Reporter is call from on the child's physician's office. Damonei started to see the physician at that clinic in October 2016 and they have only seen him 3 times. He has missed a number of appointments with them and other speciality providers. The clinic where the report is does care coordination with children with complex medical condition. Damonei has a number of diagnoses including prematurity, infantile spasm, seizures, gastrointestinal tube dependent, and periventricular leukomelacia. He has other diagnoses that are less severe. He has missed a number of important doctor's appointments. In the past, the grandmother has gotten him to his appointments but she has had transportation issues. When they are seen at the Medical Mall, it's mom and the grandmother. Reporter believes mom has custody. Recently, the child has been losing weight and it's a major concern. Per reporter, it came to their attention that he would require a feeding pump so he could have continuous feedings through his G-tube to help increase his weight. This raised a red flag because most children with his condition would go home with a feeding pump. Reporter called the company (Grace Healthcare) to check-in and they said he did get a pump but a week after the grandmother called and told them to come back and pick it up. She said the grandmother said the child was eating from a bowl but he shouldn't be eating anything by mouth. Reporter asked could they get a new one and they sent a newper recommendations of the nutritionist. Per reporter, they got word this morning that the pump had been returned. There are concerns the mother and grandmother don't understand how to take care of the child and how to appropriately feed him. His condition is not life threatening at this point. The reporting party (RP) stated her son Da' Rell Jones DOB: 10/12/85 has lived in the facility for approximately 13 years. The RP stated on a visit she observed that her son's right eyebrow was swollen and escorted her son outside of the home. The RP stated her son is non-verbal but understands and is able to shake his head yes and no. The RP asked her son if someone had hit him and he nodded yes. The RP stated she named off the names of the caregivers and when she came to a caregiver named Chips, her son identified the caregiver by nodding no. PER REPORTER: Keshia said she received the following information from a Nurse Practitioner by the name of (Tammie) that works with her. She said according to Tammie, Malia was brought into the clinic today by her babysitter (Linda) to be seen by a physician due to her suspecting that the child has been sexually abused. Tammie said Linda told her that she keeps the child for her uncle due to him being her caretaker at this time due to her mother being in jail. However, Tammie said Linda is suspicious that the child’s uncle may be sexually abuses her. On 10/16/2015 I spoke with Kalila Dowden, parent of Kaylee P. at the operation . Ms. Dowden had come to pick her daughter up while I was at operation for the inspection. I spoke with Ms. Dowden in the office. Ms. Dowden stated Shana Myers the teacher of Kaylee had contacted her and stated she believed Kaylee had a seizure on the playground and EMS had been called. Ms. Dowden stated she had told Ms. Myers she was on her way. Ms. Myers stated shortly after arriving at the center EMS had shown up and she had refused care as she believed her daughter did not need to go to the hospital. Ms. Dowden did state she did not take her daughter to the doctor or hospital as her Neurologist is in Dallas and Ms. Dowden states she is aware of what she is suppose
R/s Christopher age (7) is an insulin-dependent diabetic. R/s Christopher medical needs are not being met. R/s special insulin is required for Christopher when he eats but sometimes he doesn’t get the insulin. R/s paternal grandparents Janet and James Hariis have custody of Christopher. R/s bio dad Timothy is incarcerated and bio mom Amanda had court on yesterday for previous drug charges. R/s Christopher condition is severe and the matter of him getting his medicine correctly is life and death. R/s there is a concern if Janet is physically able to care for the child due to Janet having Pancreatitis and Cirrhosis of the liver.
Ms. Tocco refuses to allow nurse case manager to contact medical providers. Appointments are not communicated in a timely fashion and then when they are she doesn’t want me in the examination room. I do not have the opportunity to speak with a medical provider to clarify or obtain documentation supporting what Ms. Tocco reports is a result of her auto accident.
The reporting party (RP) stated the foster child arrived to the hospital via ambulance on 9/21/15 at 7PM due to seizure disorder. The foster parents were not present during the transport to the hospital. The RP was concern regarding no bedside presence from foster parents. EREPORT # 19159 stated the following: We have a toddler, Kyrie Peterson, who has been enrolled since 11/5/2015. We are starting to have serious concerns about the parents ability to care of her and they are expecting #2 any day now. Anytime we try to reach them we get voice mail from both parents. Last week on Thursday we sent her home to get checked for Hand Foot and Mouth, she comes back Friday and mom said her ears are all clear with a Dr.s note. We didn't send her for her ears. According to the reporter, the victim has a really bad diaper rash; in the inside of the vagina lips, there is white crusty stuff coming out. PER REPORTER: The mother works at Wal-Mart in Louisville. Sandy said she has custody of Deven but it's unknown if it's true. Deven is very malnourished. E-Report stated the following: Koltyn’s parents were supposed to bring him to the pediatrician’s office for a follow up appointment due to failure to thrive. His parents missed the appointment. He was admitted to the hospital for almost 2 weeks prior to this and had weight gain in the hospital. They went home and are back in the hospital for the same reason and respiratory distress. They failed to follow up after discharge from the hospital and he still has no vaccines. This has been an issue with the parents repeatedly.
