SC received a telephone call on 10/16/2015 stared 9:34 and end at 9:41 am from Tricia Crooks at Liberty Resources Home Choices (LRHC) Community Outreach and Enrollment Leader. Stating that she spoke Pa and he wants to resume his service order with LRHC for PAS service. SC informed SC that this information will first need to verify with Pa. SC expressed concerns about LRHC being able to fulfill service since they had the case unstaffed for over two weeks (09/25/15-10/15/2015). Tricia apologized on behalf of LRHC, and stated that they have someone assigned and is ready to go all is needed is the resumed service order ASAP. I called the constituent, she asked how can she file a grievance against a CW caseworker that is no longer working for the DHS. She said that Karl Kikman, who was the CW CPS caseworker had told her back in June, that if she cooperates with CW and let him get in her house, this CW case will have a UNFOUNDED disposition, but he lied, after she fully cooperated with him, the case has been coded as a FOUNDED.

Returned call to Ms Deborah Turner. She wanted to complain about a CW worker that did not allowed see her great-niece Astasia (2years old) and great-nephew, Odion (4 years old).

In Greensboro area, there are a lot of communities that can provide assistance to an older adult at different level. My patient is n general an independent one in all aspects of her life and she would like to be in control in everything regarding her life. She is leaving in her nice apartment by herself but in the same time she enjoy being in company of other people, especially people on her age or closer. Therefore, I consider that the best community for her to attend will be a day care community, where she can go almost every day and she can spend some quality time with other older adults. I chose a day care community because I know she needs company to avoid episodes of depression that she has occasionally, accordingly with what she told me at one of my visits. One of the problems that I noticed on the WIC Program here in Pasadena, CA is the lack of adequate portion control nutrition education that is being conducted. After being more familiar with the types of participants in the in the program, it was noted that the program has a high number of overweight/obese children in the program. Moreover, the number of weight retention on postpartum mothers was high. Participants come to the program every 1 to 3 months and most of the appointment last for about 30 minutes. Nutritionists usually see participants that are at a higher nutritional risk while the Nutrition Assistants (WNAs) see the rest of the participants. Before the participant are seen, they are given a nutrition questionnaire to fill out and

AJ is a sweet young boy with a bright smile at times. He makes good eye contact and is very social with people he is familiar with. He receives in home therapy from May Center Early Intervention Program which includes occupational therapy, physical therapy, speech therapy, and a developmental specialist on a weekly basis. AJ also receives biweekly ABA services to help with behavioral challenges.

MSW student called to client's mother on 10/5 at 11:05 am to follow-up on the referral. The student asked mom how the child is doing. Mom reported that the child has been hospitalized. Mom says the child is recovering from the surgery he underwent on 10/2. Mom mentioned that the child may expect additional surgery. The student attended mom's feeling and statements. SW discussed the TRP and checked with mom's interest to consider the services following the child's recovery. Mom mentioned that she would like the child to receive trauma-focused treatment. The student provided TRP contact and encouraged the mother to contact the TRP with a question. The conversation ended at 5:08

The following section is a commentary to the operation of the program in realtion to the subject for process of community devlopment. According to Maxwell, this service provided by YWCA is a a people-based change aiming to give the single mothers the resiliance to cope with the risks and problems they are likely to face in life and to acesss help when it is needed. Also, as mentioned by Brown and Hannis, effective communuty work requires awarness of all the steps in the community development process model and recogniztion the imporatnce of relationships at each point (Brown & Hannis, 2012, p. 73). After analyzing the organization’s resources and information, I have found that the organization essential has everything that the single mother needs, however there is a lack of opions, especially needs for the single mother along, with problems for the housing communities and finally the lack of public awareness to the organization.

Case management services were provided through an FTM (family team meeting) in Anaheim. Present at the meeting were WYP (Wraparound Youth Partner) Andy Ngo, WPP (Wraparound Parent Partner) Maria Gallardo, WCC (Wraparound Care Coordinator) Francisco Isais, PO (probation Officer) Yolanda Talwar, and the youth. The youth’s mother was not present, due to an appointment she cannot miss. The team started with the positives and successes. The youth is trying to reflect on his anger toward his siblings, the youth is going to school on time, and the youth is trying to behave at him. The youth was sharing more and voicing his opinion during the FTM. The team was amazed. WYP Ngo was proud of the youth. WYP Ngo praised the youth. WYP Ngo let the youth
