History The Second Republic (1931 to 1936) was an important time in Spain for their healthcare system. The political parties in power at this time supported the introduction of a social insurance system, even though each party had different ideas and political intentions. There were other significant changes during this time like universal suffrage and the establishment of a piece of legislation that allowed for freedom of speech and association. All this movement stopped when the dictator of Spain, Franco, started and was the catalyst for the Spanish Civil War in 1936. Franco remained dictator until his death, and then a shift took place. This covers all different services expect for long-term and dental care and coverage depends on each region. The reason why coverage varies from region to region is because health status and healthcare needs are different due to the demographic and cultural differences. So each region is able to customize their healthcare system as long as they keep within national guidelines. The national health system is financed through taxes and the services are provided by the public sector and then the private sector plays a small role. In 1999, administrative concession was established, which is a new hospital system for management. This system determines what provided services are managed by the private sector, and the public sector manages the property, financing, and control. As an occupational therapist, we can play an astronomical role with Spain’s at-risk population, which are elderly and disabled people in long term care. In the book it stated that they don’t have nearly enough workforce or specialties to help with these populations. Adding occupational therapy as a service is one way to help expand and improve this underdeveloped area within the NHS. We can help this population possibly be more independent so they don’t have to be in long term care and can help train caregivers so these people can live at home longer and they wouldn’t need these services anymore. Also, as occupational therapist, we could help with all populations, not just elderly and people with disabilities. We can work with children and adults of all ages and in all settings to help them gain or keep their independence, in hope that they won’t need as much health care services. If there is less people who need health care services then less money being pooled from taxes and less health
