Patient Interview Reflection

The key informant that I chose for my interview was Breanne Watson who is a registered nurse at Surry County Health and Nutrition Center in Dobson, NC. Watson explains that she has been a county health department nurse for five years and has worked in several different areas within the health department. Currently, Watson specializes in communicable diseases at the health department. Watson states that her job consist of providing services to individuals with communicable diseases, reporting the disease, planning treatment regimens for the individual, ordering medications and following up with the individual. In addition, Watson educates and counsels individuals on communicable diseases and takes part in the health team planning of the community

Respect comes in many forms, mainly in ensuring the patient never feels uncomfortable and can always express their point of view.

Even though everyone seemed to agree and felt the same way about the situation, it was difficult to rationalize or logically defend what we felt was the right thing to do, which would involve breaking patient confidentiality. Some of the principles that we thought could be used to defend our position were beneficence, non-maleficence, and utilitarianism. Going back to the Dr. Pellegrino’s article, the population-based ethics, in which the physician takes the role of a social servant for the good of the society, could also be used to defend our position. This discussion served as a motivation to not just learn the ethical principles, but also to become more knowledgeable in how to apply the ethical principles to different situations. Maria is a caregiver at Peppermint Tree Daycare in Edison, New Jersey. Maria has known me for all of my life, although I don’t remember her as a child. When I was a young girl I attended this facility where she took good care of me and watched me grow until I was four years of age. My mother and she grew such a tight bond that she would babysit my sister and I on weekends when my mom needed to work overtime. As years went by, my mom and Maria lost touch. Two years ago, I ended up getting a job at Peppermint Tree Daycare. I now work in the baby room with Maria five days a week and look at her as a second mother. Because of my close relationship with her, I felt interviewing her would be perfect. I asked Maria if she would be willing to do this interview and in a heartbeat she considered my request.

The Patient Outreach Certified Medical Assistant is responsible for performing quality improvement tasks, including but not limited to, patient chart review, contacting patients to remind them of needed services, placing orders so patients can complete the needed test/screening, documentation in the electronic medical record, and scheduling of appointments.

[Name] is now 14-1/2 -years-old. He is seen in followup for insulin-dependent diabetes, which was diagnosed exactly 1 year ago, at the age of 13-1/2. He has been followed by me since January of 2007 and I have been in contact about twice a month with his mother [Name], who has faxed blood sugars and I have gone ahead and adjusted his insulin doses and I am very please with how well he is doing. This is an amazing teenage who really seems to take pride in taking care of himself, and is very compliant with his diabetes regimen.

My paper is on my mom she is 45 years old and really close to my heart. I did an interview with her to get the answers to the questions that you gave me. My paper will talk about her health, stress, retirement, marriage, sibling relationships, and her thoughts on everything.

The 5 questions and their answers I asked in my interviews are as follows; 1). How did the professionals such as the Dr.’s, nurses, PT, OT, etc. treat and/or interact with your father/client? My first woman told me that her father was a Korean war vet and that the majority of his Dr.’s were at the VA in MN. She was not impressed because many times they all seemed as if it was a burden for them to have to see him. She also observed young men and women there in waiting rooms who were treated with respect and they seemed to get right in to their appointment whereas her dad would sometimes wait 1 to 3 hours only to be told that they would have to reschedule. It was very sad. My second woman I interviewed told me that rarely would she ever see Dr.’s

In the midst of attending a party with his friends, a random, ricocheted bullet pierced the right side of his head while he was trying to break up a fight. The bullet blew away part of his skull and lodged into his brain. When he arrived in the trauma bay, medically he was dead- no pulse, no spontaneous respirations. His family was forewarned that if he woke up there was a strong possibility that he would never speak or walk again. The patient- B.H., was a seventeen-year-old high school senior, who was class president, and captain of the football team when he was shot. During his month long stay in ICU, the hospital staff worked around the clock to ensure that his issues were properly managed. Despite all odds, B.H. is currently in his senior year of college, plays semi-professional football, and works two jobs. What was particularly fascinating, was the way the physicians were able to prioritize and execute their management in a dire situation. It is apparent from visiting with Dr. Nael P. Ghael on yesterday December 21, 2017, she was no more prepared or familiar with my medical history then she was on November 22, 2017. In November she said she would order my records from Care Now and wait on the results from the MRI. She did not know that I had physical therapy, she said to call you again about me being off work she could not do that. I told her when I did call you, you were so nasty to me and rude I did not get you to listen to why I was calling. I am going to ask you the same question I asked The Human Capital Staff if you are sending me to a doctor that does not have or will not allow one off work to heal, where will I get a doctor’s statement from. When the patient first came in I think I did quite well as I welcomed

Under CRNBC professional standards of ethical practice the nurse ensures that the patient who is receiving care is the primary concern, ensuring that the patient is receiving care in a way of preserving the patients dignity. (CRNBC Professional Standards) “To treat someone with dignity is to treat them as being worth, in a way that is respectful of them as valued individuals” (Baillie & Gallagher, A., 2012, p. 44). As I observed the situation I felt that the patient was not being respected as an individual.

Today I had a great day at the clinic. For the morning section, I had Omar Lora as my patient. Last time when he came, I collected all my assessment data. Today I updated his medical history, dental history, vitals, and EIOE, then I completed filling out the gingival assessment, the treatment plan, and the SAOP. Finally, I was ready to have my assessment data checked. It went really well, and I learned ways to helped me be more efficient with my time management, for example, I did not know how to have my radiographs up in the other monitor while I was doing my assessments. The health history assists health care providers and patients. Information is obtained that can lead to a diagnosis and the treatment options available. It is important that trust between patients and medical professionals is obtained. The information will determine what is normal and expected for the patient. The interview process is the best and most common way to obtain information from the patient (“Health History,” n.d.). The patient interviewed has numerous pulmonary and cardiac issues that have affected her quality of life.
