This is a cross-sectional study. The sample size was estimated using Epiinfo statistical software. The population of interest was estimated to be approximately 400 subjects with BMI > 30 Kg/m2 from a total population of approximately 1,000 receiving health care at Policlínica Bella Vista (PBV) in Mayaguez PR. Estimating a prevalence rate of 0.3 (30%) of Obstructive Sleep Apnea (OSA) in this population, the program yielded a sample size = 179 to estimate the prevalence with a 95% confidence level. at 180.
After approval by the IRB of the Ponce Health Sciences University, the study was conducted at the PBV. Inclusion criteria: subjects of any sex, at least 21 years old and with body mass index (BMI) of at least 30Kg/m2 who visit Policlínica Bella Vista during the study period (09/01/19 to 01/31/17). Exclusion criteria were subjects already diagnosed with OSA, or with overt symptoms of OSA or with upper respiratory infections to reduce the chance of contamination of the equipment and/or alteration of the test results. Participants were recruited according to availability, asked to complete the Stop Bang questionnaire and explained about the overnight use of a HSAT equipment (RESMED Apnea Link Air Home Sleep Test) and to return it the next day. Although obstructive sleep apnea (OSA) is known to be a common form of sleep-disordered breathing and increase the overall risk of mortality by one and a half times in serious cases, many patients with OSA remain undiagnosed and unrecognized by healthcare professionals (Stuart et al, 2013). Depending on the source, up to approximately 10% of women and 25% of men have OSA, while the overall prevalence of OSA in the general population is between 3% and 7%. OSA is more common in individuals who are obese, older, male, African American or of Pacific Island descent, have a family history of OSA, or have a large neck size/small airway. Patients with OSA have been shown to have a higher incidence of significant systemic conditions such as hypertension, cerebrovascular accident, myocardial infarction, arrhythmias, diabetes, and dementia. Some of the eye problems linked to OSA include glaucoma/other optic neuropathies, retinal vein occlusions, central serous retinopathy (CSR), papilledema/idiopathic intracranial hypertension (IIH), floppy eyelid syndrome (FES), and corneal disorders (i.e. keratoconus). This provides optometrists and ophthalmologists with a tremendous opportunity to play an important role in diagnosing and managing patients with OSA as well as improving their quality of life.
