The Importance Of Professional Communication

Cultural competency is becoming essential in American healthcare with the increasing and diversification of immigration. The Department of Homeland Security, 2014 statistics indicates over 750,000 immigrants received naturalization in the US. These naturalized citizens represent over 20 countries varying from Hispanic, Indian, Asian, African, and Middle Eastern cultures. A change in the way American healthcare treats these cultures is necessitated to provide efficient care and achieve positive outcomes. Douglas et al. (2014) guidelines are to empower patients to help diminish the inequities of their own healthcare. The guidelines for culturally competent care are meant to guide the nurse, nurse educators, and nurse managers in their competent treatment of persons of other cultures. These guidelines mandate a nurse use education, self-assessment and reflection, and diversification of the workforce to meet the demands. The ten guidelines are knowledge of culture, education and training in culturally competent care, critical reflection, cross-cultural communication, culturally competent practice, cultural competence in health care systems and organizations, patient advocacy and empowerment, multicultural workforce, cross-cultural leadership, and evidence-based practice and research.

The increasing population of immigrants in the United States has contributed to health disparities in the health care system. Cultural competence can remove health disparities by eliminating personal biases, and treating every person with respect. Simply recognizing and accepting different cultures is not enough, one must be able to consistently recognize and understand the differences in order to be culturally competent. Knowledge and culturally competent practices are a must for nurses to deliver quality care in our rapidly changing multicultural world (Edelman, 2014 p. 25).

While the density of the Mexican culture will vary in different regions of the United States, the likelihood of having to communicate within it is quite high, especially in the nursing profession. This paper will discuss

In nursing school, we are normally taught that we should respect the dignity and rights of all clients. As the "world becomes reduced" and societies and individuals become more mobile, we are progressively able to network with people that are from other cultures. Cultural respect and competence for others becomes particularly significant for us as nurses and patient supporters. Applying the principles and theories of communication is important for sufficient patient care. A lot of various communication methods are executed and have diverse focuses. Small groups use mechanisms such as objectives, standards, cohesiveness, behaviors, and therapeutic issues. Duty, process and midrange groups are separate categories. Orientation, tension, cohesion, working and dissolution are stages groups go through. Successful personal and professional communication profits the patients and other health professionals; however, the lack of applicable communication can lead to poor patient results and a hostile and fruitless work setting (Doane, 2004). However, the cultural group targeted in this paper is the Native Americans because this group has become a challenge for public health nurses.

Hispanics households with low socioeconomic status and low education coupled with low diabetes awareness have high type 2 diabetes prevalence 9. The high prevalence rates of type 2 diabetes can be attributed to a number of reasons such as; the Latinos have a genetic tendency to develop insulin resistance and they face high risks for abdominal obesity. The study emphasized that the strongest predictors of developing type 2 diabetes in Hispanic population are impaired insulin sensitivity, low insulin secretion and and glucose effectiveness 1. In addition, the prevalence rates are high in poor families who have poor nutrition and lifestyle behaviors. Since poor families have low access to education, they tend to have low awareness for diabetes hence these results in high diabetes prevalent rates. Diabetes is a prevalent health disparity among the Latino population. Diabetes is listed as the fifth leading cause of death among the Latino population in the website for Center for Disease Control and Prevention, CDC, in 2009. According to McBean, “the 2001 prevalence among Hispanics was significantly higher than among blacks.” (2317) In other words among the Hispanic or Latino community, there is a higher occurrence of diabetes as compared to other racial/ethnic groups such as Blacks and Native Americans. The incidences of diabetes are growing annually, and predicted to increase within the coming decades. The number of cases yearly is not only increasing, but the age of onset for diabetes has decreased. This means more individuals are developing diabetes at an earlier age. Diabetes not only affects individuals and communities but countries as well. The prevalence of diabetes has gathered billions of dollars in healthcare costs. Based on the United States census, it is estimated that by the year 2050 one in three people living in the United Sates will be of Hispanic/Latino origin which include sub groups like Puerto Rican, Mexican, Cuban, Central Americans, and South Americans (Heart Association, 2014). Within those subgroups, the prevalence varied for people of Mexican descent from as high of 18.3 percent to as low as 10.3 percent for people of South American descent, Dominicans and Puerto Rican descent 18.1 percent, Central American descent and Cubans descent 13.4 percent all living in the United States with diabetes type 2. On another commentary being published in the same issue of Diabetes Care, the author wrote, “the differences in diabetes and obesity prevalence among Latinos subgroups are marked when all individuals are combined into a single group” (Heart Association, 2014). Diabetes in Latino Americans has become more prevalence with aging, by the time they reach the age of 70 years, 44.3 percent of Latino men age 70 years old to 74 years old will have develop diabetes. The health issues resulting from health disparities present on the Latinos and Hispanics in America are diverse. In the Hispanic community, diabetes is one of the major causes of death and illness. The language barrier, hard access to health and preventive care and cultural barriers are factors that contribute to this disparity. (Ritter & Hoffman, 2010).

