When worker arrived worker was greeted at the door by Josilynn. Billy does not allow hitting in his home. Josilynn is now grounded. Billy stated there is still troubles at bedtime. Both are having behaviors. Mostly it is with Sarah, when Billy intervenes the children go to sleep. Both children know Billy means it is time to stop. There are no issues in school. The department has talked with Mr. Joey Beaird, Mrs. Julie Johnson, and Visiting Angles. The children are not willing to move into the family home 24/7 and assist with Mr. and Mrs. Beaird's care. Ms. Johnson is afraid of her father and does not believe it would be a safe environment for herself or her mother and Visiting Angels have stated if Mr. Beaird returns home they will be required to stop services due to fear of Mr. Beaird. currently no one is willing to care for Mr. Beaird at his residence and Mr. Beaird requires assistance with activities of daily living and 24/7 care.
Living with her mother; however her mother asked her to leave the premises. Case Manager asked client, why her mother asked her to leave. Client stated that they got into an argument. Client stated that they have been arguing on a daily basis. Client stated that she doesn’t want to raise her child in that environment. Client stated that she has being living with her mother all her life.
CPSW did a home visit to meet with Ms. Berner and to discuss about her safety plan since the children are moving with her on10/29/16 from the foster provider. Ms. Berner was late for her appointment and CPSW waited a 40 minutes for Ms. Berner. Ms. Berner apologized for being late. CPSW explained Ms. About safety plan. Ms. Berner understood and she signed them. Ms. Berner stated that she will be doing a house arrest for two weeks and the recommendation of her criminal court is to obey law and continue taking her medication on time and seeing her therapist. Also, cooperating with her PO and CPSW. CPSW consulted with the supervisor and she has approved both children to move back with Ms. Berner. Goal 1-2
On 9-15-17, Adrian and Len were standing at the stop sign, on a curve; waiting on their school bus around 7:15 am. The stop sign is not their destination pick up and drop off. Adrian and Len were unsupervised. Adrian and Len were out of eyesight from home. The home is around two curves. The Family Protection Specialist (Tanya) was in route to picks Adrian for his doctor (unknown) appointment on 9-15-17; when she said the two children at the stop sign. Due to the children's foster mom (Juanita) was unable to take Adrian to his doctor appointment. Juanita was asked, "Why are Adrain and Len were standing at the stop sign, unsupervised?" Juanita stated that: "They are the second grader and are older enough to be there; because they are in second grade." It is unknown what is meant by this. Adrian and Len were asked: " How often do they are left alone at the stop sign?" Adrian stated that:" Sometimes someone (unknown) is here and sometimes they do not."
On this date, Matthew Mitchell APS supervisor conducted a meeting on behalf of Mr. and Mrs. Beaird well-being. In the meeting, Mr. Mitchell presented Mr. Joey Beaird and Mrs. Julie Johnson with solutions that would allow Mr. Gail Beaird to return home. However, both Mr. Joey Beaird and Mrs. Johnson declined to take care of their parent independently. Each stated their home was not handicap assessable and they had jobs and could not dedicate that much time to their parents care. The other option discussed was hiring another service besides Visiting Angels since they are refusing to continue services if Mr. Beaird returns home. However, this is the only service offered in Jasper. When the discussion of Mrs. Johnson and Mr. Beaird hiring sitter independently they stated they had tried this before and it never worked out.
The boys dropped the spinner and it would no longer spin. Tamyia became upset and began to play on mom's phone. Lmayia spoke with the boys about their foster mom. The boys report she is mean. Worker stated the foster parent is not mean, but strict. Lamyia reports that she does not want the foster parent to be strict with her children. A, B) MSTT met with the caregiver to examine how she plans to increase the supervision and monitoring of the youth while out in public. Caregiver expressed while the youth is at home she is going to speak with her mother and his aunt to help while she's at work. MSTT encouraged caregiver to reach out to the school and ask them if they would be able to supervise and monitor him. Caregiver contacted the school and is waiting to hear a response. MSTT will continue to work with the caregiver in increasing the supervision

Mother shared that client talks back and doesn’t follow directions at home. Client shared that he doesn’t like to be the only child doing chores at home. UCM: CPSW did a home visit to Ms. Brenda Borkovec's home. Both children were in the living room playing together. Also, Ms. Brenda was in the living room holding Aydden and feeding him. Ms. Brenda stated that she has not received any papers regarding her foster care license. Ms. Brenda stated that she is frustrated with the process and that it has taken a longer time. Ms. Brenda mentioned that it has taken a long time for her to complete her fingerprint. She reported that currently she is waiting the background study and fingerprint forms to be sent to her. Ms. Borkovec's phone ring and it was Elizabeth Borkovec. Ms. Brenda asked CPSW if Elizabeth can come and join the meeting. CPSW approved it and Elizabeth Borkovec showed up less than 5 minutes.
Carl explain the dynamics of the family and how important it was to have Don at the initiation of the family session, and asked the family to think if they wanted to be a part of the family they are in, and if they did to call and set up a time for the next session with Don include. Dave did not hesitate and agreed to set up an appointment then and there; he looked over at his wife and asked he if she agreed. Carolyn agreed and towards the end of the session Carl connected with Laura the youngest daughter by engaging her in dialog and asking what she thought about everything that was going on in the family. By doing this Carl was able to show the family how every member of the family has a valuable place in therapy, and it is not just about one person.
Kathy and Raymond have visits one day per week for two hours per day. Worker supervises these visits once per month. The first visit this worker will be supervising is on November 23, 2016. These visits occur at the Godfrey, Illinois McDonalds play place, but were taking place outside in the community prior to this when the weather is nicer. During the last supervised visit the case assistant observed Raymond reading the newspaper. The tried children interact with the case assistant; the case assistant redirects the children to interact with Kathy and Raymond. This is where the conflict between Carolyn and Nick started. Carolyn said yes and Nick whom decided not to be supervisor earlier said no. Nick stops the resident from going outside and asked the resident to sit and talk; the resident sat down and attempted to listen to Nick talk about why his answer was no. The resident was becoming agitated and got loud with his words; he started cussing and then becoming aggressive. The other staff members and Carolyn went out to the living room to comprehend what was going on.
He walks around with a hard on. Ja’Kevion alleged that Tray who is 4 or 5 years older, messed with him when he was 4 years old. The incident happened one time. Tray stays in Wilkinson County and has MDDD. His mom name is Stacey and the reporter stated Marion would have more information for that family. Mom said Ja’Kevion told her about the incident and she told him not to go around Tray. She did not report the allegation. Reporter stated mom does not leave the children home alone with each other. She makes Ja’Kevion sit by himself and she keeps the younger children with her. She is afraid to leave the children around Ja’Kevion. Reporter feels there is something wrong with Ja’Kevion. He appears depressed. He look like he has a lot on his mind. At school last year the reporter noticed he did not socialize with the other child and he was to himself.
