1 Jaden’s Treatment Plan Abnormal Psychology- O2 Prof. Rosenberg Catherine Leal November 21, 2022
2 Client The client Jaden Harris is a 22-year-old Caucasian male that is presenting at 5”10 and 182 lbs. Jaden is fully oriented x3 with good hygiene. He lives in a private house with his father and a younger sister. He reports that his father and sister get along well, but struggle to pay the bills each month, which in return causes tension and concern. This is ongoing as reported by the client. Jaden recently moved home six months ago after graduating from college with a computer science degree. He currently works as an administrative assistant at a vet’s office to generate income. Jaden has been unable to find a job that fits into his field of computer science, as applications have not led to interviews. Concerns and Problems Jaden states that dogs at work tell him “secrets” which shows he experiences auditory hallucinations. He reports paranoia as he states that the FBI is “watching him” in order to collect his student loans. When it comes to his occupation he demonstrates a feeling of inadequacy and inferiority due to his inability to obtain a job in his field. Jaden states a feeling of constant worrying and anxiety in his life. Supports and Strengths Jaden was able to accomplish and obtain a degree in computer science. He is able to maintain a job, even though it is not in his field, to generate an independent income for himself. He continues to apply to jobs in his field, even when denied, showing ambition and determination. He maintains good hygiene as well as a good relationship with both his father and younger sister. He listened to his father’s request for him to receive clinical assistance which demonstrates he is open-minded. Jaden was able to identify an issue he wished to work on which
3 shows his desire to obtain help. The client is able to keep friendships from college as well as maintain the connection by seeing them from time to time. He has goals for the future as he wishes to one day marry and start a family. Diagnosis - Primary diagnosis: Auditory hallucinations and paranoia - Secondary diagnosis: Anxiety - Borderline personality disorder, or possible schizophrenia - Occupational problems - Problems related to the social environment - GAF rating: 54 Diagnostic rationale The primary diagnosis is auditory hallucinations and paranoia as that is the reason Jaden is seeking clinical assistance. For the secondary diagnosis of anxiety, Jaden stated he wished to work on his “freakouts” and “constant worrying” but was not the main reason he was recommended to get clinical assistance. The diagnosis for a personality is borderline personality disorder as Jaden suffers from anxiety freakouts that have no discernible triggers. He experiences anxiety and seemingly depression as he has feelings of worthlessness and undesirable. When at work he has psychotic experiences which involves hearing the animals tell him “secrets”. Jaden also demonstrates paranoia. These symptoms together are in the criteria for BPD. It also seems that Jaden may have schizophrenia, possibly in the prodromal phase, but more monitoring should be done. The psychosocial stressors are occupational problems and problems relates to the social environment. The occupational problems that the client experiences is being unable to find a job

4 within his field. The stressor in his occupation also comes from applying to jobs but being denied interviews. The problems relating to the social environment have to do with his transition from college life to the real world. This transition is particularly stressful as it is a major shift in life and maturity as expectations increase. The rating of a 54 is due to Jaden still being functioning as he has his own job and maintains relationships but suffers from anxiety attacks as well as hallucinations and delusions. Treatment recommendations An antipsychotic medication should be recommended to Jaden in order to treat and reduce the hallucinations that he is experiencing at work. The type of antipsychotic recommended for Jaden’s treatment would be the atypical antipsychotic risperidone. Risperidone is a second generation antipsychotic, it is recommended because it rebalances dopamine and serotonin to improve thinking, mood, behavior as well as helping to alleviate hallucinations as well as delusions. For psychotherapy treatment a cognitive-behavioral approach would be recommended to go along with the medication. A supplemental family therapy would be helpful as the client lives with his father and sister, so the client feels supported by his family. For treatment the medication and its effects should be monitored, and therapy should be regular. The family therapy should be once to twice a month but not as frequent as the psychotherapy. Reassessment of this treatment plan should be in three months in order to check how the medication is affecting the client as well as if the clients symptoms are improving or worsening. The prognosis for Jaden is good as it appears that the symptoms are not severe yet as he is still able to maintain relationships and work, the intervention was early and not at a late point where it would have been more severe.
