2. WHEN STOPPED
After arriving on location, I approached from the driver side and made contact with the male, later identified as Steven R. Dennis. I advised Steven the reason I was called out to the scene and I asked what his whereabouts of the night where. Steven stated that he was over a friend’s house and they were hanging out and that he consumed only two (2) beers. As we were engaged in a conversation, Steven was extremely slow with his responses and his eyes were bloodshot. As he was speaking, I could also smell an odor of an alcoholic beverage emanating from the interior of the vehicle. I then asked Steven to step out of the vehicle so I could administer SFST's. Steven complied and opened the door. He then stepped out and nearly fell. He used to left hand as balance as he rested it on the roof of the vehicle. I also observed the Steven was in a soft cast on his foot from what he called an "Achilles’ injury." I advised Steven then walk towards the front of my vehicle so I could speak with him further. I asked Steven again how many drinks he consumed tonight.

After Steven advised that he did not, I explained the instructions to him, which he advised that he understood. I began the test the Steven moved his head two (2) times before he finally followed the instructions. I did observe a lack of smooth pursuit in both of his eyes. There was a distinct and sustained nystagmus at maximum deviation in both of his eyes. I did observe nystagmus prior to forty-five degrees in both eyes also. After completing the test, I asked Steven if he could stand on one leg, either leg since he had a leg injury. Steven advised that he could not put any weight on it without his crutches. I advised Steven that I could not administer the rest of the rests due to his leg injury. I advised Steven to turn around and place his hands behind his
Case if he would do a field sobriety test. Mr. Case agreed. Deputy Akins assisted Mr. Case to the flat part of Centennial Road in front of the squad. Deputy Akins asked Mr. Case 3 times if he had any back leg or knee problems that would prevent him from doing the leg stand. Mr. Case replied no and stated he would do it. Deputy Akins instructed Mr. Case to stand with both feet together and his hands straight down to his side and not to start until told to do so. Deputy Akins demonstrated the one leg to Mr. Case and asked him if he had any questions. Mr. Case replied he understood. At that point he was instructed to begin the test. Mr. Case began immediately using his arms to balance. On the count of 7 he placed his foot down got his balance then lifted it again and continued. Between 11 and 20 seconds Mr. Case put his foot down again and continued to use his arms to balance. Mr. Case lost count at the 17 second mark. Between the 21 and 30 second mark Mr. Case put his foot down again and miscount 3 additional
S: "I got dizzy and lightheaded." Subject stated he was on hi way home to Kansas City when he took a surge energy drink. Pt attend after that he got lighthead and dizzy so he pulled over. Pt stated at that time he started having anxiety problems and hyperventilating and his hands began to cramp. Pt stated he called his mom who called 911. Pt stated he felt better after he claimed down and got his breathing under control. Pt states he rarly drinks caffeine and avoids coffe or any type of energy drink. Pt states his mom gave it to him for the long drive home. Pt denies any current s/s and states "I feel fine now." Pt states, "I will be fine and don't need to
Your affiant interviewed C1’s attending doctor. IT reported that IT visually examined C1’s injuries. The injuries were appeared to be minor. IT further reported that C1 was under influence of alcohol and pending other medical testing report.
In summary, on 09/26/16 at 2220 hours, I assisted Ofc. Drakulich #255 at 3113 S 54th Ave. in regards to a traffic stop. Upon arrival, I made contact with the driver (Lopez, Cristian D.O.B. 08/09/93) of the vehicle (14' Dodge IL IL E822075). I observed signs of impairment on Cristian, he had red, glassy, and bloodshot eyes. While I spoke with Cristian, he had a strong odor of an alcoholic beverage and cannabis emanating from his breath and person. Cristian related that he smoked cannabis earlier, but was unable to provide an exact time. Cristian would repeatedly give the response of "yes sir" or "no sir" without being asked any questions.
Horizontal Gaze Nystagmus (HGN) test - I conducted the Horizontal Gaze Nystagmus (HGN) test on Abraham. He related he does not wear contacts or have any problems with his eyes. I observed Abraham to have equal pupil size with a slow reaction to light and equal tracking in each eye, so I continued with the HGN tests. I gave the instruction and Abraham stated he understood them. Abraham had no clues during the test.
While on duty, I observed a burgundy Chrysler 200 turning left on to Elm Street off of George Wallace drive. The vehicle failed to signal. I performed a traffic stop on the vehicle with the final being on Gibb Street/ Charles Henderson Middle School exit. I approached the vehicle and asked the driver for his drivers license and his insurance. The driver gave me his drivers license and his Blue Cross Blue Shield Insurance card. I then asked the driver for his vehicle insurance and he advised that the Blue Cross Blue Shield card was his vehicle insurance. The driver was identified as Lawrence Tyler Mcclellan(18YOA). While on patrol, I observed a silver passenger car traveling on Aster Ave with an expired tag as of 2015. I performed a traffic stop on the vehicle, with the final being in front of 516 Hubbard Street. I asked the driver, Carrie Henderson, for her drivers license. Henderson then advised that she didn’t have one .Henderson also advised that her license was suspended. Upon my first approach, I then smelled a strong odor of an alcoholic beverage coming from her expelled breath. I then observed a open beer can and a cup of liquor sitting in the cup holder. Henderson did state that she had drunk 5 beers earlier. I asked Henderson to perform the following test: HGN, Walk & Turn, One Leg Stand and Finger Count Test. Henderson stated that she did have medical problems but she was not physically

My schooling began in Cali, Colombia. At the age of two, my mother matriculated me at local nursery because she worked in the daytime. When I was four, my mother enrolled me in a non-religious private pre-school. The school was known for having a strong program in math and literacy. In my country, education is mandatory until middle school. My homeland uses an alphabet and similar to English it goes from left to right. In pre-school, I remember singing the national anthem every morning. I would stand up alongside of many other students and with our hand across our chest we would sing the entire national anthem. I also remember doing a lot of hands-on activities like counting the elements of each set and making a line to the corresponding number
When I was younger I had always been described as mature. Although I wasn’t anywhere near being “mature”, it was a word always used to describe me, well-mannered and mature. While my brother was goofy and social, I was shy and serious. We were twins yet total opposites. As a child, because that was the word almost always associated with me, it crae unusual, almost toxic idea about myself. I had to be mature to be what people liked about me. So, it never occurred to me to be able to not take myself seriously and say something like “Oops that was dumb” and laugh it off. For some reason that didn’t make sense to me to say silly things like that.
