Per Reporter: Elaine tested positive for drugs (marijuana & meth) April 2017. Elaine tested negative via UDS on admission. The baby teste negative via UDS 8/7/17; the meconium drugs screen is pending. Elaine received very little to no prenatal care due to no transportation. Elaine stated that she hadn’t used drugs since April 2017 and still without transportation. Elaine mentioned that she has a second child (unknown) who is in Iowa with the maternal grandmother (Tracy). There are concerns that Elaine doesn’t have custody of the unknown child. Elaine mentioned that the child is visiting, but did not disclose how long the child had been in Iowa or when the child will return. The family is schedule to discharge 8/9/17. The client arrived on time for his scheduled assessment. CPS caseworker Danita Brown referred the client test positive of illicit substance. The client is 30 years old African American male who is not married. He is the father of 3 children ages 1 month old 1, and 5 years old. He is currently in a relationship with two of the children mother. The client reports there was incident of family violence. The client reports his girlfriend was the one to assault him. Brent reports that he works at Sam Clubs in Pearland and has for the last 3 years. Brent reports three years of colleges. The client reports growing up with his parents, however they divorce later, but they both are supportive. Brent denies any medical or mental health concerns. The

The reporter stated that Arlena delivered Aubrey in TX at the age of 16 and both mom and child tested positive for cocaine at the child’s delivery. The reporter stated at that time Aubrey was taken in to TX CPS custody and placed in the care of her paternal aunt. Ms. Lowry is unsure who has custody of the child at this time, but believes Arlena may be running from TX CPS or law enforcement. According to the reporter, Arlena hops from state to state with the child and is in between TX and MS often. The reporter stated Arlena goes between the states from week to week. Per the reporter, the child does not have Medicaid, WIC, she doesn’t have formula and her mother gives her any kind of milk she can get, her shots are not up to date, and she has not been to any well baby check-ups.
The birth mother was brought to the attention of the DCFS in regard to her sons, Caleb and Brayden Lane. She left the home she and her sons shared with her mother for extended periods of time, without giving her mother permission to consent for the boys' medical treatment or attending to paperwork regarding the boys' financial supports. The birth mother was twice asked to submit to an urinanalysis by the DCFS, which she refused; she indicated that she has been smoking marijuana on a regular basis for the past six months. The birth mother reportedly failed to return to her mother's home on the 14th and 19th of March to provide care for her sons as requested by the DCFS. REPORTER: The reporter/Counselor (Marilyn) called with concerns for the victim, Jerrik. The reporter said the victim came to school late today (8/28/2015), but he was fine. According to Marilyn, the victim was messing with his arms and the teacher (Tabitha) asked the child “what was wrong”, and the victim had bruises on both of his upper arms. The bruises “looked like finger prints” (were not dark marks). After the victim showed the teacher his arms, he then said “look” and “pulled up his shirt, “there were two big bandages covering up the burn marks” in the middle of his back. When I attended the Terrace Child Development Centre, I witnessed one therapy session with a 7.5 month old who was born 5 weeks earlier. This child had two prior sessions with the occupational therapist to work on independent sitting, and rolling over. The therapy session for that day was about getting the infant to roll over properly and trying to get the infant to start crawl. The infant would roll over with a wide stance of legs and arms, using the arms and head to roll over, and not using the hips or legs. The occupational therapist showed the parents how to properly help the infant to roll over by getting the infant to roll with the leg and hip first with the use of the parent’s finger to guide the hip to roll over. Given these points, this was a very informative journal article. The authors effectively catered to the appropriate audience and education was provided with the different models. Moreover, the authors proved their hypothesis, discuss the methods in great detail, and provided missing discriminatory analysis. The authors also allowed the reader to see a clear picture into the emotions and discourse experience with Mixed Orientation couples. As a matter of fact, the authors also gave advice on ways for the therapist to explore issues experienced by the couple. INVESTIGATION: On 09-18-2015 at 0830 hours, I was notified by Corporal Cherup # 1117, of a traffic crash with serious injuries that occurred on Madison Avenue, between Dacca Drive and Palm Drive on 09-17-2015 at 1955 hours. I responded to the crash scene and upon arrival at approximately 0905 hours I met with Detective M. Carter # 181968 and Deputies Thomas # 251327 and Maiello # 235946. The Deputies advised me as to the approximate positions on the roadway of the evidence, pedestrian and Vehicle 1.
