s nothing." I told him to step out of the pod and have a seat in the multipurpose room of the housing unit.
Upon my arrival as I approached Cell 15 I noticed large amounts of blood on the floor inside the cell as well as the words "I'm sorry" smeared in blood on the back wall of the cell. Inmate Whitehead, Richard MCSO Booking Number T337608 was lying on his bed unresponsive. Moments later I entered cell 15 with Officer Shoemaker B3008 to help remove Inmate Whitehead from his cell and place him on a gurney by lifting him using the sheet from his mattress underneath him. Castillo kept acting in belligerent manner and stating that he was going to leave. I, Steven Evans then spoke to him about the fact that he appeared to be intoxicated and that he could not leave at this time. I reiterated to him that any attempts at leaving would not work and he could possibly end up being restrained by medical staff with Security assistance. The patient did not like what I said to him, so he threatened to leave and then proceeded to step out of his room. Security Officer Alonso and I asked him twice to go back to his bed, at which time he became unreceptive and refusing to go back to his room. Officer Alonso and I had to physically and forcefully direct him back to his bed. Once on the bed, he became physically aggressive and attempted to hit Officer Alonso. We instantly took control of the patient's arms and upper body while Security Officers Paz and Weiland controlled his legs as he kept screaming and fighting with us. Nurse Baptiste proceeded to contact the patient's Doctor Cleveland so that a sedative could be given to him. On august 13, 2016 I was assigned to follow one of the ICU Nurse. It was a very calm day. She had two patient one was more critical than the other. Both patients were on the ventilator because they had to be intubated the night before. The lady is obese and had gastric bypass surgery two years ago and suffering from severe sleep apnea, but the patient is non-compliance to the CPAP treatment. That was her second time being intubated. She was admitted for seizure monitoring because she was constantly having seizures the day before while she was at home. Due to the fact that she did not want to wear her CPAP machine while in the hospital, after pain medication was administered she was found unresponsive, that was the reason for her intubation the night before. Patient was on intermittent suctioning, she has sinus tachycardia . I had the opportunity to observe some of her daily care. The patient was on fentanyl but when the Dr. try to wean her out of the ventilator she stop breathing, therefore, the DR. discontinue the fentanyl temporarily in other to retest her later.
