At 10:18pm on Saturday, December 3, 2016 RA Tyler Shaw was on his round on the second floor of Hanby when he was going to confront a noise violation in Hanby 220 when he heard a person in the room chanting, “chug” and could smell the alcohol emanating from the room. RA Shaw then contacted OPD and RA Shania Zeigler for backup since there was another alcohol violation on the floor. RA Shaw waited to approach the situation until backup was present. While copying their information, the guest of the room were saying that they were not drinking in the room, but they were drinking off campus. They were all saying that Hanby Resident Heaton was the only one drinking. While in the middle of this process, Hanby Resident Heaton returned to the room and we proceed to collect his information as well. While RA Shaw was talking to everyone present in the room, RA Shaw saw that Hanby Resident Heaton was acting a little strange. Hanby Resident Heaton was not giving direct eye contact, not speaking to his guest, and when OPD Office Frazee was asking him questions Hanby Resident Heaton was not answering some of the questions. After RA Shaw presented Hanby Resident Heaton and Garst Resident Hann with the red cards, OPD Office Frazee ask the guest to leave Hanby Hall. The guests of the room then proceeded to thank us and to shake our hands. RA Zeigler decided to contact ADRL Jean Barnard once again to clarify on what we should do. ADRL Barnard said not to search the room for alcohol, only document what they see. RA Shaw, RA Zeigler, and RA Bokar found a case of 24 12 oz. bottles of Miller Lite, with five in the case and two in the trash. On the floor and in the trash we found 11 cans of Natural Lite beer. Upon our arrival, the resident, Mark Denote requested that his visiting girlfriend, Suhail Lavecchia be removed from his home. The two intoxicated parties were separated and interviewed. Mr. Denote explained that the two became accidentally separated at the Philadelphia Eagles game this evening and returned to the apartment separately. At three o’clock in the morning, Chris Salyer (109) and I discovered Steven Strominger (117) drunk being walked home by Morgan Hall from Davis (232). According to Steven, he fell asleep outside on a bench and Jordan Hawkins told him to come inside Davis. Somehow, Steven made his way to the second floor and knocked on Morgan’s door. In chapter 1 of Sister Citizen titled Crooked Room author Melissa V. Harris-Perry illustrates a study of subjects placed into a crooked room. The subjects are then asked in a variety of scenarios to attempt to re-align themselves. The results came as much of a surprise to the researchers as what the subjects perceived as “straight’ was heavily skewed. Harris-Perry used this crooked room example to support as well as confront the gender and race stereotypes that black women still face in society today.
The reporting party (RP) stated on 4/1/16 resident Clifford Soares exited the building and was found by the local Police Department and returned at approximately 2:00AM. According to the RP the resident lived in the memory care section of the facility. When the resident exited the facility the alarm didn't sound or the caregivers did not hear the alarm sound. The RP stated the facility failed to secure and monitor residents in the Memory Care Unit.
The reporting party (RP) stated minor Anthony Silver DOB: 6/19/99 resided in the facility until when he was given notice on 10/4/16. According to the RP, Anthony went AWOL from the facility on that date. While being transported into custody to Juvenile Hall he disclosed that staff members brought drugs and alcohol into the facility. The RP stated she spoke with the minor privately regarding his allegation and disclosed several staff members were involved in bring in pills, tobacco, alcohol, and meth into the facility. When the minor was asked which staff members, he replied he didn't know and could not remember. Consequently the RP continued to speak with the minor and he revealed the names of Gary Stanley, who no longer works in the facility.
It was reported that on 10/10/16, by resident Darcia Getty (age 53, DOB 07/08/63) and resident's sister Donna Cowell that the facility has not provided the resident with her medication. Per ED physician note dated 10/10/16: "Woman" at the the board and care would not give the resident her medication until she ate her oatmeal. It was also mentioned that the resident has wondering thoughts and hysterical at times. EMS reports chaos at the scene, the resident was throwing things and screaming. LPA contacted RP to verify the name of the resident and original reporting party, as the name on the narritive did not match (Ms. Reed was a misprint).
I was asked to meet with Administrative Nurse Supervisor Debra Reilly. I, Steven Evans conferred with the Nurse Supervisor about what transpired in room #401 and what was reported to her. Nurse Supervisor Reilly felt that the problem was a grave security concern for the patient’s nurse and the entire medical staff. She advised the problem should be reported. She contacted the (CFRC) Central office and spoke to a Correctional Supervisor about the issue. I did not make contact with the inmate patient’s Nurse in room #401 and was therefore unable to obtain a written witness statement from her. This report was generated, pending further investigation. Reporting party (RP) stated that she wishes to remain anonymous and is a social worker. RP overheard staff Patricia Salvo stated that there is not enough food to give the residents. RP stated that the staff and residents are afraid of the caregivers (Emma, Aziza and Lara). RP stated that staff is afraid to tell Emma that there is not enough food for the residents for fear of retaliation. RP stated that resident Fernanda and Cheryl are too afraid of Emma to file a complain on their own. RP stated that staff yell at the residents and tell them, "If you don't like it you can leave." Upon arrival, I noticed an older gentleman sitting in Desk B lobby area. The man was later identified as Veteran Thomas Keller. I contact RN XXXXXXX, who stated that Keller was intoxicated and causing issues. I then contacted Keller who, stated he was here to get a cain. I noticed an odor of alcohol emitting from his person. I then asked OFC Massey to get the PBT so I could check his level of Intoxication. After Massey left, Keller attempted to leave, I informed him that he was been detained. He replied “I’m fucking leaving, you can’t hold me”. Then He asked “what are you arresting me for?” I replied that I could smell and odor of and alcoholic beverage coming from him. Keller then stated “Hell yes I’ve been drinking it’s still a free country”.
Hany did not return my call. He had questions about his SNAP benefits. By checking DHR and TRACS, he already was in contact with the Branch workers. He has questions about how his income was calculated and he was informed was used the NC2 Non Citizens. He understood. (TRACS # 216)
The reporting party (RP) stated resident Arev Dabbagh DOB: 12/27/00 disclosed on 4/15/16 she inappropriately touched by a male peer. According to the Arev on 4/14/16 a peer Ibraheem Mamoori DOB: 9/2/98) placed his hand on her shoulder and continued downwards towards her breast. The peer touched her breast over he clothing. The resident was distraught while she recounted the incident. Arev alluded to their being other incidents with the peer and other unknown males. Arev didn't disclose details or clarification. Arev was placed voluntarily in the facility by her mother and will continue her placement for a few more weeks. The peer is currently placed on a 5150 hold and will not have access to Arev in the future.
The group walked from a condominium complex off campus to get to Triad, an on-campus residence hall where the party was located. On their way there, the group was passing a plaza with the XPress Mart around 9:13 p.m. It was near the convenience store that Farrell started lagging behind the group. Four to five men came up to Farrell to ask where they could find a party. As they talked to him, two of the men attacked him. Farrell’s friends and bystanders called 911 when they realized what was happening.
