On 6/13/116 at approximately 1811 ICS was activated in RVST unit for offender Mcfarland OID 179096 who fell on the floor in the shower room and medical was requested to respond. Upon arrival on the scene offender was found on the floor naked, hyperventilating, shaking, cold, unresponsive. She was alert and conciuos. Intial assessement was completed on the scene and offender was lifted from from the floor by staff and was placed in a wheelchair. As soon as she was placed in the wheelchair, she started making verbal statements and answering simple questions. Offender was assisted by medical staff to get dressed and was wheeled to the clinic for further evaluation. offender was assessed in the clinic and had no signs of improvement. On call
Upon arrival to Western State Hospital, deputy J was asked to pat search the inmate Kelly due to unavailable female staff at the facility. I informed the intake officer Robert that inmate Kelly has a history of mental health issues; she has assaulted a few police officers. Inmate has stated she wants to go to a spaceship. During transport inmate Kelly sustained a head injury. She was taken to a local hospital and evaluated by medical staff. A copy of the hospital evaluation/discharge document was given to the intake officer.
On 5/29/17, at approximately 1804 hours, I was calling the last inmate for medication distribution in D-4, A pod. Myself and Nurse Bob were stationed at the entrance of A pod. Inmates began to crowd around the entrance of the bathroom. An unidentified inmate started yelling, "There is a guy on the floor in the bathroom having a seizure." I ordered the inmates to move out of the doorway of the bathroom. I had a partial view of the bathroom from the pod doorway and was able to partially see an inmate lying on the bathroom floor. I directed all inmates to lockdown in A pod and made a radio call to Security Control indicating there was a man down in D-4, A pod.
On 11/07/17, I D/O B. Bailey #D1200 was assigned to the Medical Clinic as a floor officer. At approximately 0230 hours. Inmate Bowman, Torry BKN #1720190 refused medical treatment. As a result, he was given a refusal to sign so he could be returned to his floor. Inmate Bowman was given a pen so that he could sign the refusal. However, Inmate Bowman did not return the pen. Inmate Bowman was given several verbal commands to return the pen; however, he failed to comply. Lt. Steele was notified. Consequently, 8 male inmates in 3LB 23 were searched also a strip search was conducted. No pen was recovered.
On 02/06/2017, at approximately 1715 hours, while I was conducting meal distribution and ID Headcount in Durango Jail, 3225 W. Gibson Lane, Phoenix, AZ 85009, Housing Unit D-3, Pod D. When Inmate Peterson, Jordan T324870, cell 57-2 received his meal tray, I noticed he had dried blood stains on his shirt, his left eye, left cheek and left corner of his lip were bruised and swollen. At that time I asked the inmate what happened and he replied, "It’s nothing." I told him to step out of the pod and have a seat in the multipurpose room of the housing unit.
REPORTER: The reporter/niece (Deshannon) called with concerns for the victims, Louise and Lorinn. Louise can not walk or dress herself (needs assistance with ADL’s); the reporter said she is very confused. Lorinn is disabled (needs assistance with ADL’s). The reporter said the victims were admitted into the hospital (unknown) when and released on 12/23/2015. Prior to being admitted, the reporter said the doctor said he would see if he could have them placed into a nursing home or facility. The reporter said they were released from the hospital and didn’t go to the nursing home. According to the reporter, the doctor said they were dehydrated. The reporter said they need help; Lorine has a nurse who comes out to the home and gives her bathes,
On 4/27/17 at approximately 1835 Medical was requested to respond to Higbee to do an assessment on offender Smith OID 252929 following an altercation with another offender in Tubman Unit. Offender was assessed and was noted to have a small abrasion on the left arm and scratches on the legs. Vital signs were stable. The dressing was applied on the abrasion on left arm and ffender was informed to contact health service if she has any other medical concerns.
