On Wednesday, December 16, 2015, at approximately 2:15 p.m., I, Investigator Torres, traveled to meet with Human Resources Manager, Ms. Sylvia Yu at The Hat Restaurant located at 2300 South Atlantic Boulevard, Monterey Park, CA 91754. She refused to provide a recorded statement and refused to sign a confidential waiver in regards to providing a statement of this claim involving Claimant Diana Guzman. Ms. Yu stated that the claimant started her employment with The Hat 9 years ago, but she did not have the claimant’s exact date of hire. She referred us to the Claims Adjuster handling this Workers’ Compensation claim that would have this information. Ms. Yu was not aware that the claimant had ever reported any injuries to the Human Resources Department or Ms. Celina Larios, the claimant’s immediate supervisor at The Hat Restaurant in Monterey Park, CA. Yu stated that there were no injuries ever reported to her or to anyone else from any witnesses that came forward to report that the claimant was injured at work. Ms. Yu claimed during her interview that she had no information to give as to the claimant’s evaluation, attendance and her past employment history. Furthermore, she had no information about the claimant’s medical history, injuries at previous employers, or any other work related injuries with this employer. Ms. Yu’s appeared nervous and apprehensive to offer any information about the claimant’s alleged claim as she repeatedly would say “everything is inside the claimant’s personnel folder.” Ms. Yu said she confused about why Amtrust North America had not turned over the claimant’s personnel file for review before this investigation started. She insisted that we should have reviewed the file before commencing the investigation. Ms. Juanita Machado is a Line Assembly Technician. She was employed with the insured approximately for seven months. Within the seven months, she has been with the company; she had known of the claimant, Mr. Donald Arauz after he had been hired early on last year in 2016 when he was hired to the same position that she currently holds.
On 11/4/16, I, Account Manager Enmanuel Cabrera, was notified via email by Shift Supervisor Massiel Perez that Security Officer Bailey Forte no called no showed. S/O Forte is usually off on Fridays, but was scheduled in advance, 0200-0600, to cover the open post. S/S Perez made several attempts to call the officers from 1st shift to cover the open post, but was unsuccessful to get someone to come in. Also on 11/3, during the afternoon parking lot detail AVP1 was 1 flagger short, all vital positions where filled with the exception of the parking lot supervisor/rover. As well as on 11/4, morning parking lot detail was short 1 flagger. Daniel Canela no call no show for the AM parking lot detail. All vital positions in the parking lot were filled for the exception of the supervisor/rover position.
SC received phone call from provider Moravia home health care Sherri Hill RN. Requesting that the SC call the provider to discuss the Pa’s scare plan. SC called the provider and spoke with Sherri Hill. Sherri reported that the Pa call the office on 3/21/2016 and reported that aide/ niece Iysha Gayman did not show up for her evening shift and does not want her to work for him anymore. Sherri reported that she called Iysha to enquire about the why she did not show and informed her that the Pa does not want her to continue to work for him. The Nurse reported that Iysha became upset and used disrespectful language at that time Sherri reportedly end the call with Iysha. Asia Walker an investigator for the Guardian law office met with Ms. Rojas, Mr.Perez and Aleena. She inspected the apartment and asked the family questions about Aleena's health, development and things they may need to better care for Aleena. Ms. Walker offered the family some advise on how to deal with DCP&P, when it comes to getting times done in a timely manner. She told Ms. Rojas , if she really feels like her case work is not doing her job than to contact her supervisor. Ms. Walk spent an hour explain her job title, the rights Aleena have and the rights of the parents.
GAO called Ms Lisa Everett. She was inquiring if her daughter, CAITLYNN, was removed from her case. She stated that received a letter from APD informing something about her SNAP, but did not understand if it was about her daughter been removed.
This role play was on November 9th, 2016 at 11:45am and Evan and I were dispatched to 123 Main street in regard to a complaint by Erica Hunt about a man, Jeff, who was hired to clean her apartment. Erica was not happy with Jeff’s cleaning job, as well as, he took a pizza and a beer. From Erica’s perspective she was very upset, and yelling at Jeff, about how she did not want to pay for his cleaning job, and wanted him out of her house. From Jeff’s perspective, he wanted to get paid and would not leave until he was paid. Unfortunately, neither Evan or I received Jeff’s last name, and did not do a CPIC check.
SC received VM from Lisa Woods scheduler at MarsCare on 2/1/2016. The VM stated that she completed her meet and greet with the Pa, his wife, herself and the worker Debbie Harris on 1/28/2016. Lisa reported that the meet and greet went well, but she has some concerns about the poor condition of Pa’s home and expressed hesitation about the aide safety. Lisa stated that the Pa’s kitchen is badly in need of repairs and the ceiling is appears to be falling down. In the VM Lisa asking the SC to verify that the home is safe for the aide to be in it. SC placed call to Lisa but VM and left a detailed message with contact information. SC then called the Pa’s to inquire about the meet and greet and if the aide is coming. On Wednesday July 19. 2017, @ approximately 1:55 pm there was a meeting at 1660 7th St, Santa Monica, CA 90401.The meeting was held in the second floor in conference room. The meeting was in regards to the organizational changes and other information to the supervisors staff. However, it was brought to my attention that I was personally brought up in that meeting without me being present. My coworker asks Rolando Cruz about training for supervisors in the scheduling department for a position called Scheduler. The question was asked if the scheduler position will be biddable in the upcoming bid. Rolando Cruz answered no. He further stated the only person that applied for that position was Lamicia Butler.