The Hispanic population is constantly growing and we have an estimated 13.7% of Hispanics making up the United States. “The annual percentage of patients with prediabetes in whom overt type 2 diabetes develops is about 5% in the general US population and may reach 15% in the Hispanic American population” (Idrogo & Mazze, 2004, para. 7). This a community health issue because of the percentage of Hispanic individuals that may end up with diabetes. These individuals need to be educated to help the promotion, protection, and maintenance of diabetes in this group. That is why I have created the intervention program for this ethnic group.

In rural Iowa, there are few individuals who are knowledgeable of the Hispanic culture due to the small number of Hispanic individuals that have sought out care in the past. Recently, there has been an influx the number of Hispanic Americans seeking care in this area, making the delivery of culturally sensitive communication and care an important topic. According to DeNisco & Barker (2016), the nature of nursing care encompasses the need to be aware of cultural diversity (p. 581). Meaning that nurses need to strive for cultural competence to reach the societies expectations in the delivery of nursing care (DeNisco & Barker, 2016). We are going to take a look at how nurses can make a few changes that will make providing culturally competent care to the Hispanic population easier in these settings.

One of the greatest things about nursing is that we have the opportunity to share with different cultures and learn about them. Our patients are complex; they each have their religion, culture, and life choices. Delivering health advice and not knowing much about a patient’s cultural background will influence how the patient may perceive the nurses’ advice. It is essential when entering health care as a professional nurse to realize that health care workers, doctors, nurses, and other health care providers, form their own culture with their own beliefs and attitudes about the care that’s delivered and the patients whom they serve. Because there are significant barriers to health care for Hispanics, particularly those that are Mexican-American, in order to provide culturally competent care, the professional nurse must implement effective communication, convey respect to the patient, and take a thorough health history from each patient. The purpose of this paper is to discuss the background of the Mexican-American Hispanic patient, compare and contrast their culture to the culture of those that work in health care in the United States, and recommendations for the professional nurse using Purnell’s Cultural Theory for support and a reflection on Wellness Day for Health Promotion focusing on Hispanic heritage including Curanderismo.

With the large increase multicultural population in the United States, nurses encounter patients with differences in healthcare beliefs, values and customs. To provide adequate nursing care, nurses must be aware of these differences. They must respect and acknowledge the patient’s culture. To do this, nurses need education on cultural competence to ensure patient satisfaction and better patient outcomes.

Professional Communication is a very important element in the foundation for a strong health care system. Communication can also serve as a basis for basic health care administered. Communication is not only the one tool in health care in which we can control, but also what helps prevent the derailment in patient-healthcare professional trust.. Types of model that can help express how important professional communication is within health care is the movie titled, The Doctor. There are various examples in this movie in which the importance of professional communication is portrayed and supports the argument, as well. These types of examples and supporting data can also be found in real life testimonies of what type of situations can result from lack of professional communication in the health care setting. Communication teaches the importance in human-to-human contact that some hospitals and health care setting may lack due to the certain personality type that is more attracted to the roles of doctors. Professional communication should remain at the forefront of bed side manner and proper etiquette in the health care field and setting.
