Nicu Reflection

John Wayne Gacy was convicted on the murder of 33 teenage boys between 1972 and 1978. These murders took place in Cook County, Illinois; all inside of John Gacy’s home. The city of Chicago, Illinois, during this time was very violent, with crimes happening regularly; this played out well for Gacy. Twenty victims have been identified, while thirteen victims still remain unidentified. All of John Gacy’s victims were teenage boys, all dying due to strangulation, except for his first victim who was stabbed to death.

After graduation, my immediate nursing goal and plan is to find employment as a registered nurse on a pediatrics unit at a local hospital. Although I am striving for a position in the pediatric intensive care unit, I will be grateful for any job on any pediatric nursing unit. In this position, I will gain a more concrete foundation on the multifaceted needs of both the pediatric patient and their families. Today, Amanda Gordon, an Occupational Therapist can and talked to us today. It was such an honor for her to talk with us. I was especially interested in her lecture because I was thinking of becoming an Occupational Therapist, and she taught me so much more information about OT. Amanda was working at Moss for 5 years. She works with spinal cord injury patients and teaches them skills to accomplish everyday tasks. She teaches and practices them real life activities such as how to dress, cook, tie shoes, and much more. Amanda loves seeing her patients improve and slowly going back to their normal everyday routine. Because of Occupational Therapist Amanda, patients become more independent and be able to do an assignment.

This essay will demonstrate my reflective abilities within an episode of care in which I have been involved with during my practice placement. It will discuss several issues binding nursing practice with issues of ethics and the model of reflection which provided me with a good structure and which I found most appropriate, is Gibbs model (Gibbs 1988). This particular model incorporates - description, feelings, evaluation, analysis, conclusion and the action plan.

Reflection is a process of exploring and examining ourselves, our perspectives, attributes, experiences and actions / interactions. It helps us gain insight and see how to move forward (Nursing Times 2018). I believe reflection is particularly important when it comes to Nursing, as medicine is constantly changing/ improving and us ourselves medical professionals must adapt with the changes in medicine. I find that reflection is extremely useful in doing this as we can look over procedures or experiences that we have had, how that made us feel, whether we would change anything, then in the future we can see the changes that may have been made, whether this has changed our feelings and opinions on medical practice.

Overall, I do not feel like a great deal of improvements needed to be made in my delivery of care. I felt comfortable communicating with my nurse and the health care team, assessing the patient, and educating family members. My nurse did not feel well, so she started off the day a little on edge and irritated, but through showing that I was willing to assist her in any way possible, she was more than willing to assist in my learning. I could improve my delivery of care by asking my nurse more questions and looking up more information in the patient’s chart. I noticed that the layout of the NICU was quite different than last week, so I did not have a computer to look up patient information, but rather, I looked alongside my nurse.

During the past few shifts, I have been astonished, and thankful about how often the nurses, doctors, and respiratory therapists have been providing me with learning opportunities. I feel this is because they have observed my enthusiasm and willingness to learn. For example, I asked a nurse if I could observe her giving care to a 24-week old preemie with necrotizing entercolitis. This nurse explained, that because of this baby’s immature age, she had to cluster the baby’s cares, and monitor this baby’s oxygen saturation. If the baby was overly stimulated her oxygen saturation would decrease, and the nurse needed to stop working with the baby. This nurse also pulled up a PowerPoint about NEC, so I had a visual of this baby’s condition. She provided me with a NICU parent book, and I read the section about NEC. This nurse said that she gives NICU parents this book to read, so that they have a better understanding about their baby’s condition. This is an awesome education technique that I want to adopt. I accomplished my initial goal of becoming a certified preschooler teacher and acquired a further understanding of the pediatric realm of occupational therapy based on my educational experiences. The objective of this reflection is to explore and reflect upon a situation from a clinical placement on an orthopedic unit. The incident showed that I did not provide safe, timely and competent care for my patient when the oxygen saturation was low. Furthermore, this reflection will include a description of the incident, and I will conclude with explaining what I have learned from the experience and how it will change my future actions.

In recent studies, there are about 11.5 million undocumented immigrants in the United States. The undocumented immigrant population has grown 27% between 2000 to 2009. Immigrants from Mexico make up 59% of the undocumented immigrants in the United States. These undocumented immigrants can help the economy and country grow. These undocumented immigrants do have some downside to them, which makes people question do they really help this country. Skin integrity is an important concept that’s nurses assess on their patients. A key skill in nursing practice is to frequently assess the skin for possible breakdown or decreased skin integrity. Skin assessments should be conducted thoroughly once a shift and frequently reassessed for any signs of change. Skin discrepancies may be the first sign of an underlying issue. Early detection of any breakdown can help to implement interventions sooner. Unfortunately, unless there is a major skin discrepancy, skin issues can easily get overlooked, specifically in documentation and report. The focus of this paper is to research new skin integrity assessments to improve documentation effect and accuracy, resulting in decreased prevalence of skin breakdown in hospitalized patients. Topics discussed include reviewing current practices and new skin assessment techniques that decrease the prevalence of skin breakdown and pressure ulcers.

During my senior year of high school, our graduating class had to take more than one career assessment. I was given choices in the health and human science field. I was a lover of science and the human body so there is no wonder on why I was given these career choices. You never think about choosing a career until you are given a list of what there is to do in the world. I put a lot of time and effort into choosing the career I want the rest of my life. I found that a lot of my childhood was spent in hospitals. When I was there I made sure to make time to look at the babies in the nursery. My mind would come up with a ton of questions regarding their life, family, and health. This lead me to choose a career pathway in neonatal nursing. Neonatal nurse practitioners normally deal with babies who need immediate medical attention, but sometimes there are instances when that is not the case. I would like to become a neonatal nurse practitioner so I can apply my education and experience to better the newborns’ health and well-being.

Today is my first day doing my practice observation in the Infant Room Classroom Number 3, my fears are how the infants go reaction to see me because I am new person for them that they never have seen before, how the teacher go feel to have a student in the classroom, if they will support me and cooperate with practice, and how the parents will response to see a new teacher in the classroom, I know for my teaching experience that parents are very special about the people who is round to their children.

Reflecting upon my performance during the CAPSTONE simulation, I feel I did really well. After reading about the history of the patient, recent procedures, complaints, and lab work I quickly determined he most likely had suffered a perforated bowel. However, I did not limit my thinking to that specific ailment. Maintaining an open mind as a nurse is the only way you can prevent overlooking something because you are not open to considering it.

Through time we have come to observe that the outdated system of management has transformed and undergone various changes within the century. Before, employees would carry out their assigned occupational duties during an allotted amount of time and once that time elapsed they would go home. However, now times have changed. Employees are now encouraged and motivated to grow, contribute input, as well as contribute as a whole in order to improve the overall state of an organization. There is a great deal of information for leaders to become more informed and knowledgeable of how to achieve personal growth, which, ultimately enables the organization to grow as a whole as well.