On 10/11/15 at approximately 1944 ICS was activated to an offender OID 236740 who was bleeding and Medical was requested to respond. Upon arrival offender was sitting on the chair in no apparent distress. Offender had dry blood on her hands and on the head. Offender was brought to the clinic and was assessed treated per protocol. On 10/28/15 at approximately 1930 an ICS was activated at Higbee for an offender McDonald OID 200046 who was unresponsive . Upon arrival offfender was lying in bed, but was not responding to command. Officers entered the cell cuffed up offender and assited her to the sitting position. At that point offender started talking to staff. Offender was in no medical stress and denied any health concerns. She stated that she was tired. Vital signs were normal. Offender was advised to let staff know if she has any health concerns.
On January 19, 2016 at approximately 1415, I was called to the special housing unit to perform an immediate use of force medical debriefing. Upon arrival to the special housing unit Captain Roan McCollough and Lieutenant Scott William were present in the SHU and informed me that Officer Enrique Velez had been bitten by an inmate. I questioned why the staff member wasn’t sent to medical for immediate evaluation, due to this being a potential blood-borne pathogen exposure and received no response from either individual. I then took Officer Velez back to the medical room in SHU for a clinical interview to determine the course of treatment that was required for his exposure which was not classified as a blood borne exposure based on clinical guidelines.
The reporting party (RP) stated on 4/1/16 resident Clifford Soares exited the building and was found by the local Police Department and returned at approximately 2:00AM. According to the RP the resident lived in the memory care section of the facility. When the resident exited the facility the alarm didn't sound or the caregivers did not hear the alarm sound. The RP stated the facility failed to secure and monitor residents in the Memory Care Unit.
On 12/31/16 at approximately 1959 ICS was activated in Bethune unit for offender Suwuan OID 248356 for a seizure like activity. Upon arrival at the scene, offender was sitting on the floor bleeding from her right side of her face. She was alert, confused but in no acute distress. Officer who witnessed what happened, reported that offender fell from the chair and hit her head on the floor and started shaking for about 30seconds. Offender sustaned ab abrasion on her right side of her face. Offender's vital signs and Neuros were checked and was put on the wheel chair and brought to medical for further evaluation. On 10-30-2017 I Deputy Mignone #95 was assigned to Intake/Release. At approximately 1622 hours, I was told that inmate Matthew Spradlin J#13109516 was to be placed in the restraint chair per Dr.Dehner. I retrieved the chair and met Deputy Williams #32, Deputy Staton #166, Deputy Ivey #30 and Sergeant Johnson #150 at cell HD-6. Inmate Spradlin was handcuffed through the food portal and placed in the chair. Deputy Ivey secured the right leg. Deputy Williams secured the left leg and right arm. Deputy Staton secured the waist, left arm, and left chest strap. I secured the right chest strap and un-cuffed inmate Spradlin to have his arms placed in the restraints by those stated above. Inmate Spradlin was cleared from the restraint chair per clinic
On June 10th, 2016 at approximately 2205 hours, FHEO Security was notified by clinical staff in reference to an incoming (51D) Disorderly Patient in E.D., located in the EMS offload. Upon arrival, Security staff made contact with Orange County Fire Rescue (OCFR) Engine #83 and restrained patient James Patrick Hogg (DOB: 03/26/1968 - FIN: 85758458). OCFR staff stated patient, Hogg is Altered Mental Status (AMS) and was uncooperative with rescue personnel. Security staff escorted OCFR and patient, Hogg to the Special Care Unit (SCU) room #37. Patient Hogg agreed to remain calm and be cooperative during his care at the hospital. Clinical staff removed the soft restraints off of Mr. Hogg without incident. All cleared, the final call disposition
On January 22, 2016 at 1230 hours, I was monitoring inmates in the Hobby Shop area located in the recreation department. I noticed Offender SMITH had taken something from Offender JONES, who had been painting a picture at the station beside Offender SMITH. I believed this object that Offender SMITH had taken to be a paintbrush that belonged to Offender JONES.
Thanks for the feedback. As I stated in my post, I am an ex-offender and in the previous years there were program to assist one who had been released from a confined setting.
