On 02/26/2016 client Gregory Spencer came to the SSO for a House Meeting with HS De La Torre. The purpose of the meeting was to develp a Housing Plan and explore other possible housing opportunities.
Family arrived on time well dressed and groomed. Ms. Spencer informed HS that last permanent address was located at 2075 3rd Ave. New York, NY from August, 2014 to May 2015. As per Ms. Spencer family could not stay anymore. Family did not have any other option that go to the shelter system. The family composition consists of GREGORY SPENCER (HoH, 33y); TISHAWN SPENCER (legal spouse 39y); NEPHATANI SPENCER (child, 7y); SHAWNTI SPENCER (child, 5y); GIOVONNIA SPENCER (child, 3y). Furthermore, Ms. Spencer stated that two children Naquarry (17y) and Jashawna (15y) will move with the family if permanent address is found. Also, Ms. Spencer is not pregnant, and any adults in the family has been convicted of a crime or parole. Family stated that they feel secure in the premises.

Family has not been able to view any apartment. Ms. Spencer and CoA stated that they called and brokers and LLs are not taking any program. During meeting HS reached out to LL Michael who has a 4 bedroom located at 1068 Intervale Ave. Bronx, NY. However, Michael is looking only for a family of 5 members. Other brokers were called and they do not have any apartments available. Furthermore, HS brief family about the other subsidiary vouchers. Both adults stated that they want to utilize the Section 8 voucher. Family is not interesting on moving out of State. Lastly, HS provided client with a Broker’s list, housing search log, apartment review checklist. Also, HS reminded client to provide any updates to HS or CM and client must complete Housing Search Log and Apartment Review Checklist on weekly basis. On this date worker received a call from Tori Pettit. Ms. Pettit stated that Serv-Pro had visited Ms. Smith's home did an assessment and sent off the mold samples. The results came back that the house is not livable due to mold infestation. The mold is in the walls, ceiling, basement, and air vents. Ms. Pettis has spoken with Ms. Smith and Ms. Rachel Rodgers about Ms. Smith moving ASAP. Ms. Pettit stated that Tri-County could place Ms. Smith into a group home now due to her home being deemed unlivable. However, Ms. Smith is not interested in-group home placement and would rather live independently. Ms. Rachel Rodgers has agreed to assist Ms. Smith with apartment applications and any other thing need in order for MS. Smith to find residence.
SOCIAL SUPPORT: Client continues to complain why DHS didn’t submit her housing package to this shelter. She also states that this worker is mental health and she doesn’t have any mental health disorder to be placed in a SRO. Client also threatened to this worker to call 311, DHS advocate and the Coalition. As she quotes:” is not about you or CAMBA, I am doing this because DHS have all of my information”.

This letter is in regard to Zynaria Deas and her placement with SAFY. Zynaria struggles with communication, complying with the rules of the home and community, and display of verbal and physical aggression towards the foster parent. Due to Safety concerns Mr. and Mrs. Cassandra & Thomas Bryant decide to submit a 30 day notice with the agency to have Zynaria removes from the home. On 06/24/2016 HS De La Torre met briefly with Ms. Bethune for a follow up with any housing leads. As per client it has been difficult to view apartments. Brokers are not taking her due to lack of stable income, poor credit. Client stated that she viewed a condominium located around 141 Street and Atlantis Ave. New York, NY directly with a Landlord who is willing to assist her. Client did not provide any information regarding the Landlord. Client is concern about the fact that the apartment is a condominium that DHS will not approve the apartment. HS reached out to DHS for a correct follow up, HS is waiting for a response.
Ansonia asserts that the decision to reject Complainant’s application based on his household size (four) for a one-bedroom apartment was correct and was consistent with the company’s policy and the law. Ansonia states that its follows the United States Department of Housing and Urban Development (“HUD”) occupancy policy of “two-persons-per-bedroom” as outlined in both Handbook 4350 and notices published in the Federal Register on December 18, 1998. This policy is acceptable and reasonable under the Fair Housing Act and consistent with and in compliance with the City of Chicago’s Municipal Code in rejecting Complainant’s application.

The family consumes a nutritionally rich diet including adequate portions of fresh fruit and vegetables. However, their intake of processed food such as sausages and sandwich meat is a cause for concern. Children are also allowed to have nutritious snacks twice each day. Since Mr. Schaefer suffers from weight issues and high blood pressure, his wife refrains from keeping salty snacks at home. They have family gatherings almost every weekend when they indulge in cocktails and beer. The daughters are only permitted to drink juice at such gatherings. Her story primarily focuses on her family relationship issues being that she neglected her family for her friends and other aspects of her life. This case study explores the relationship transition from family first to friends first as Myra went from childhood to adolescents. Like many adolescents, Myra’s shift is further driven by the newfound independence of adolescence, However, her desire for independence and means to gain it may have been more extreme than the stereotypical adolescent, causing turmoil in her and her family’s lives. Her story also analyzes the concept of family and the roles that each member plays within the unit. Eventually, Myra tries to strengthen her relationship with her family during her college years but wonders if it’s too late because the damage of their roles is already done.
Client presented a pleasant and patient demeanor. Her behavior is acceptable and in accordance with the DHS rules and regulations.
