To what extent are recreational supports accessible to all community members? • The parks located in downtown North Judson are free to the community members, which makes them very accessible and affordable for families as well as visitors from other communities. • Kathy Lambert, an administrative assistant at St. Peter Lutheran School states that art, music, physical education, sewing and knitting classes are free to the students that attend St. Peter Lutheran School. There is no children’s hospital in the town, which also puts children at risk for some health issues. The closest hospital is ten miles away. Both of these potential problems lead to the big problem of access to proper and prompt health care treatment in case of an emergency. There is no convenience or department store, and many small businesses are closed. This paper will conduct a community assessment of Marion County, Indiana. Marion County is located in the heart of Indianapolis, which is the capital of the state of Indiana. Marion County was created April 1, 1822 and was formed from Delaware New Purchase. It was named after General Frances Marion from South Carolina in the Revolutionary War period. This assessment will address the health needs and risks of Marion County through the use of the WGU assessment forms identifying the population’s economic status, neighborhood and community safety inventory, cultural assessment, and the disaster assessment and planning guide.
Another strategy would involve utilizing data from existing public sources to assess the health and safety needs of the people of Utah County. The main elements of the collected data would involve the identification of the key health indicators and the corresponding risk factors. I would also collect information related to the impact of the social, economic and the environment on the health status of Utah inhabitants. The key sources of data for the assessment of needs of Utah populations would include US census bureau, healthy people 2020, environment public health tracking program, department of mental health and substance abuse and Utah hospital discharge database. Nestled in Southern Indiana just west of Louisville, Kentucky and south of Indianapolis, Indiana is Dubois County, Indiana. Dubois County is comprised of the towns of Birdseye, Ferdinand, Holland, Huntingburg and Jasper. Of those towns, Jasper is the largest and is the county seat (Kelly School of Business, 2014). Over fifty-two percent of the population consists of adults ranging from 25-64 years of age (Kelly School of Business, 2014). The leading cause of mortality in this county is heart disease, cancer, suicide and injuries (Memorial Hospital and Health Care Center & Dubois County Health Department, 2011).
The purpose of this paper is to provide insight on health disparities seen within DeKalb County. The residents of DeKalb County are diverse in race, ethnicity, and income. Since the county is so diverse there are many health disparities seen amongst its residents. Health disparities occur within a county when one group of a population has noticeably better or worse health than another group within the same population. These groups of people can differ by income, education, sex, race, location, and even sexual orientation. This Paper will discuss the health disparities that exist within DeKalb County, and explain why these disparities exist from a historical, political and social aspect. Furthermore, this paper will give background information on health outcomes from national, state, and local epidemiological perspective in order to support the claims that disparities actually exist within this county.
A community’s health can be established through a myriad of differing factors. Three specific factors that will help to determine the health of the county in which I live are community safety, disaster preparedness, as well as an analysis of the population. The community under review, Merced, is located in central California and made up of seventeen cities and communities whose individual populations do not exceed more than 75,000 and covers an area of approximately 1,972 square miles (United States Census Bureau, 2010) with a total population of approximately 255,793 people in 2010 (Merced County California, 2012).
The community I decided to research and compose this paper is one from which I live, Kent County Michigan. I gathered data from several different resources such as the Kent County Health Department and Michigan Department of Community Health in order to create a community assessment. The data I collected can be broken down into four different assessment pieces.

The community identified its priority health needs by doing Community Health Status Assessment(CHSA). The aim of this was to obtain feedback from residents regarding their health status, access to healthcare and their perception of community services. The study included residents of Atlantic County and surrounding areas in Camden and Cape May counties that are serviced by Atlantic County hospitals in2002. The first research component was a secondary data collection. This secondary data profile contained health and wellness statistics collected from New Jersey, the Department of Health and Senior Services NJDHSS, CDC, and U.S. Census Bureau data. For the purpose of this paper, the community being analyzed will involve the population that lives within the boundaries of the Nutley, New Jersey. Nutley, a township within Essex County, New Jersey, is home to a dense population of individuals within its small borders. It meets the criteria of a community due to the fact that the individuals share some mutual characteristics (Kirst-Ashman, 2011). Nutley is a residential neighborhood that consists of 11,301 households. Nutley, New Jersey is also a geographical community, where individuals share the common variable of location (Kirst-Ashman, 2011). Subsequently, Nutley contains one high school, one middle school, and five elementary schools (Nutley, New Jersey, 2016). Within the 3.428 square miles that Nutley is comprised of, there are 10 public parks where residents are welcomed to play soccer, basketball, lacrosse, and baseball among other sports.
