I assessed 71 years old female, Ms. Smith (fake name), who appeared in good physical health. She was alert and oriented because she was able to tell me the correct date and place. She did not seem to be in any distress. Eyes were symmetrical, sclera was white, and pupils were equal, round, reactive to light, and accommodating, and extraocular movements were intact. No hearing deficit was noted because Ms. Smith was able to repeat the words that I whispered into her each ear. I did not note any breathing difficulties as she was able to breathe through both nares equally, lungs were clear bilaterally upon auscultation, and she denied shortness of breath. No swelling or tenderness was noted on palpitation of lymph nodes. Ms. Smith denied heart pain or palpitation, and heart beats were regular upon auscultation. Extremities were symmetrical bilaterally, and muscle strength was strong upon assessment. …show more content…
Smith scored 15 on balance and 12 on gait assessment which is 27 out of 28, making her fall risk low. She set on the couch without any support but used her hands to stand up. She did not stagger while standing with both feet together and eyes shut for 10 seconds, as well as maintained balance upon nudging. Ms. Smith made a full 360-degree turn without stopping. She had steady gait during walking with appropriate length between both feet, picked up and touched the ground with each foot, and was able to turn back without staggering. She also did not have loose rugs in the house as well as medications stored in the high cabinet in the
D-The patient arrived on time for her session. This writer witnessed the patient at the dosing window. This writer addressed with the patient about her treatment goals, referring to tapering off on the methadone. The patient then requested to decrease her current dose by 1mgs as she wishes to be stabilized on a lower dose. This writer completed the dose change request form and had the patient initial her name on the application as proof that it was done with the patient presence. She stressed that she wants to take it slow and not rush it as she wants her body to adjust to a lower dose each time a dose change request form is submitted, at which this writer agreed with the patient. Furthermore, this writer requested for the patient work schedule
Metacognition is a complex notion that encompasses numerous mental processes in all areas of life. It is often simplistically described as “thinking about thinking.” However, there is no commonly recognized definition of metacognition. In his article “The Power of Reflection”, Stephen Fleming, a well-respected cognitive neuroscientist, agrees with the definition of metacognition given by John Flavell in 1970 as “our ability to evaluate our own thinking” (2). Fleming also accepts Flavell’s proposition that metacognition is “key to educational success”; indeed, the development of metacognitive thinking is vital in learning processes (1). Metacognition is a teachable concept, and has been proven to be crucial in education.
One day I was ask to transfer Mrs X from bed into wheelchair. Mrs X is paralised so to transfer her a full body hoist is needed. I had to call for help, it wouldn’t be safe to do this by myself. So while I was waiting for the other carer to arrive I have explained to Mrs X what and how we are going to do it. I have also cheked the hoist and battery if its fully charged and operational. I put right size sling on Mrs X with her cooperation. I carefully manovered hoist close to bed, then I hooked sling onto the hoist with short hooks on to of the body and long hooks on bottom. That was when my collegue arrived to help me with transfer. We asked Mrs X if she is ready when she said yes by pushing

During the course of this class I have been introduced to the different disciplines of writing and researching. The separate styles of documentation are the social sciences, humanities, and history. Each style and discipline call for its own structure and specific style of writing. The journey and education received through this class can be construed as an event created of multiple emotions. To be frank, a love and hate relationship was formed among the modules assigned throughout this class. The articles and stories provided were wonderful to read, learning how to construct the information into each corresponding style however, was a bit frustrating.
This week during my supervision with my field instructor and I started our discussion, talking about making up my hours, due to an accident I was unable to attend one day I was scheduled . My field instructor stated, making up the hours would not be an issue, especially with the instructor working on Saturday's. We discussing in more detail about the agency's policy/operating procedure, as well as the following: billing, client eligibility restrictions, dictation of client's notes, etc. I will learn more in detail and hands on regarding the various intricacies of the agency as the week's progress.
The major activities of the week included to have a meeting with the parents of one of my target clients. This is a new experience for me as an intern counselor, at the end of the meeting the parents were very appreciative of our concerns and for supporting their child when she needed it. With this experience, I also was able to do my first referral to the department of Human Service (DHS) in Arlington. Also I was able to have mediation meetings with students, as well as individual sessions. Follow ups were also important, they involved communicating with the students to make sure they stability and well-being.
I believed both are important for me as I begin my doctoral journey. The purpose of continuing my education and obtaining a doctoral degree is to increase my knowledge base, broaden my educational horizons, and strengthen and empower me to become a professional role model who will make a greater impact within the minority community instilling the importance to obtaining a higher education.
