The EBP change project has shown that educating patients with prediabetes can increase diabetes knowledge and promote an increase in health promotion behaviors. The EBP change project outcome has indicated that participants who completed the 6-week diabetes education program were able to increase their knowledge regarding type 2 diabetes. Therefore, with the positive outcome of the EBP change project, the EBP change project site can incorporate certain aspects of the diabetes education program into their patient education to increase diabetes knowledge, picking healthier food options and participants in activities that would increase physical fitness.

To improve the well-being of individuals through health education, counseling and detection screening programs. For many years, health promotions and prevention has been the focus for healthcare providers, especially nurses. Health promotion seeks to improve a person or population’s health by teaching about and helps people become more aware of risky behaviors associated with different diseases. It encourages individuals to take preventative measures to prevent onset or worsening of a disease or illness. It encourages a healthier lifestyle (Edelman & Mandle, 2010). The strategy is to help people make health improvements or prevention before illness occurs.

The appropriate approach for this health promotion is through an educational approach. With this type of approach, it provides knowledge as well as helps the people to gain or developed necessary skills to guide them in making an informed decision about their health behaviour (Lucas & Lloyds, 2005). This approach does not set out to persuade or motivate change in a person’s decision-making process; the promoters are simply acting as a guide in educating the people about the disease

The Institute for Healthcare Framework developed the Triple Aim. The framework of this model is an approach for change. Within this framework there are five concepts to focus one when implementing in different organizations. These include: “Focus on individuals and families, Redesign of primary care services and structures, Population health management, Cost control platform and System integration and execution”(Institute for healthcare Improvement, 2016). The goal of the Triple Aim is for organizations to have healthy populations while reducing per capita cost of care.

Health promotion is a process, which encourages individuals to increase their knowledge through information and individual choice to recognize and improve their health (WHO, 1986).

Health promotion consists of three levels, primary, secondary and tertiary. Although all are levels of health promotion they differ depending on the extent of illness. The first level is the primary level. This level involves people who are still healthy. At this level the susceptibility of contacting disease is reduced through nutritional encouragement, hydration, exercise, and immunizations. Primary level is designed to keep the healthy free from disease.

Effective health seeking behaviors related to the expressed desire for a higher level of wellness, evidenced by, involvement in physical/recreational activity in the community. The nurse may assist the members of the community to explore motivations, behaviors, and feelings regarding their determination to increase their level of wellness. In response to the person’s self- awareness for the desired elevation of maximum health, the nurse may reinforce the need to modify and plan more achievable personal goals.
