The Board office received a letter from Mary Fahey, MPHP via fax on 05/04/2015, advising that Dr. John Medley, M.D. a current “ILOD” status had informed her that he had relapsed over the weekend of April 25-26, 2015.
Dr. Medley had lost his grandmother on the weekend above and he began to drink and relapsed on alcohol. According to MPHP, the doctor had been struggling with the grief of the loss of his family member and realized he must take a leave of absence from his job and return to treatment at Pine Grove in Mississippi.
The doctor contacted his supervisor and advised him of the problem and his plans. There were no patient issues as he was off for the weekend.
Mary Fahey contacted the doctor’s counselor at Pine Grove and as of the admission date, they recommended a 6-8 week

INTERVIEW OF DR. JOHN H. MEDLEY, MD
Dr. Medley met with me at approximately 11:00 am on July 14th, 2015 at the office of MPHP in St. Louis, MO. Present was the doctor, Bob Bondurant, Mary Fahey and myself.
I asked the doctor how long he was at Pine Grove and he advised 8 weeks. May 7th through July 2nd 2015. He has seen his Psychiatrist, attending more than the 3 weekly 12 step meetings required by Pine Grove and will be tested weekly for alcohol for 52 weeks. He was being tested 26 times a year.
The doctor gave me a fit to practice letter from Dr. Peter Kamp, MD Psychiatrist at Pine Grove and Leah Claire Bennett, PhD the Clinical Director at Pine Grove.
The letter is dated July 2, 2015 and states the doctor is cleared to return to the practice of medicine with monitoring under the MPHP. There are 6 guidelines and recommendations noted.
In summary, follow the guidelines and recommendations from MPHP; follow the monitoring contract with MPHP including drug screens that should include Phosphatidylethanol and Ethyl Glucuronide screens as well as periodic hair

Dr. Sinclair reports the choice for MDA medical director was promoted in Houston; a new search to begin. He says MDA creates challenges. Dr. Debbie Kuban is serving as interim medical director. Karen says defining affiliate physician private practice/flexibility on both sides being looked at. Dr. Sinclair says Scripps has more control than what may appear evident.

On 11/23/16 I met Mr. Westover and his daughter Jackie Syms. Mr. Westover has made good progress with the continued physical therapy. He has resumed driving locally. Mr. Westover reported he had been in the hospital for 5 days recently for pneumonia. He is not using his cane now unless it is slick outside. He reports still feeling weak in the left leg. The Neurontin did work but Mr. Westover reported he stopped taking it and feels fine. Per Ms. Syms Mr. Westover’s PCP has him taking the blood thinner again. Mr. Westover agreed to take it until January 2017. Mr. Westover asked to have his home physical therapy continue, he doesn’t want to drive on winter roads. The patient returned to the clinic as he was hospitalized at the Hartford Hospital from 8/10-17-8/14/2017. According to the patient, he was hospitalized due to cardic arrest and difficulties breathing. Denies any recent relapse. The patient reports that Hartford Hospital shared with the patient that Saint Francis Hospital had released him too soon as the patient still had pneumonia. The patient was not medicated during his 4 days stay, only recieved clonidine. Furthermore, the patient was prescribed with several new medications from the Hartford Hospital that hasn't yet been refilled. The counselor met Reynaldo for his scheduled individual session. Mr. Reynaldo denies any substances use this week. Reynaldo and I went over the AA meeting scheduled for the 24hour group in his community. Reynaldo agreed to attend two AA meeting by our next individual session. Reynaldo denies any HI/SI during the session. Reynaldo appears to be in the contemplation stage of change as evidenced by him continued to seek recovery. The counselor next individual session with Mr. Ventura is on Tuesday September 8, 2015 at 2:30pm.
