5. The patient is a very pleasant 67-year-old female who comes into the
hospital after waking up with shortness of breath. She recently visited the
emergency department 3 weeks ago and was sent home on azithromycin
and a prednisone. She says that she was improving, and she forgot to take
her inhalers daily. She states that she has a chronic cough that goes on and
off throughout the day. Her sputum is thick yellowish. She denies chest
pain, fever, chills, or night sweats. She has no lower extremity edema.
Blood Pressure 142/74, Pulse 92, Respiration 14, Temperature 98.6. oxygen
saturation 93%. Crackles heard in bilateral lower base of lungs; heart
sounds S1, S2 present and regular, bowel sounds present.
