He explains that Mr. Hart has not been attending services for the past several months; he’s heard that Mr. Hart underwent some type of surgery and would like to know what it was, and he would like to go visit him, but Mr. Hart moved recently and he does not know his new address.
Would you release the information in question? No
Rev. Smith, I understand your concern for Mr. Hart and your desire to visit with him. Unfortunately it is against the law for me to release Mr. Hart’s personal information. Also,
HIPPA as well as our state statutes mandate that I protect Mr. Hart’s medical privacy; therefore I am unable to confirm whether Mr. Hart is a patient or provide any update.
I would suggest that maybe you could contact someone else in the congregation that may be able to provide you with his current contact information.
Scenario 4
Deputy Sheriff Parker arrives in your office requesting to see the records of Sally Jones. Sally was involved in an altercation at a local establishment last night. Deputy Parker does not have an authorization, search warrant, or subpoena.
Would you release the information in question? No.
Why, or why not, and how would you proceed? It would be unlawful to release Ms. Jones medical information without the proper documentation to authorize the release. I would explain to Deputy Parker that he would need to obtain a signed authorization from Ms. Jones or obtain a subpoena that specifies what

John Doe is an individual that left his country in an effort to make a better life. However, he does not have legal status in America and was recently arrested for shoplifting merchandise, which was valued over $1,000. The other bit of news is that Mr. Shufner has started doing street corner preaching with the bible. I never knew him to be religious, but considering where his life has headed these past few years it’s his only means of comfort. You’ll usually find him in the Safeway plaza with a small crowd handing out fliers. I went to approach him the other day after getting myself some meal milkshakes, and he seemed rather… distant. He used to be pretty friendly towards me before, but now he’s acting like I’ve done something wrong to him. Rather than engage me in conversation, he starts to recite verses to me with the sternest of looks. Maybe I have a bad habit of getting under people’s skin? Maybe he’s still bitter about Robin’s untimely demise that he’s taking it out on me? It seems a bit petty, but he hasn’t been right in the head since the tragedy. As I sit here at my desk typing up this article, regarding Shufner 's small Christian healing seminar I try to figure a way to approach him for the next part of the article. He didn’t seem interested in promoting it out loud, but randomly approaches people off the street and offers them free prayer and healing. He’s also started a Christian get a way program, he takes people out to an old isolated church town

Last month, I made a telephone call to a man I’ve known for more than 20 years. For the purpose of this article, I’m going to call him “Luke.” I hadn’t seen Luke for several months and I wanted to see what was going on with him. Before he disappeared, I saw him at least once a week in the adoration chapel.
