I spoke with Mr. Rasak on 8/14/17. He reports he has had the AFO since 7/26/17. He wears it about 10 to 12 hours a day and has not noticed any improvement with his pain. He said the biggest thing is the pain and that it has affected his life dramatically. He said that he wonders how much longer he has to live and doesn’t want to live with the pain. I suggested again that he speak with his PCP regarding medications. He is taking Tylenol. He is extremely sensitive to pain medications and is very limited. I suggested his PCP since he knows his history and what he may be able to take.
On 8/17/17 I met Mr. Raska and his wife at the Michigan Orthopedics office. Mr. Raska walks using a cane, he is wearing the AFO and new shoes to the right

01/13/16 Progress Report noted that the patient underwent ORIF with rod placement. He was cleared by ortho to start PT on 08/04/15. He still has decreased ROM and difficulty with ambulation. He does need to continue PT to improve his functional performance, so he can successfully return back to work.
Based on the progress report dated 05/17/16, the patient presents for reevaluation of her right knee pain. It has been about the same though she states that when she gets acupuncture, it does decrease pain for a period of time and allow her to exercise more. I met with Richard Reisinger in his window office located at the OPUS building in Natomas on Friday, October 22, 2015. Mr. Reisinger is a Business Taxes Administrator III (formerly titled Principle Property Appraiser) for the State Assessed Properties Division (SAPD) of the California State Board of Equalization (BOE). I also met with Dennis Kauffman in the lobby of New City Hall and we walked to Claim Jumper on Wednesday, November 18, 2015. (We both had fish tacos, and they were delicious.) Mr. Kauffman is the Operations Manager for the Department of Finance for the City of Sacramento.
I spoke with Mr. Sutter several times since 3/2/17. Mr. Sutter moved his medical appointment from 2/23/17 to 3/2/17. I was not unable to attend nor was I able to find a co-worker to attend. I spoke with Dr. Olenyn’s office staff many times for updated scripts and next appointment time. Mr. Sutter has lymph edema to the right and left legs. He was supposed to elevated and also wears compression stocking. His legs are so big the stockings cut in. Dr. Olenyn told Mr. Sutter he must get the swelling down because that will affect the healing of the fractures. He is at high risk for infections from open skin. Some of the bones are healing but not all. Mr. Sutter continues to use a bone stimulator daily. I have spoken with Mr. Sutter and
I wanted to let you know we had Dr. Cohen call us twice yesterday unsatisfied with the way the report was read. He spoke with Dr. Rotblat who had read the report and ask him to change certain things that he wasn't happy with. But Dr. Rotblat told Dr. Cohen to call our front desk and ask them to get another radiologist to read it. I called Dr. Cohen today to get more information about the situation and know what could we do to help resolve the situation and make it better as well I apologize to him for the way the situation was handled. He explained certain words and Diagnoses in the report was incorrect and not true. So We had Dr. Panasci do an Addendum. I did follow up with Dr. Cohen office and spoke to Julie left
Mr. Rasak is working very hard in therapy especially since his dizziness seems to have subsided. He was able to hop 3 times today with parallel bars. He is able to use a slide board with stand by assistance. He needs help with moving the leg due to the external device being very cumbersome. They are starting to train him in using a walker and being able to non weight bear with the right leg.
Having a dad that is in the military can be tough. I had to adapt to new places and go through situations that made me doubt who I really could be. Through it all my family has always been there for me. When I moved to Virginia it was hard for me to adapt. After I joined the soccer team the situation brightened up. It’s coming to that time of leaving my family. They will be moving to Knoxville, Tennessee when I graduate. Radford is close to Tennessee but still in Virginia. I seek to join Radford’s nursing program and soccer program. Radford has everything that I dream in a college and more.
On April 7th, I met up with Mr. Studnicka. He is a special education teacher at Richland Center High School. Mr. Studnicka started working in the education field this school year. The reason that he entered the field of education is because he enjoys working with kids. He loves interacting with the students and loves to see them learn something.
LaShawn is a personable, eager to learn, and professional candidate. Her last position was working as an Admission Specialist for IU Health North Hospital. She was responsible for registering the patients, verifying their information, and entering updated or new information. She left IU Health when she had to have surgery and used all of her FMLA time, but ultimately needed more time to heal and had to leave the company. She took the next year to focus on getting better and making sure everything was taken care of. She had previously worked for Comcast as a Customer Account Executive. I was then expelled from Quisqueya Christian School at the age of thirteen and I was in the seventh grade. Being expelled from this fine institute I got in trouble with my parents. Coming from a conservative Muslim family, it was kind of difficult to go to any establishment that had anything to do with sex. In order to go I had to lie to my parents telling them that I was meeting up with a group for a project. Going to these places was a lot different than I thought.
