Module Assessment The aim of this assignment is to explore the four stages of APIE, explaining their importance in nursing, as well as identifying possible problems within the stages, in relation to the videos of Joe. These issues will then be anaylsed using theory, to create possible explanations and consequences for the behaviour and actions shown by Joe and the nurses. APIE is a nursing process which guides health professionals through the problem solving approach, which promotes the individualised, holistic delivery of care. It is tailored around the patient’s needs and allows nurses to holistically assess the patient, then plan and set goals according to the information gathered. Stage Two autonomy is also undermined as he is not given the independence of choosing him own meals or choosing what time he wakes up or eats breakfast. This shows that Joe was not assessed in accordance with his age. To rectify this problem, Joe needs to be made more autonomous, given encouragement to use the urinal instead of a catheter and allowing him to make his own choices. The government White Paper (2006) focuses on the need for individualised care and calls for service to be tailored around the needs of the individual and not the service provider. It aims to place the individual in control of their life and promote independence, by providing a more flexible service, with a view to a reaching and fulfilling a healthy old age. In addition, violence, stress and abuse which pose a threat to an individual’s overall well being, must be identified and addressed. The nurses did not view Joe as an individual because they did not give him the opportunity to choose what he wanted to eat, or allow him to get dressed before leaving his room. To adopt a more individualised approach, the nurses should have let him choose what tine to get up in a morning, allowing him to get dressed and further choose what he wanted to eat for breakfast, therefore adhering to The White Paper guidelines.
This presentation is going to talk about person- centred care, confidentiality, respecting privacy and dignity and protecting from risks and harm. The common core principles are important to every Health and Social Care setting as they provide a basis for a general understanding of promoting good mental health and recognising signs of poor mental health among everyone receiving care and support. The aim of care home are to meet the identified needs of individuals who live in that home. An older person might need to live in that care home for years. It is important for staff to be aware of responsibilities in delivering care to support the individuals who live at that home. These common principles of health will help develop the workforce that respond confidently to the individuals and supporting the life they are leading.
Two types of data were collected through surveys, both before and after implementation of the combined approach (Sand-Jecklin and Sherman,2014). The first data was on nurses’ point of view with regards reporting process, and the second on patients view regrading nursing care. The baseline survey included 233 patients and 148 nurses, while the survey three months into the implementation period included 157 patients and 98 nurses. The final survey, 13 months into the impanation, was completed by 154 patients and 54 nurses. The patient survey also included responses from patient families. These were 70, 72, and 53 responses for baseline survey, three-month postimplementation surveys, and 13-month postimplementation surveys.
As healthcare professionals, nurses are governed by and must have understanding of ethical, legal and professional frameworks which underpin practice (Gallagher and Hodge, 2012). They follow these frameworks to help direct themselves in making decisions in collaboration with service users to ensure person centred care is delivered.
A problem solving approach is a process of planning care for a patient (McFerran and Martin, 2003) using ASPIRE, based on APIE (Yura and Walsh, 1988); Assessment, Plan, Implementation and Evaluation. ASPIRE is a nursing process which includes six stages: assess, a systematic nursing diagnosis, plan, implement, recheck and evaluate (Barrett et al, 2012). Using a range of assessment tools, some potential risks can be identified although, these tools should be used as guidance for treatment and not used in the decision making process as to who and who does not receive treatment, (National Institute for Health and Care Excellence (NICE), 2011) but through gaining consent during each process, as it should be before any care or treatment (NMC, 2008).
