Attempt CC =1 CM was unable to reach Lisa Marshall (caregiver) in regards to services and upcoming CFT meeting for Alexia (youth). CM left caregiver a detailed message informing her that at today’s, Wednesday 4/12/17 CFT meeting caregiver has to complete a full Medicaid package, 3560 OOH application. CM noted caregiver must bring the following: copies of private insurance card, youth’s birth certificate and youth’s social security card. CM requested to be contacted.
On January 31st, 1969, Gail Miller had been on her way to work as a nurse, when she was raped and murdered in the snow. Meanwhile, sixteen-year-old David Milgaard went on a road trip, along with his friends Ron Wilson and Nichol John. They stopped to pick up their friend Albert Cadrain in Saskatoon. The man living in Albert’s basement was serial rapist Larry Fisher, who was then unknown to the police. He was responsible for Gail Miller’s rape and murder.
CM spoke to Theresa (FSO partner) regarding FSO services for the family. CM informed FSO partner that the family is interested in FSO services and the scheduled 30-Day CFT meeting is on 12/12/16 at 3:30pm in the family’s home. FSO partner reported that she would not be able to attend today’s meeting due to a schedule conflict and I she will try to get someone to cover the meeting.
Debra Kaylene McKee related she related she worked at Grillsmith at Brookstone, 2000 Piazza Avenue #100, Wesley Chapel, Florida 33543, from 1110-1500 hours. McKee said she then went home and then went to Ybor city about 1730 hours and went to the Bad Monkey Pub at 1717 E 7th Avenue, where she had one “Johnny Vegas” on the rocks. McKee related around 1800 hours, she went to the Carne Chophouse at 1536 E 7th Avenue, where she had dinner and one “Johnny Vegas” to drink. After diner at Carne, McKee related she went back to the Bad Monkey around 1930 hours and had one “Fireball” to drink. McKee said she left the Bad Monkey about 2030 hours and drove to Chipotle at 12827 N Dale Mabry Highway. Mckee related she left the Chipotle about 2215 hours

Client also mentioned she signed up with Brightpoint Health Home Health Services since 5/9/2015, Client report she will like to sign up with CAMBA/Home Health and she provided BrightPoint Home Health approval letter for CM to submit to CAMBA/Home Health Coordinator. Client is waiting for her coordinator at Brightpoint to return from vacation to close her case, so that she can sign up with CAMBA/Home Health
On Tuesday July 7, 2015, at approximately 3:01 PM, Kiana Beekman, (MFCU Investigator) (Beekman) received a call on the state office telephone from HILL, Lucy (Service Facilitator of Lucy Hill Services (LHS). During the conversation, Beekman asked HILL to clarify her role and responsibilities as a service facilitator, in addition to the role and responsibilities of HARRIS, LaFrance as the Employer of Records (EOR) for Medicaid Recipient DANIEL, Rose and MCGHEE, Inocencia as DANIEL’s aide. She was also asked to provide any documentation of training on timesheet submission and approvals that she provided HARRIS and MCGHEE under the Department of Medicaid Services (DMAS) Consumer-Directed care aide program.

On December 1986, Nora Priscilla Lindsay made a Will with the Public Trustee which was then later revoked. Nora Lindsay was divorced, had two children, a son Geoffrey Lindsay, who is the appellant, and a daughter Heath McGrath, who is the respondent, and died on 16 October 2012. The appellant believed the deceased died without a Will and therefore applied for the Letters of Administration. This was then granted on 17 June 2013. However, a five-page hand-written document was then located. Following this, the appellant brought before the primary judge that the document was the subject of the application for probate. My intention in writing this paper is not to point out Lisa Lowe’s failing topics, but to broaden the discussion that she initiated by including my knowledge and cultural point of view as a Luso-American. I want to make an effort to include my single cultural contribution to her initiative. I would not support Jennifer Hall’s proposal to add a dollar surcharge to every traffic fine to pay for the college education of police officers. Like Senator Maggie Oates, I believe the proposal results in special treatment of police officers as it is something that is not offered to other state employees. Police officers’ opportunity to obtain a college education should not rely on people getting stopped. A direct gain from each traffic fine could result in police officers giving out fines for violations they would have otherwise given warnings for. Lastly, fines are costly enough that adding a surcharge sufficient to cover all employees would be too large and have a significant impact on the community.
It is a great pleasure to recommend Jenny Simpson for admission to your Master of Arts in Curriculum and Instruction program. During the previous academic school year, I had the privilege of coaching and managing Jenny.
