On 11/21/2016, at about 1421 hours, at the Durango Jail located at 3225 West Gibson Lane Phoenix, Arizona 85009, Inmate Knops, Anthony T309345 arrived to medical to see CHS Staff member Myriam CS298 for pain in his right side and back. PA Vanschill CH114 ordered Inmate Knops to be sent to Maricopa Integrated Health System (MIHS) located at 2601 E Roosevelt St, Phoenix, AZ 85008 via ambulance to rule out Rhabdomyolysis. At about 1629 hours, Officer Gonzalez B3269, FTO Ranger B1958 and Inmate Knops departed Durango Jail via ambulance AMR #102 and arrived at MIHS at 1710 hours.

West County Detention Jail Facility is located in Contra Costa County (5555 Giant Hwy, Richmond, CA.) and it is the primary jail for the county. The prison has a capacity up to 1,104 inmates. There are five different housing areas, one for females and four for males. Inmates serving time in this facility are either awaiting trial or have been already sentenced in the Contra Costa County Court System for a period of time of one year or less. Opened in 1991, WCDF is the newest of the detention facilities in the County of Contra Costa.

Officer Perry and myself arrived on scene and found Officer Isett out with Kellen Grewell and Annie Tracy. Tracy was in custody and in wrist restraints. Grewell was standing by the motorhome that they both are currently living in. The motor home (WA 611XAR), is listed as Tracy’s address with DOC (where ever it is parked).
I placed my firearm, OC, and knife in the lock box at the jail. I gathered the paperwork I needed from the vehicle then Sgt. Garwood and I escorted Mckenna into the jail. Upon arrival at 2328 hours Security met with E.D. Charge Nurse Johnathan Bacal who stated that there was a combative male patient being transported by the Orange County Fire Department. At 2330 hours, the patient, Alan Castillo (DOB: 08/03/84; Fin #86501337) was brought by Orange County EMS (Engine #83) with an escort from Orange County Sheriff's Deputy. Secondly, I mailed my Chinese version of my new form I-589 to a wrong address of my current attorney. Then, I was under tremendous pressure mentally and financially, plus weather and new environment, I got very sick during the period between the end of 2015 and February of 2016, so I was unable to get 2nd Chinese version of my new form I-589 to my current attorney until sometimes of March or April in 2016.
