Mary is a 74-year-old female who suffers from mixed hyperlipidemia (E78.2), along with atherosclerotic heart disease, multiple aortocoronary bypass graft, and hypertension. Mary’s most recent lab results revealed triglycerides 115, HDL cholesterol 44 and LDL cholesterol 116, and total cholesterol 191. Mary has tried and failed various treatments including zetia and Lipitor, with her cholesterol level sub-optimally controlled. Mary’s LDL remains elevated, uncontrolled, and still, did not meet the criteria of being less than 70 in the presence of known cardiovascular disease. Repatha is Mary’s best treatment option at this time. Although, Repatha is not on your formulary, I am requesting an exception for coverage. Without this treatment,

statins reduce the blood cholesterol level and decrease atherosclerosis build up in the coronary arteries.

Robert is a ninety-year-old man, who has been experiencing progressive and worsening heart failure. He has a long cardiac history and has been under the care of a cardiologist for the past 30 years. His medical history includes, hypertension, osteoarthritis, dyslipidemia and heart disease. Robert lives in a senior condominium community with his wife Janet.

Lana Co was in REA from October 2016 until the end of the REA 2 Program in January 2017. I am concerned about heart disease in my patient Regi Stephen. We can identify the risk factors and detect heart diseases early. This way we prevent this disease from happening. Heart disease kills many people in the United States. I chose heart disease as the most concern for my patient because, he have high cholesterol now and is currently taking medications for it. This tells me he needs to work or modify on his diet. Another reason is the family history; his father, mother and uncle had heart disease. His mother and uncle passed away due to heart disease. Last but not the least, according to BMI he is obese. High cholesterol, obesity, family history is risk factors for heart disease. Due to that, heart disease concerns me about my patient.

Taking Statins to lower cholesterol is the top prescription drug, for adults over 60, issued to defeat CHD (Qiuping, Dillon and Burt 5). Although, the Statins prescribed are shown to lower the risk of a second heart attack in men; no significant reduction in risk is found with men who do not already have CHD, if the research is examined closer (Abramson). Once again, moderate exercise coupled with a sensible diet proves to be just as effective as an expensive drug, although once a person has CHD the Statins can help (Abramson). The drug companies have effectively penetrated every demographic in the US with “needed” prescriptions and in many instances the American people are asking for it.
