While on patrol, I was parked in the entrance to Mansard's plaza, facing E Ridge Rd, monitoring traffic. A female in a silver pickup truck pulled into the lot and flagged me down. The female pointed towards a white and maroon Corvette traveling east on Ridge Rd and stated that she believes the driver may be intoxicated. I immediately pulled onto Ridge Rd and got behind the Corvette. As I was following the Corvette east on Ridge, another vehicle with a male driver pulled up to the side of me and shouted out the window, "That guy is drunk". Orueta stated that he had not. I asked Orueta where he was coming from and going to. Orueta stated he was coming from a buddies house in East Chicago and was now heading home to Schererville. I Observed Orueta to have a leg brace on his left leg and I observed crutches sitting in the passenger seat. I asked Orueta if he is ableto walk. Orueta stated he was not. Due to the unsafe conditions (two-lane road with no room on shoulders, and low light) where I had Orueta stopped and his medical conditions I did not attempt to have Orueta perform standardized field sobriety tests. I asked Orueta to submit to a portable breath test. Orueta stated he would comply. Orueta failed to provide a valid sample on the first attempt. Orueta stated he would submit to a chemical test. I helped Orueta out of his vehicle and assisted him into unit 65. I then transported Orueta to the Griffith police station. At the Griffith police station, I checked Orueta's mouth of any foreign objects. Orueta was then observed in excess the mandatory fifteen minute waiting period, starting at 10:07 Pm. I then moved Orueta into the booking room (location of Intox II EC/IR machine) and read him the Indiana implied consent from a printed card. Orueta again stated he would submit to a chemical test. Newland provided four insufficient breath samples. This officer again read Newland the Indiana Implied Consent and she agreed to a blood draw. This officer completed the necessary paperwork and transported Newland to Southlake Methodist Hospital for medical clearance and a blood draw.
At 0216 hours on Thursday, June 25, 2015 your AFFIANT was on patrol in the area of E. Lloyd St. and N. Emerick St. While at a standing position at the stop sign your AFFIANT observed a blue Chevrolet Cavalier traveling North on N. Emerick St. After the Cavalier passed your AFFIANT could smell a strong odor of marijuana. Your AFFIANT mentioned it to Captain Michael ONEILL about it. ONEILL stated “I can smell it to”.
On 6-5-2016 Edwin Roger was stop for speeding (51/35). Upon speaking with Edwin an odor of an alcoholic beverage was detected from his breath. Edwin agreed to conduct a field sobriety test. There were several indicators during the Field sobriety test to lead me to believe Edwin had consumed an alcoholic beverage. Edwin admitted to consuming some alcoholic beverages before operating a vehicle. Edwin was transported to Madison Township police department to preform the BAC test. Edwin's test results were .153. Edwin was arrested for 4511.19A(1)A and 4511.A(1)D. Edwin was transported to Lake county jail and given a court date of 6-7-2016.
On 5/10/2016, Ms. Francazio submitted urine to TASC for a drug screen. The results came back positive for alcohol. 05/19/16, RP briefed Supervisor Hume on the case. Approval was given by Supervisor Hume to arrest Ms. Francazio. RP briefed Officer Holmes and Lehnert on the arrest. Ms. Francazio reported to Probation and Parole as directed. Officer Holmes handcuffed Ms. Francazio. Also, Officer Holmes searched Ms. Francazio. She was in possession of car keys and cell phone. Ms. Francazio contacted John Pleaston (302) 382-9869 to pick up her car. Please note, he does not have a valid driver license. He stated that he will bring someone to pick up the car. McManus had red glassy blood shot eyes, constricted pupils and a low raspy speech. I requested McManus to exit the vehicle and submit to a series of psychophysical divided attention test some of which he could not successfully complete. McManus placed in the rear of my patrol vehicle and I requested him to submit to a drug evaluation at the Shenandoah Police Department which he agreed on.
