After completing the self-health assessment with my health partner, it was obvious that nutrition was where he needed the most improvement. My health partner is a 20-year-old male, he is a construction worker and typically works 10-12 hour shifts 5 times a week. Most of the time his job sites are not local, he often stays out of town causing him to eat out at least 6-10 times a week if not more. Due to working long hours five days in a row many weeks it is not uncommon that he drinks at least 3 energy drinks a day. Working out of town and not having access to a stove or a place to keep groceries refrigerated all the time can make it hard to eat healthy every day. The boost energy drinks give are short lived and can lead to other problems such as increased blood pressure and rapid heartbeat. When he gets into the habit of eating healthier and drinking more water he may notice that he won’t need the energy drinks anymore! My partner currently seems to be in the preparation/determination stage of the Transtheoretical Model of health promotion stage. According to () individuals in this stage are planning to take action and are making the final adjustments before they begin to change their behavior. However, they have not established their ambivalence and still need more convincing. This stage fits my partner due to his past attempts to making heathier choices when it comes to his nutrition habits and his readiness to change. Although he has already taken small steps towards change such as cutting back to two energy drinks a day, and eliminating soda from his diet, it is important to show individuals support in this stage. Three different measurements were taken before and after the study. They included, pain intensity, disability, and quality of life. Pain was taken using a visual analog scale (VAS) which ranges from 0 to 10; 0 equaling no pain at all and 10 equaling the worst pain ever felt. Disability was taken using the Neck Disability Index (NDI). The NDI consisted of 10 items and were scored using percentages, the higher the percentage the higher the disability. And lastly, quality of life was taken using the Medical Outcome Study Short-Form 36 Health Survey (SF-36). Scores in SF-36 ranged from 0 to 100 and the higher the patients number got, the better quality of life.

There are many forms I use to assess an individual’s needs. The first bit of the information comes from Derby City Council, which is called a outcome based support assessment. This is what they use to identify someone’s needs and how much care they require. The information on this document is great for Derby City to use, but I also need to do my own and adapt it so it’s easier for a care worker to understand as they are the ones who will be doing the care. During the Personal Wellness Assignment, various factors influenced my fitness, nutrition, energy, and sleep numbers. I noticed that my nutrition, energy, and sleep scores were higher on Saturdays and Sundays because I didn’t have school on those days so I had more time to eat and sleep. My energy scores were high on those days because I was happy that I didn’t have school. I also notice that my fitness scores are low most of the time, except on Tuesday. This is because I have an extracurricular activity only on Tuesday. My graph shows that when my sleep scores are high, so are my energy scores. This indicates that I have more energy when I sleep for a longer time. My graph also shows that the nutrition and energy scores are exactly the same.
Trends that can be see are that countries that earn a higher income will most likely have a higher literacy rate, than a country that has low income which would have a lower literacy rate. This could be because countries that have a low GDP do not have enough funds to afford good education so that is why they would have a lower literacy rate.
After consultation with the client, a healthy diet will be the focus. The client is not overweight, exercises adequately and consistently, and is generally in very good health. The client reports eating ‘junk food’, not eating breakfast, and not getting the recommended daily servings of each food group as endorsed by the United States Department of Agriculture (USDA).

It can sometimes be too late. I have learned over some years by watching those that mean a lot to me let their lives become of nothing because they do not worry about their physical appearance or the foods that they eat, and even though you can give them all the advice to try and persuade them they are determined to do what they feel and what they want to do.

Stephan, Bilard, Ninot and Delignieres (2003) conducted a study in which they compared subjective well-being of retired athletes to that of current athletes. Specifically, 16 retired, elite French athletes were compared to 16 elite French athletes who were still competing (Stephan, Bilard, Ninot, & Delignieres, 2003). Stephan et al. (2003) utilized the French version of the General Health Questionnaire (GHQ) as a quantitative measure for this study. The authors also employed qualitative methods by administering semi-structured interviews with each of the participants. Four interviews, which spanned across a 12-month period, were conducted, and all retired athletes included in this study retired by choice (Stephan et al., 2003).
