Under the scope of practice of an RN from the New York State Education Department, an RN can diagnose and treat human responses to actual or potential health problems. To be able to perform those tasks a care plan must be made for each client. An RN manages the health care services such as observing and assessing the health status of clients and implementing/assessing nursing care. This all falls under the initial assessment of a client, which is within the scope of an RN. An RN uses information gathered as part of client assessment, they then have the capacity to assign client care to other members of the nursing team, RNs and LPNs, and assign tasks to other care providers such as nurse’s assistant. Even though there are parts of the nursing process that may be delegated to qualified personnel, the initial assessment is the RNs responsibility. Collaboration implies “working together to achieve something that neither agency could achieve alone” (cited by Biggs in Day, 2006, p9). It involves effective communication and contribution to a common goal – and the health and wellbeing of the patient and shared responsibility of the outcome. Each team requires a quality leader, regular meetings attended by all members, joint assessment, regular reviews of patient records which should include ‘shared care plans’; joint decisions following consultation and task delegation to individual team members with the outcome being that “care must be structured, organised and systematically provided to each person in a variety of ways” (Creating an Interprofessional Workforce, 2007, p10).

The rehab unit works in tandem with nurses and patients’ progress are closely monitored, their records updated and shared amongst other nurses within the rehab unit. The patients’ condition or any other issues that may arise are also monitored closely. The seamless communication we exhibit among the various units across different disciplines helped me choose this nursing care model. According to Sullivan (2015), “Interprofessional collaboration is based on the premise that when providers and patients communicate and consider each other’s unique perspective, they can better address the multiple factors that influence the health of individuals, families, and communities. No one provider can do all of this alone.” This is absolutely true that no one healthcare provider can solely take on all aspects of the patient’s care alone. Over time, studies and research have shown that patient’s outcomes and quality of care improve when healthcare providers join together as equals, it also allows for providers to bring their competence and experiences on how we can improve (Naylor, 2011).
It is no secret that communication is key when providing direct patient care in a skilled nursing facility. However, there is a noticeable lapse in the communication between the care team when providing care to the individual or groups of individuals. Two main parts of any care team are the registered nurse and the certified nursing assistant, as these are the two people whom have the most direct and impactful roles with residents in a skilled facility. The Registered Nurse and the Certified Nursing Assistant play similar roles in providing patient care, but have different roles in its entirety. The role of the Registered Nurse (RN) is defined as having the competency and skill to provide direct and indirect health care to individuals, their families, and communities around them. Services are also provided designed to give out medications, to promote comfort or healing, promote healing, and to also provide the dignity of their patients and patient’s families (American College of Rheumatology, 2015).
I have numerous experiences volunteering at the hospital, interning for an RN and a Vice President for Human Resources, which have all led to my interest of this particular program. Based on my experiences, I was able to view both sides of the medical field, both the administrative and the clinical side. I had an experience that stands out of the rest, it was during my clinical internship at Mount Sinai Hospital for Kristen McEvitt, an RN, who introduced me to the various professions that the medical field has to offer. She introduced me to Dr. Schmidt, who is a breast surgeon for the Dubin Breast Cancer Center.
