Member stating she was discharged home from Hanover Health and Rehab on 03/14/2017 with home health services in place. Member stating she had requested Encompass Home Health Agency but Bon Secours Home Health came out on 03/18/2017 and she stated she told Nurse she did not want that Agency. CM placed a call to Hanover Health and Rehab and spoke with Discharge Planner and she stated that Encompass could not take the case because of a shortage of staff. Member stated she would agree to have Bon Secours Home Health. Discharge Planner stated she would send the referral back to Bon Secours Home Health. Member stating she had post hospital follow up with PCP yesterday with no new changes. Member stating she reported the pain she is having to PCP
Mr. Mendivil was referred to the STAR CAST on January 1, 2018, by SGPRC due to his noncompliance with treatment and medical appointments. He is diagnosed with type 2 diabetes and is prescribed insulin four times a day. Nursing staff from Care Unlimited Home Health come to the house every day to check his blood sugar, check his blood pressure, and administer insulin as ordered. However, Jesus is reported to often refuse to have blood sugar checked and to have insulin shots. This noncompliant behavior to treatment poses a high risk for complications from poorly controlled diabetes.
A patient is admitted to your surgical center for minor surgery that involves a breast biopsy under local anesthesia. The surgeon has previously informed the patient of the surgery risks, options, desired outcomes, and possible complications. A staff member brings the surgical permit form to the patient for her signature. The patient readily states that she knows about the surgery and has no additional questions. She signs the form with no hesitation.
10:24-10:50 AM SC completed monitoring phone call with Pa on 12/24/2015. SC spoke with Tina Carroll Pa’s dtr and CG/DOC. Tina reported that Pa is getting worse and she is just doing the best she can. No falls, ER visit, hospitalization or medications were reported since last contact. Tina reported that Pa is satisfied with services and her agency and aides. Tina is requesting weekend PAS support. SC informed the Tina that SC will assess Pa’s needs that home visit in January, 2016 Tina agrees. Seniors with breathing issues are not able to take in as much air into their lungs as they did prior to their breathing issues. Further, the air that they do take in is not processed as efficiently. In the end, this means there is less oxygen getting to the cells. Less oxygen means less energy.
I believe each younger generation is the current generation future so we should teach our children right and watch them grow. We should support our children to the highest standard and let them know to never be afraid to want to know more. We should encourage our children to do their best and try their best at all times. I say this because our children need us they need us to let them know they are safe and that we will not harm them in any way. I want to know why and how some people could ever harm a child. Unfortunately somethings cannot be explained or figured out sometimes we just have to live without knowing. Before, during, and after trauma we are supposed to be there to shelter our child.
