The Personal Health Assessment, was pretty objective in my view. The questions ask about your life and habits that view everyone from the same vantage point. That’s to say everyone is a healthy, happy, active person. It doesn’t take into consideration if you have food allergies and can’t eat whole grains, fruits, certain meats or fish. Or that while dealing with some of these health issues you are too exhausted to go to the gym multiple times a day/ week, not to mention your outlook on the world. I feel like if the tests could be more broad of open ended there could be a more accurate outcome. I know though that, you make a situation, what it’s going to be by the way you approach it. Whereas, Assessing Your Wellness, was very subjective in the way it asked for your input. It also felt less weighed in that you got to choose how socially, financially, mentally, and emotionally aware you were.
Personally, I was not surprised by any of the findings because I feel as though I am pretty aware of myself and my surroundings. Yet, I was concerned to see that my physical and

So believing that it is possible to recover from an illness and working towards that, will probably get you a lot further than just lying in a bed believing that it is impossible. Which goes with the statement in the video, changing IMPOSSIBLE to I’M POSSIBLE. Being able to realize that life is a gift, but not a promise, and that we as people have a duty to be kind and uplifting one another. Whether that is though simple acts of kindness or just sitting with a patient in the hospital so that they can tell you their story. A change in perspective can change the world, and the lives of so many people, because that invites in creativity, collaboration, and connection, which for people in the hospital is a way to be more active and motivated to get back to a happy and healthy
Three different measurements were taken before and after the study. They included, pain intensity, disability, and quality of life. Pain was taken using a visual analog scale (VAS) which ranges from 0 to 10; 0 equaling no pain at all and 10 equaling the worst pain ever felt. Disability was taken using the Neck Disability Index (NDI). The NDI consisted of 10 items and were scored using percentages, the higher the percentage the higher the disability. And lastly, quality of life was taken using the Medical Outcome Study Short-Form 36 Health Survey (SF-36). Scores in SF-36 ranged from 0 to 100 and the higher the patients number got, the better quality of life.

While completing these assessments on myself, my overall wellness score rated very well. My highest strategic area was intellectual (mental). I am an avid organizer and this allowed me to score the best in this area. I have close relationships with girlfriends and this allows me to utilize them as my counselor as needed. I also have a great relationship with my husband and communicating with him on a daily basis allows me relieve some of the stress from the day. I believe laughter is the most important gesture of emotion. Laughter is good for the soul, and I tend to laugh often each
I'm going to try a weekly planner for writing down what is due and when then checking off the completed assignments.

Using Gordon’s functional health patterns to assess a family will guide the nurse in developing a comprehensive nursing assessment that is holistic in nature. Gordon’s functional health patterns are founded on 11 principles that are incorporated within the nursing practice. These 11 principles serve as a framework for a thorough nursing assessment in which to build a holistic and individual family care plan (Grand Canyon University, 2011). The author has developed family-focused questions for each of the 11 principles and utilized these questions as a tool to assess her own family. This paper will summarize the family assessment that was performed by the author and discuss two wellness nursing diagnoses that
It can sometimes be too late. I have learned over some years by watching those that mean a lot to me let their lives become of nothing because they do not worry about their physical appearance or the foods that they eat, and even though you can give them all the advice to try and persuade them they are determined to do what they feel and what they want to do.
Overall, I was not surprised that my scores varied depending upon the specific category. I grew up in a household that mainly revolved around school and physically activities including sports so I was expecting to score higher on these two categories. This survey accurately represents what aspects of health that I focus on the most. I was surprised to see that my social health was the highest category because I am usually a quiet person but I have tried to talk more with people since coming to college. My family was never very spiritual or religious so I was expecting my spiritual health score to be lower than all the other categories. This survey is an accurate representation of my overall wellness because the scores matched my expectations.
This report will discuss the health assessment of a client who presented to my PEP facility, Clinic 275. Clinic 275 is a confidential and complimentary walk-in sexual health service which provides medical consultation/advice, testing and treatment for sexually transmitted infections (STIs) (SA Health 2016a). Ultimately, this paper will illustrate how an ongoing health assessment, history taking, provision of client education and care options of certain STIs are fundamental to guiding the planning, implementation and evaluation of care for specific people. All information regarding the client, who will be referred to as R, was permitted for use by the facility and will maintain confidentiality in accordance with the guidelines of the Government of South Australia (2015, p.7; Nursing and Midwifery Board of Australia 2016).
Format of the Instrument and Administration: The instrument format and administration type should be matched with target population characteristic. The level of literacy and administration type may affect the validity and reliability of the instrument.
Throughout this complete health assessment, I will approach my patient, a 49 years old, female, married patient, and perform a head to toe examination. Starting with the gathering of information, I will start with biographic data, reason for seeking care, present illness, past health history, family history, functional assessment, perception of health, head to toe examination, and baseline measurements. The subjective data will be collected first, where the patient will provide necessary information about every organ system for further examination while the objective data will be amassed in every system based on my findings. This assignment serves as an opportunity to establish a nurse-client interpersonal relationship that
