TERMINOLOGY
CLINICAL CLARIFICATION
Onychomycosis is a fungal infection of the nail bed, matrix or plate. [1, 2]
It was historically referred to a non-dermatophytic infection of the nail but is now generally used as term to denote any fungal nail infection. [1] CLASSIFICATION
Onychomycosis is classified to several types characterized by clinical presentation, the route of invasion, or causing organism: o Distal subungual onychomycosis (DSO) is the most common form characterized by invasion of the nail bed and underside of the nail plate beginning at the hyponychium. [1, 3] o White superficial onychomycosis (WSO) is less common than DSO (10% of cases) and occurs when certain fungi invade the

[4] o Endonyx onychomycosis (rare) does not involve the nail bed only the interior of the nail plate is infected. Signs include [1]: o “Twenty-nail” dystrophy o Onychorrhexis (exaggerated longitudinal ridging) o “Angel wing deformity,” in which the central portion of the nail is raised and the lateral portion is depressed
A patient with 20 dystrophic nails is unlikely to have onychomycosis. o Contact dermatitis
Occasionally resembles onychomycosis.
Diagnosis is facilitated by [1]: o Knowledge of known irritants or history of contact dermatitis o Presence of contact dermatitis elsewhere on the body. o Nail trauma
Repeated nail trauma can cause distal onycholysis, which can lead to colonization by microorganisms that produce pigmentation of the area. [1]
If the nail is clipped, the nail bed will be normal if the symptoms are caused by trauma rather than onychomycosis. [1] o Other
Nail products containing formaldehyde may also cause onycholysis. [1] o Nails may present as yellow and all exposed nails are affected. Diagnostic groups o Not applicable
TREATMENT
GOALS o The primary goal is to eradicate the infection as there is always a relapse in undertreated conditions. [1] DISPOSITION

I agree artifical nails should not be worn. I know there are some nurses who are very diligent with handwashing unfortunately not all. There was an incident where patent died because of an infection that was artifical nails.our hospital doesn't have a policy but it will come very soon.I think depend on where you work or hospital policy but should not wear fake nils. when any nuese come with patents.they all cary bacteria and incerse the rick diease.

Onychomadesis occurs whenever the matrix is damaged and stops producing new cells. Localized trauma can cause single digit onychomadesis, meaning it only occurs on one nail. Local trauma can be any damage to the fingernail that harms the matrix. When the matrix is harmed, the fingernails are unhealthy and will not grow. “Multi digit onychomadesis suggests a systemic problem such as fever, use of drugs, or viral infections” (1 – MD Edge). This disease can also present itself after chicken pox. Onychomadesis is cause by a fungus in the family of Moniliacaea (3 – Remedy Land).
Music can be the lead to freedom from darkness. The story “Sonny’s Blue” by James Baldwin takes places in Harlem around the 1950’s. The story is about two brothers. One of the brothers is the narrator of the story whose name is unknown. The narrator tells the story of the struggles in Harlem but mainly the struggles of his brother with drugs, and his passion for jazz music. Onychocryptosis is otherwise known as an ingrown nails. It may affect the fingers or toes but most commonly found on the big toe. When a toenail is ingrown it starts to curve and grow into the skin which usually happens at the border (sides) of the nail. This irratates the skin which can often lead to pain, redness, and swelling in the toe. Bacteria may enter the skin if the ingrown nail breaks through, causing and infection in the area. Even if the toe is not painful, red, or swollen a nail that is curved down into the skin can progress to an infection.
