Florence Nightingale gave us 13 canons of the environment. The major canons include ventilation, light, noise, cleanliness of rooms and walls, bed and bedding, personal cleanliness, and taking food. The Dreyfus Model states that in the acquisition and development of a skill, a person goes through five levels of proficiency; from novice to advanced beginner to competent to proficient and finally expert.
The novice has no experience so are taught objective attributes and measureable parameters. They are then taught rules to guide them in working with different attributes. The basic problem is the novice, because of a lack of experience is unable to use discretionary judgment. The advanced beginner has had enough experience to begin to recognize situational components called aspects. The advanced beginner can formulate guidelines for actions in terms of attributes and aspects. But, the advanced beginner treats all attributes and aspects as equally important. They lack the experience to determine whether one aspect is more important than another. They must focus on following the rules they have been taught. When a nurse has a few years of experience he or she begins to view patient care in term of long-term goals or plans. A competent nurse knows a plan establishes perspective based on extensive evaluation of the problem. This contrasts with the earlier stimulus-response level of nursing.
