Module 3 Worksheet – Assessing a Community
Name: Adedotun Taiwo
Please read the assigned chapters in
Community and Public Health Nursing (

rd
ed
.)
and use the
information to answer the following questions.
1. Describe the place where you will conduct your Windshield Survey (Demarco & Healey-Walsh,
pp. 232-233). Include the geographic location, boundaries, and name of the neighborhood or
area.
The windshield survey will be done in Cincinnati's Reading neighborhood. The survey
area includes the section between Clifton and Reading Road. The Cincinnati Zoo, The University
of Cincinnati, the College of Medicine, and the University of Cincinnati Medical Center are
among the commercial, residential, and academic landmarks on the site. Because of the nature of
the economic, sports, arts, culture, and literary environments, the region is densely populated.
The University of Cincinnati Medical Center, The Children's Hospital, The University of
Cincinnati Research Facility and University, as well as the College of Music and the
Conservatory, are all located in the vicinity. The University of Cincinnati Health Medical Center
is in the Uptown, in its surroundings are also The Good Samaritan Hospital and TriHealth
Medical Center. To the East is Vine Street and Reading Road, to the North is the Cincinnati Zoo,
to the South is Martin Luther King Drive, and to the West is Burnet Avenue.
2. List and describe the steps you will need to take in conducting a community assessment (Demarco
& Healey-Walsh, pp. 233-237). Also, include the varied sources of information you will need to
use. Include the information from the textbook using an in-text citation.
A community assessment entails evaluating the present state of a community, determining
the desired state, and comparing the two to prioritize concerns. A community assessment is
typically conducted to gain a comprehensive understanding of the community and determine the
most effective approach for addressing its concerns.
The community assessment process should include pre-assessment, assessment, and post-
assessment stages. During the pre-assessment phase, it is important to consider the following
questions: What will be the subject of the assessment? What data is required for collection? The
methods and individuals responsible for data collection and analysis are also to be determined.
The community serves as the starting point for a community assessment. In the pre-
assessment phase, one can engage community partners to self-assess their resources and identify
the health issues they consider a priority for intervention. After answering these questions, the
assessment can proceed by following these steps.
The process includes the following steps:
1. Establish the objectives and extent of the evaluation. Determine the scope of the assessment,
whether it will target a specific population within the community or encompass the entire
community. Determine the prioritization of health topics, while considering the feasibility of the
assessment's scope.
2. The assessment's goals and objectives need to be clearly defined.
3. Choose appropriate data collection methods. Determine the presence of desired information or the
necessity for a new data collection endeavor. Common methods for data collection typically
involve surveys, interviews, focus groups, and observation.
4. Choose or create the appropriate tools and methodologies for gathering data. Search for validated
and reliable instruments that have undergone testing in comparable communities and with similar
populations. Adapting an existing instrument is more efficient and cost-effective compared to
developing a new one.
5. Conduct a pilot test of the instruments and procedures.
6. Create a timeline and budget.
7. Gather the necessary data. Data may be established using a Geographic Information System,
mappings such as internet platforms, Google maps, hospital records, and data from past research
found in medical publications. Choosing the best community assessment tools will require
deciding whether to utilize digital, offline, or a combination of the two.
8. Conduct a data analysis.
9. Compile and distribute a report summarizing the findings.
10. Assess the value and significance of the assessment. The evaluation should include an analysis of
successful aspects, encountered challenges, and potential areas for improvement.
3. Select 4 websites listed under "Module 4 - Resources for Community Assessment" and provide an
example of how to utilize each one of the four sites to obtain information for a community
assessment.
The Centre for Disease Control (CDC) website, located at http://www.cdc.gov, provides
statistics on the prevalence of diseases in various communities. The CDC is at the forefront of
protecting public health and safety. Their website provides a wealth of knowledge useful for
statistics, progress, programs, challenges, and improvements toward public health and safety.
Research programs and information are also accessible on the CDC website.

PLEASE INCLUDE ALL SOLUTIONS.Rain gauge X was out of operation for a month during which there was a storm. The rainfall amounts at three (3) adjacent stations, A, B, and C were 4.2, 3.7, and 4.9 in., respectively. The average annual precipitation amounts for the gauges are X = 36.5 in, A = 42.1 in, B = 37.1 in., and C = 39.8 in. If plotted in an x-y plane (unit of miles) with gauge X in the origin, the coordinates of other stations are as follows: A: (3,7); B: (4,6); and C: (5,9). Estimate the missing rainfall data of Gauge X.

A vertical bar screen is constructed of 20-mm diameter bars that have a clear spacing of 25 mm. Determine the headloss and velocity through a clean screen when the channel approach velocity is 1.0 m/s. For the previous, we assume that the bar screen is at 90o angle from the horizontal. If the screen is inclined at a 50, 55, and 60° angle from the horizontal, how will the headloss and velocity through the clean screen be impacted? Hint: using a spreadsheet will allow you to quickly calculate the headloss and velocity at different angles.