Over two thirds of my life have been spent living in a western suburb of a large metropolitan area which has access to extensive resources. I grew up in a rural area and thought it would be interesting to look at what resources they have and how their needs are defined in the community health needs assessment (CHNA). The two CHNAs chosen to review were Pekin Hospital and Morris Hospital and Healthcare Center (MHHC), both are nonprofit organizations. Pekin Hospital is a 107 bed medical center located in Pekin, Illinois. Their defined their area of service with the use of only a map and its total population of 135,394. Morris Hospital and Healthcare Center CHNA has a more extensive definition of its physical community which included 5 counties, 22 cities and a population of 112,742. Their medical facilities include a main hospital campus with a total capacity of only 89 beds, two other hospital campuses and nine health care centers.
The impression about Hamilton after the windshield survey and interview of two of their citizens were in a way similar and different at the same time. Similar because I was informed already of the drug problems in the city which can easily be related to the actual high crime rate; however, it was interesting to observe that even when it is higher in relationship to the country that the number of crimes in Hamilton are decreasing according to previous census. It was interesting to find out that there is still a few manufactures located in the city which contribute jobs for the city residents. On the other hand, I was surprise of the fact that a great number of the population is under the age of 18, that school grades are low grades according to the AMO as well as the student progress on a city with a high number of young population. In reference to environmental health, I was amaze about the animal waste report as well as finding out about the companies contributing to pollution. Finally, I can say that the mix of both sources of information provide me with a better picture of the city which can help me at the time of understanding some of the problems of the community. Glenwood Park was founded in 2003 and sustainably consists of thousands of pounds of reused and salvaged materials. Green Street has salvaged and reused 259,200,000 lbs of concrete, 800,000 lbs of granite ruble blocks, 30,000,000 lbs of wood chips (An Earthcraft Community, 2003). The wood chips were converted to energy via a waste to energy plan that produced enough energy to power 1355 homes for a year (An Earthcraft Community, 2003). The three dominant health issues were mental health, substance abuse and dental care (Fig.5 ) The difficulty in accessing care for the mentally ill people was one of the most important issues that almost all of the interviewees and online survey participants noted. The main sources of mental health difficulties were seen as 1) depression or anxiety, 2) substance abuse. The condition even is poorer for the children in need of mental health services, because there are not enough child psychiatrists available in the area. Consistent with the survey data presented in this section, substance abuse was seen as another significant problem among key Informant Interviewees. The majority of interview participants talked emphatically about substance abuse being a problem in their communities. Most participants talked about “access” as the leading cause for drug use especially in Haverhill. Other causes included “unemployment,” and “loneliness.”
The result of the healthcare related questionnaire shows that, substance abuse which was of greatest concern in the County, followed by chronic disease such as cancer, lungs, heart, diabetes, and high blood pressure, next was the lack of access to health care, followed by teenage pregnancy; and finally homelessness. On the other hand the result of the safety questionnaire revealed that residents were most concerned about alcohol and drug abuse, followed by Gang-Related violence third was school violence, then child abuse and neglect, and unsafe/unprotected Sex (“Hudson County community,” 2007).
Encompassing a total of 42 units, Burlington Heights was just constructed in 2011. This modern condominium was completed in two phases. The first phase included a total of 16 units. Meanwhile, Burlington Heights II included an additional 21 units. Both phases of the project included garden-style condos on one level.
