The preceptor experience went very well. We started off the day looking at the preceptors To Do List. She explained that everyday she keeps a tasks list going on for when to complete things to make sure they are done by the appropriate deadline. My preceptor then introduced me to a few colleagues, who included the primary pharmacist, and another community nurse who ended up going and doing home visits today. My nurse was staying in the office to catch up on all the paper work and to make follow up calls with the patients and their primary care providers. Even though the nurse I was observing did not see any patients face to face she was still in contact with them. It was her duty to call and check/follow up on the patients to make sure they were doing okay and that they were following the recommendations made in previous sessions. I learned how to navigate the Care Management Information System, which is the system my preceptor uses to access patient information. It was interesting because she was able to look up patient information from both CHS and Novant. Going along with the objective of assuming roles and functions of community health nurse, my preceptor would make sure to complete the patients initial assessment within 3 days, do a behavioral health assessment screening (PHQ9), medication reconciliation through Pharmacy Home, create short term and long term goals for the patient, connect the patient with a primary care provider, and complete follow up care. By doing all of those tasks it supports the main goal, which is to decrease the number of emergency room visits and Medicare spending. Some duties will be conducting physical exams, making urgent care visits, prescribing medications and providing preventative care to residents. Providing education to families and staff education, attending care plan meetings to ensure quality care is received is also a role of an NP. Adequate documentation for billing Medicare and Medicaid or private insurance for reimbursement of services provided, is an ongoing process for both the physician and facility for payment and an NP could make the process
It is not his job to do this, however, information pertinent to the patient should be made available to the hospital. Chief Nursing Officer (CNO)—is in charge of the entire nursing staff at hospitals and hospital institutes. CNO’s are responsible for the management of nurses in their department by providing feedback, training, support, and disciplinary actions when called for. CNO’s are also accountable for the development of programs and procedures that will help the hospital institutions run efficiently and promote growth (www.ejgh.org). The role of the CNO was that of an ineffective leader in that she was unavailable (why?) when the incident occurred, however she took responsibility and accountability.
My site for my rotation was the Walgreens on Thomasville road. My preceptor was Doctor Marva Bates. I thought my experience during the first two week of the rotation was going to mainly be with the production aspect of retail pharmacy. I thought I would be pulling and staging medications for prescriptions to be filled and eventually take a role in the actual prescription filling process. There have been several studies exploring the development, implementation and evaluation of preceptor programs for advanced practice nurses. Bahouth and Esposito-Herr (2009) described a formal, 12-week orientation program for newly graduated nurse practitioners that helped to integrate those providers into the clinical teams. One part of their program was the assignment of a NP preceptor for the new employee, which was found to be effective in reducing the new NPs feelings of isolation. In addition, Baumgartner and Williams (2014) developed and evaluated a mentoring pilot project, inclusive of a variety of advanced practice providers (APPs), which supported the program’s ability to improve inter- professional networks and system supports. Furthermore, Sorensen (2010) developed an orientation and mentoring program for APRNs and physician assistants.

Having worked as a nurse in different settings; both hospital and community I have every opportunity of being in charge, capable and assisting in the management and organisation of work as required effectively, this therefore has deepened my understanding of managing clients condition, promoting and contributing to health screening and education.
Daily there are thousands of people being found guilty for a crime they did not commit because of a prejudiced judge.Sidney Lumet and Reginald Rose the writers and directors of 12 Angry Men wrote and produced a play about 12 jurors that briefly discuss a trial and come to a verdict , personal issues develop which causes conflict and only makes the process more grueling. The accused boy is being found guilty for murdering his father, 12 jurors are put in a hot room in New York and spend hours briefly viewing the scenario. Although one might think that the justice system should be left in the hands of citizens ,the director and writer of 12 Angry Men , Sidney Lumet and Reginald Rose demonstrate that the United States justice system is unfair and is simply corrupt , inefficient , and injustice due to the jurors biases minds, ignorant attitude ,and the lack of time and energy put in the trial while trying to decide a verdict.
My first encounter with a Certified Registered Nurse Anesthesia (CRNA) was during my undergraduate nursing OB/GYN rotation. I was impressed with the CRNA when she placed the epidural to the patient in labor. I remembered patient had difficulty staying still due to the contraction, but the CRNA took her time to explain the procedure while comforting the distressed patient. Once the epidural was in placed and the medication started working, I could tell the relief the patient experienced. I realized then that a CRNA goes beyond the delivery of anesthesia, pain management and monitoring of patients. Thus, obtaining this degree will prepare me to ease the patient’s mind through education, pain management, monitoring, experience and compassion.
The nurse should educate the patient about the diagnosis, how to acknowledge signs/symptoms of the disease, who to contact if they have any questions, and most importantly, the understanding of the medication they are given and why it is important to take it. The nurse also answers any questions the patient may have, and encourage and support the patient throughout the process. It is also very important for the nurse to educate the patient on a commonly made error with people taking medications; the patient should never take a double dose at one time if a prior dosage is missed or forgotten.

Lady Macbeth and Macbeth’s relationship is a trusting and close relationship, “my dearest partner of greatness.”
Over the course of this preceptorship, I have written a few different journals with the nursing process as my topic, the reason for this is because the nursing process is everywhere and has become a part of my day to day life, not just during my different clinicals over these past couple of years, it has spilled over into work, school work assignments, and home life, as well as. I now find myself assessing everything that needs to accomplish throughout my day and developing a plan of action, setting priorities to works towards their completion. Another topic that I feel is very important for a nurse is professionalism and important in developing a trust with the patients in your care, as well as, to earn the respect of your fellow nurses and other members of the hospital staff. Over these past couple years and I have seen that many of the patients and their families look to their

If Ms. W does not have time to follow through with community services that could benefit Ms. R, then Ms. W has a responsibility to refer Ms. R to people who have the knowledge, resources, and time to help her.
It is no secret that communication is key when providing direct patient care in a skilled nursing facility. However, there is a noticeable lapse in the communication between the care team when providing care to the individual or groups of individuals. Two main parts of any care team are the registered nurse and the certified nursing assistant, as these are the two people whom have the most direct and impactful roles with residents in a skilled facility. The Registered Nurse and the Certified Nursing Assistant play similar roles in providing patient care, but have different roles in its entirety. The role of the Registered Nurse (RN) is defined as having the competency and skill to provide direct and indirect health care to individuals, their families, and communities around them. Services are also provided designed to give out medications, to promote comfort or healing, promote healing, and to also provide the dignity of their patients and patient’s families (American College of Rheumatology, 2015).
As I was shadowing the nurse, I felt that clinical nurse’s responsibility and duties are quite different from hospital nurse responsibility and duties. Most of the services that are preform to patients in the hospital are not done in the clinic setting. The second week of my preceptorship brought many new experiences for me, and I can honestly say that each day I spend with my preceptor is better than the last. This week I focused on time management of a full patient load with continued documentation practice as well as admission and discharge procedures. I’ve had brief experiences in my past rotations assisting with discharge teaching and admission assessments however I have never been able to fully take charge and complete the process from start to finish, so this was a great learning opportunity for me.
