According to Dr. David Sackett, Evidence Based Practice (EBP) is the conscientious, explicit and judicious use of current best evidence in making decisions about the care of the individual patient. It is a clinical decision-making process in which clinicians use theory-derived, research-based knowledge to inform their decisions about care delivery. Most importantly, consideration of individual needs, preference and resources must be included.
In a world of increasing competition for health resources economic evaluations are essential to provide evidence to decision makers that allows them to make appropriate decisions regarding the best use of those resources (Cohen and Reynolds, 2008; Williams et al., 2008). Critical appraisal is the means by which the validity of this research is assessed and is essential for true evidence based practice, and decision-making (Burls, 2009; Ciliska, Thomas and Buffett, 2008).
In my opinion, the rapid critical appraisal (RCA) process was not rapid. It was a process requiring considerable effort and time. Though tedious, the RCA process helped me to stay focused, analyze in depth information about the studies, and select most relevant articles that would be beneficial in answering the PICOT question and appropriate for my evidence based practice(EBP) proposal. According to Melnyk & Fineout-Overholt (2015), RCA is one of the key components to the evidence-based practice process. It helps to evaluate the validity, reliability, and applicability of a research article.
I am in agreement with your process of critical appraisals; identifying the steps in the research process, determining the strengths and weaknesses in the studies and evaluating the validity and credibility of the studies seem to be the root elements of critical appraisals. I appreciated the scholarly article you chose linking depression and mortality among diabetes mellitus patients, particularly because of the straightforwardness of the article. it seemed the conclusion and points of the article were straight forward and the article was easy to understand. There are a number of different ways for promoting evidence based practices (EBP) inside a health care environment. One of the most common is the organizational approach. This is when staff members are sharing their knowledge with each other through experience and discussions about the best techniques. At the same time, information is disseminated through policies and audit reports. (Gerrish, 2004)
Evidence based practice is the basis for needed change in practice and function. It is a sound method for scientific, fact-based change. Changes which have no evidence to support them are fragile, unscientific, and subjective. These changes don’t effect real change over time, as they aren’t able to be proven to a more general population.
A qualitative study, by authors Bogossian, Winters-Chang & Tucket (2014), was examined using the Critical Appraisal Skills Programme (CASP) tool (CASP, 2013). Through the use of the CASP tool, the article was examined by this author to determine if the results of the study were valid, what the results were, and what application or purpose they support. The qualitative study demonstrates attrition from nursing, contributing factors, and possible solutions to maintain nurses in the profession.
Evidence-based practice will be extremely helpful among the hospital setting because it will allow for me to learn from reliable resources. Using evidence-based practice thus far has helped for me to learn about different cultures and become more prepared when meeting with patients. I will continue to work within the hospital setting and need to be sensitive of how different cultures feel about the medical system as well as treatment options. At times, I have found myself wondering why my patient does not want to go a certain treatment route and many times I can use evidence-based practice to get a better understanding. Many times evidence-based practice has allowed for me to share articles with coworkers and have dependable evidence to fall back on when advocating for my patient within team meetings.
