As stated in, CMSA Standards of Practice for Case Management, the core steps of case management are: 1. identifying and selecting clients, 2. recognizing problems/opportunities and making an assessment, 3. developing the plan of care, 4. implementation of the care activities, 5. evaluation of the case management plan, 6. termination of the case management process (Marion et al., 2010). All the steps allow for an organized process to occur when evaluating a patient and creating a proper, specific care plan for them. The case managers use evidence based guidelines to create their observations and assessment on the plan of care (Marion et al., 2010).
Constructing a treatment and service plan for a client involves various components. Prior to starting a treatment plan the professional should carry out a client evaluation. The evaluation should determine the basis of the difficulty or issues and assess the back ground of the client. After the completion of the evaluation, a professional can start constructing an agreement to fulfill the needs of the client. After going through a lot of trial and error in my career, I have decided that I want to pursue being a Registered Nurse. Many things have led me to this career choice, including personal and financial gain. I have tried many other occupations and educational programs and none of them have fulfilled me like I believe nursing will. I am currently staving to earn my bachelor’s degree in marketing with a special interest or minor in fashion merchandising. Ideally, my end goal is to become a fashion marketing manager. Some people wear different color scrubs that describe their personality. Also working in a children's hospital people might wear scrubs with cartoons just to make the children feel a little bit better about their situation. You maily would need basic tools when you’re an RN, and of course knowledge of a nurse. One of a registered nurse's job duties is to measure and record a patient's vital signs.