Nursing process is a systematic process that involves a continuous cycle of five interrelated phases: holistic assessment of a client, nursing diagnoses, nursing care planning, implementation, and evaluation (Wilkinson et al. 2015). It enables nurses to assess the person’s health status and health care needs, to create plans to meet the identified needs, and to provide and evaluate individualised nursing interventions according to the person’s needs (Luxford 2015). The holistic assessment is the first step of the nursing process that includes the collection of subjective and objective data related to the physical, psychological, social, developmental, cultural, and spiritual status of a client (Wilkinson et al. 2015). This comprehensive approach to nursing assessment is essential because it allows nurses to comprehend not only clients’ health status, but also their routines and needs in order to incorporate their life-styles into the care interventions (Luxford 2015). It ultimately enables nurses to provide appropriate quality person-centred care rather than nurse-initiated care (Luxford 2015). Responsibility for holistic nursing assessment is supported by the Registered nurse standards for practice (2016), ‘Standard 4.1: The registered nurse conducts assessments that are holistic as well as culturally appropriate’ (Nursing and Midwifery Board of Australia [NMBA] 2016, p. 4). This piece of work will be based on the pre-assessment process that patients go through on arrival to an endoscopy unit in which I was placed in during my second year studying Adult diploma Nursing. I am pleased to state that I have been a registered nurse for forty years. At this moment, I continue working in nursing while pursuing my Bachelor of Science in Nursing degree. My intended month of completion is October 2012. My experience in nursing education has motivated me to continue my education and pursue a Masters degree in a nursing related field commencing in 2013. Thus, in terms of my career planning, my education is a crucial part to my professional development. Extending and deepening one's education, especially within the career field of one's choice, can only bring upon additional professional opportunities as well as opportunities for personal growth. My career planning includes building upon my decades of experience in obstetrics, my particular area of specialty, expertise, and passion, with the assistance and addition of higher education. I believe in terms of career planning, my strengths include foresight, preparation, internal motivation, and focus.
The national league for nurses defines critical thinking in the nursing process as “a discipline specific, reflective reasoning process that guides a nurse in generating, implementing, and evaluating approaches for dealing with client care and professional concerns” (Kozier, 2008). This definition is imperative to help a nursing student learn how to think in terms of nursing care. Nursing students must achieve a comprehensive understanding of critical thinking in order to understand the nursing process. Lily was a 65 year old lady with stage 5 CKD, she had recently begun hemodialysis treatment three times a week as an inpatient and had been responding well to treatment. During dialysis treatment on the morning of the first day, Lily’s observations showed that she was: tachycardic, hypotensive, tachypnoeaic, had an oxygen saturation level of 88% and was becoming confused and drowsy. It became apparent that Lily had become hypovolaemic. Patient’s decisions may sometimes be affected by various factors i.e. Their surroundings, they are often vulnerable and out of their normal environment. A conventional health care setting may be in a hospital environment where patients are most likely feel sensitive and insecure. Matiti and Trorey (2008:17) conducted interviews involving 102 patients in 3 different hospitals across the United Kingdom, over a period of 18 months. The purpose of these encounters was to annotate what was being said and also the fundamental meaning of how patients believed that their dignity was being put in jeopardy. One of the key aspects of patient dignity is making choices. Whilst conducting these interviews Matiti and Trorey (2008:17) discovered that despite the fact that patients accepted the loss of a little independence within the hospital surroundings, they wanted the freedom to participate in the decisions about their healthcare. Patients’ understanding in terms of choice, authority or participation in care and autonomy varied extensively. When it comes to assessing the cognitive intact patient it can be a bit challenging for sure. Prior to assessing the patient you must do your part and assess their past medical history as well as language barriers prior to assessing them. The patient may have comordities and or don’t understand English very which in turn makes the assessment a bit more complex. You also have to consider their home environment and who they live with because that can play a role as well. As many of the students mentioned a lot of them tend to live home alone and do not have much social contact with others so they tend to want to talk more.
Nurses play a variety of roles while providing patient care. These roles vary between caregiver, communicator, and teacher. According to the American Association of Colleges of Nursing, nurses play a critical role in protecting patient safety and providing quality healthcare to patients (Hallmarks of Quality and Patient Safety, n.d.). Nursing process discipline is a nursing theory developed by nursing theorist, Ida Jean Orlando. This theory, one of the first written about the nursing process, was written to help establish nursing as an independent function in providing health care for a patient. Through this independent nursing function, Orlando developed her theory on the concept of the nurse-patient interaction.
