Theory This research question is well tied to Pender’s Health Promotion Model. The research question will highlight in some ways activities that individual do on their own to care for their disease, such as diet, regular monitoring of blood sugar levels and following up with their health care providers as recommended through their plan of care (Polit & Beck, 2012). It also will show emphasis of the health promotion model, incorporating the health promoting behaviors through modification of their prior behaviors to ones that will allow them to live a somewhat healthier life with their new diagnosis. In the same token, the patients will have to have conscious awareness of the benefits of this change in their lifestyle, identify the barriers that can hinder the change from happening and know what benefits they will get out of it so they can enable themselves to commit to more health-promoting behaviors (Polit & Beck, 2012). This model can help nurses empower their patient to advocate for their health, to understand that they have to coexist with their environment (interpersonal and situational barriers) and yet still able to learn to make wise choice, especially when it comes to health, and be able to relate priorities (Ho, Berggren, & Dahlborg-Lyckhage, 2010). I believe the most important aspect to successful implement evidence-based practice is a common understanding and stress of the importance of EBP among health care professionals. Everyone need to be on the same page and understand that practice methods that are not backed by evidence are no longer acceptable. I believe the facility needs to have a mandatory meeting with all the health care professionals and discuss how EBP will be implemented into their facility. I also believe that there should be repercussions if EBP is not put into place.
The process of using Evidence Based Practice in a patients care plan consists of five key stages. Asses the patient and formulate the problems from this you will then need to access the relevant clinical articles. Using this information you should then be able to assess which is the best method of treatment discarding any misleading or ambiguous articles. You then need to incorporate this knowledge into the patients care plan. Finally you need to evaluate and assess the patient during treatment to ensure effectiveness.
Evidence-based practice is an approach used by health care professionals to continually use current best evidence-based research to make ethical and reliable decisions regarding patient care. “Research to promote evidence-based practice is becoming more and more a part of the regular work of health care leaders” (Grand Canyon University, 2015, p. 1). However, it is important to determine the difference between solid research and flawed research that provides unreliable inferences. Evidence-based research includes focusing on a clinical question; and includes the review and incorporation of several studies to strengthen the results of the new study (Grand Canyon University, 2015). Roddy et al. and Ganz et al. articles will be assessed to determine if the recommended changes were backed by solid research that warrants changes in a hospital.
The appropriate approach for this health promotion is through an educational approach. With this type of approach, it provides knowledge as well as helps the people to gain or developed necessary skills to guide them in making an informed decision about their health behaviour (Lucas & Lloyds, 2005). This approach does not set out to persuade or motivate change in a person’s decision-making process; the promoters are simply acting as a guide in educating the people about the disease

Evidence-based practice is the practice of making clinical decisions based off the best available research evidence coupled with the nurse’s own expertise, while also taking into account, the patient’s assessments and own personal preferences. This use of research has proven effective at providing better outcomes and lower healthcare costs, yet there are several barriers, such as time, education, and support, which prevent nurses from consistently using evidence-based practice (AJN, 2012). The top three barriers to the use of evidence-based practice are lack of time, education, and support in implementing new practices and using them consistently.
Nurses implement health promotion strategies in a variety of ways. Due to the high level of credibility nurse’s have with their patients, they greatly influence their patients with their passion. By emphasizing health promotion strategies such as self-responsibility, proper nutrition, exercise and stress management, nurses plant the seeds of wellness in the minds of their patients. While ultimately, it is up to the individual to make healthy changes in their lives, the role of the nurse practitioner plays a huge role in motivating individuals to adopt healthy habits.

Often in practice, we as nurses deal with a variety of diseases and treatments and often have to react to the illness that the patient presents with upon our interaction. While this is an essential piece of our practice, we also have a duty to our patients to be proactive in preventing specific health-related consequences based on their risk factors and to promote their health and well being. Health promotion as it relates to nursing is about us empowering our patients to increase their control over their lives and well beings and includes: focusing on their health not just illness, empowering our patients, recognizing that health involves many dimensions and is also effected by factors outside of their control (Whitehead et al. 2008)..
Evidence based practice is the basis for needed change in practice and function. It is a sound method for scientific, fact-based change. Changes which have no evidence to support them are fragile, unscientific, and subjective. These changes don’t effect real change over time, as they aren’t able to be proven to a more general population.
Itroduction: Evidence-based practice is an approach to medicine that uses scientific evidence to determine the best practice (Beyea & Slattery, 2006). As nurses perform their daily tasks they must continually ask themselves, “What is the evidence for this intervention?”. Nurses are well positioned to question current nursing practices and use evidence to make care more effective. In order to improve patients’ outcomes it is the responsibility of the nurse to transition evidence-based practice into the norm, through application of daily practice (Flynn Makic, Rauen, Watson & Will Poteet, 2014). Continual evaluation of current practice must be performed to ensure the use of evidence-based practice opposed to practice based upon tradition. The implementation of evidence-based practice standardizes healthcare practices and diminishes groundless variations within care. These variations lead to the production of uncertain health outcomes (Stevens, 2013).
Evidence-based practice will be extremely helpful among the hospital setting because it will allow for me to learn from reliable resources. Using evidence-based practice thus far has helped for me to learn about different cultures and become more prepared when meeting with patients. I will continue to work within the hospital setting and need to be sensitive of how different cultures feel about the medical system as well as treatment options. At times, I have found myself wondering why my patient does not want to go a certain treatment route and many times I can use evidence-based practice to get a better understanding. Many times evidence-based practice has allowed for me to share articles with coworkers and have dependable evidence to fall back on when advocating for my patient within team meetings.
