She had two loose large stools the first day and now her mother reports that she has been less active, is not interested in playing, and has been sleepier today. She is unable to keep any feedings down today. She has had four loose watery stools and emesis three times this morning. She has not had a wet diaper since yesterday evening. She is 25 inches in length and weighs 6.4 kg. She weighed 6.88 kg at her last office visit two weeks ago. The patient was directly admitted to the pediatric unit where you are the nurse responsible for her care. Patient's mother is 21-year-old and is a single mom but gets support from her family, though she feels consistently overwhelmed as a new mother. Based on the patients past medical history, it was known that patient has not had any immunizations from birth including Rotavirus vaccine. Upon physical examination, temperature is 39.0 deg C thru the axilla, pulse rate is 158 bpm, respiration is at 38 cpm, blood pressure of 62/42 mmHg, and an oxygen saturation of 95%. The patient is irritable when awake and has no tears whenever crying. Her skin is pale, cool to touch and a capillary refill of 3-4 seconds in both hands.