In summary, on 09/26/16 at 2220 hours, I assisted Ofc. Drakulich #255 at 3113 S 54th Ave. in regards to a traffic stop. Upon arrival, I made contact with the driver (Lopez, Cristian D.O.B. 08/09/93) of the vehicle (14' Dodge IL IL E822075). I observed signs of impairment on Cristian, he had red, glassy, and bloodshot eyes. While I spoke with Cristian, he had a strong odor of an alcoholic beverage and cannabis emanating from his breath and person. Cristian related that he smoked cannabis earlier, but was unable to provide an exact time. Cristian would repeatedly give the response of "yes sir" or "no sir" without being asked any questions.
COLIN was requested and agreed to submit to a chemical test of blood. I read the chemical warning and report of refusal form (DL-26) to COLIN before leaving the Shenandoah Police Department and requested him to sign the form acknowledging he has been advised of the form to which he did. I then transported him to the Schuylkill Medical Center South
I Ofc Lazic responded to 303 E Manor Dr to assist Ofc A. Sekula #74 for a subject passed out inside the vehicle. The subject was later identified as Andrew Dobosz by officers’ previous encounters with him. The vehicle was parked backwards in the driveway of 303 E Manor Dr. I walked around the vehicle to position myself in a tactical area for officer safety. Andrew was already out of the vehicle and standing by the driver’s door with it being open. This officer noticed Andrew was dazed, confused and appeared to be intoxicated on an unknown drug.
I read Eric the Warning to Motorist from a pre-printed form at 1840 hours. Eric agreed to provide a urine sample at 1845 hour, which I was present for the voiding of the bladder. The primarily result were positive for opiates. The urine sample was inventoried to be sent to the State of Illinois Crime Lab. He agreed to perform a breath test, which I administered 1905 hours, I am a certified breath operator and the results were (.000).
R/s Michael Alexander is a severe alcoholic and suffering from cirrhosis of the liver. R/s Mr. Alexander can’t walk. R/s within one week Mr. Alexander has lost his mobility. R/s Mr. Alexander is refusing medical attention. R/s Mr. Alexander has been without food for one week and he isn’t using the bathroom. R/s Mr. Alexander has no water in the home.
Officers responded to a noise complaint on Wednesday, Jan 19 at approximately 9:30 p.m. the location was 932 Garnavillo Ave, McGregor Ia. The ranking officer responding was officer Jason Bogdonovich. Officers administered breathalyzer tests to 18 individuals, all of whom were visibly intoxicated and under the legal age to drink, all breathalyzed tested positive for alcohol. Four of those underage drinkers were over the age of 18, three are students from MFL MarMac Highschool.
A preliminary breath test indicated a blood alcohol content level of at least .08 percent
The accused was escorted into the booking room where he was removed from the cuffs and sat on the bench. The twenty minute (20) observation was then started at 17:35 hours. Melvin was read his Miranda Rights and the N.J. Attorney’s General Standard Statement for Motor Vehicle Operators form. Melvin agrees to providing samples of his breath for the purpose of chemical testing. After the observation period had expired, he was guided over to the Alcotest Machine. All electronics were removed from the area for the testing process. Operator William Rogers #605 of Wenonah PD then administered the test. Melvin provided two (2) good samples of his breath as instructed. Melvin’s reported breath test result was 0.00% BAC. I entered the results into the Pinizzotto calculator and the results showed a “true reading.” I then reapplied the handcuffs and transported Melvin back to Deptford’s PD for the booking process at 18:10
While on patrol, I was dispatched to a motor vehicle accident. Upon arrival, I observed both vehicles in the roadway. Both drivers claimed they wasn’t hurt and the accident was moved to Fastenal parking lot. I then asked for both drivers insurance and license. Upon asking for Patrick Harold Sweany license, I smelled a strong odor of alcohol coming from his expelled breath. I then asked Sweany if he had anything to drink and he said, "yes, about 3 hours ago." The other driver was then released while I performed field sobriety test on Sweany. I asked Sweany to perform the folling test: HGN, Walk & Turn and Finger Count Test
On the 12 of March of 2016 I went to McAllen International Museum of Arts & Science (IMAS) to attend an exposition about the brain. The exposition covered the entire museum, especially one big room where there were many interactive activities. Although the room was full of people (mostly kids), I had the opportunity to experience some of the activities. These activities were all related to the brain and the way it reacted to certain situations.