After reading the article, I feel responsibly, action and asking questions are my strongest characteristics. In the beginning of my career I started working in large inner city emergency department in Detroit, Michigan. I quickly learned that I am responsible to patients for making sure that they receive that best care in order to achieve the best outcome. By not being accountable to take the necessary actions could lead to less than poor outcomes & satisfaction. I actively sought out the answers by asking questions and clarifying those things I did not understand. I also took charge in my own learning curve in order to be successful in my career. As a new graduate already stressed, working in the fast pace environment, those strong characteristics
The focus of reading in EDAT 6115 this week was on utilizing student grouping to improve achievement within the classroom. In addition, there is a focus on strategies teachers can use to differentiate instruction to meet individual student needs. When thinking about effective instruction it is important to think about and reflect upon grouping our students and differentiating and personalizing our instruction to meet the needs of our students.
Taking the Bachelor Science of Nursing broadened my knowledge, especially while taking the Community health nursing II, this course taught me strategies and tools that can be used to improve the health status and health needs of the residents of the community. According to Clark (2015) “In caring for populations, population health nurses employ the same steps of the nursing process as are used in the care of individuals and families: assessment, diagnosis, planning, implementation, and evaluation” (pg. 356). As I was doing my community assessment, through research and interviews of my target audiences, I was not surprised that they have limited knowledge about diabetes and how important that changing their unhealthy lifestyle to health lifestyle can reverse, prevent, or delayed the complication of the diabetes such as hypertension, blindness, and foot amputation. This class also help me to learn and sensitive about the person’s difference in age, culture, race, gender and living situation that could impact my comfort as a nurse in taking good care and communicating with them. As a nurse, being cultural competence can make my interactions both easier and more successful with my target audience. Most important is being self-aware by identifying my own beliefs and culture
For my first paper, “Creativity is just as important as Literacy and Math” I choose to revise my paper from last semester which discussed the importance of creativity is just the same as literacy importance. I enjoyed writing about this topic and felt like I did a really good job; but to improve my writing skills I believe I can write a better paper this semester compared to last semester. I reread my paper for starters and organized the way it was based I had written in. As well as listing the facts and research I had supporting each topic. I added a few more facts and opinions to help support my points made. I didn’t receive enough feedback the response, “it’s well-written” is too vague which couldn’t help me improve my paper in any way. I feel like my introduction and conclusion are both strong, but i’m sure it could be stronger. I feel like the most I struggled in the paper was the desire for it to be written so perfectly though I knew there could always be room for improvement. This also worked as a strength due to lots of time and dedication I spent on the paper both last and this semester.
As a student, in the beginning of my professional career, writing reflection seems a sensible idea to record learning experience. The early stages of any career would play important role in learning as one settles into the ways of profession. The complex part is recognizing such experience and implementing on practice. In this reflection, I will be discussing about the role play we have had last week in our classroom. Gibbs (1998) reflective cycle will be utilized as it illustrates a clear structure for the process of reflection which includes six main stages; description of the event, feelings, evaluation, description (analysis to
Throughout life it is important to take time to reflect because it is so easy to become tangled in our worries, stress, and busy schedules. Reflection can differ depending on the individual. For example, reflection to one individual can be accomplished through attending mass and having time to pray to God. For another individual, reflection could consist of participating in meditation or yoga. No matter how it is done it can be beneficial to one’s health and well-being. Reflection consists of taking time to carefully think and so it is necessary to make time for it especially since we are so use to a lifestyle of constantly going and never stopping. Personally, I try my best to reflect each day. In order to make goals for myself at the
During my final placement, one strength which was highlighted, by both my support teacher and tutor, was embracing the values of social justice. This strength was evident in various ways. Firstly, my support teacher commented that I was able to build trusting relationships easily, not only with the children but also with the staff. This is something I see as being an important strength, simply because many of the people I came across during my placement showed, in return, both openness and honesty towards myself. She also saw my effective questioning as a strength within my professional values and commitment. Specifically during weekly discussions, my high-quality questioning I made sure that each and every child was included in generating their own learning. Secondly, I felt it was important to know the background I was teaching in and therefore created a community lesson as well as participating in a community walk. Ultimately, these actions showed great strength in my inclusive teaching (according to my support teacher). Lastly, during my tutor visit, it was pointed out that I demonstrated a commitment to engaging the learners. My tutor emphasised that I showed that I understood the importance of developing positive relationships with my pupils and quickly praised and encouraged during the lesson. These experiences allowed me to progress further in the second part of my placement, I soon found myself open feedback, from both the children and the staff. This, in turn,
