DEBBIELY RAMOS On 10/18/2017, CM met for the first with the client since the case was re-assigned on 10/12/2017. Client is a 35 year old Asian female with no children. Client was dressed appropriately for the weather. Client appears to be friendly, cooperative and forthcoming with information. She maintains good eye contact and shows no sign of psychosis. Client reported she does have family members here in the USA and abroad in the Philippine. Client reported during the days she is busy at the library searching for scholarships. Client report she is exploring scholarships programs for immigrant students. Client reported she is seeking online employment as a retail sale person or any computer jobs. Client report she is an immigrant with a student visa. Client is aware she will need to participate in free immigration service. CM referred client to CAMBA Legal Immigration via email. Housing Update: client NY NY I, II was approved. Client is waiting for DHS manifest to tour apartment. Another alternative housing is MRT once client SSI is approved. Client also mentioned she signed up with Brightpoint Health Home Health Services since 5/9/2015, Client report she will like to sign up with CAMBA/Home Health and she provided BrightPoint Home Health approval letter for CM to submit to CAMBA/Home Health Coordinator. Client is waiting for her coordinator at Brightpoint to return from vacation to close her case, so that she can sign up with CAMBA/Home Health
On 2/11/2016, CM met with the client for Bi-Weekly ILP Review. Client appears to be cooperative and friendly. She was alert, satisfactorily groomed, and casually dressed. She was very loquacious.
I: CM guided client through ISP goals. CM inquired about updates related to the client’s housing goals. CM used open ended questions to inquired about the client most recent drug use. CM reminded client that starting Monday the 2nd he would have to meet with CCM for weekly case management going forward. CM continued to assess for PTSD symptoms, substance abuse, and medication compliance.
On 9/15/2016, CM met with the client for Bi-Weekly ILP Review. Client was dressed appropriately for the weather. She was well groomed. As usual client in the meeting was loquacious, and loud. Client continues to be hostile towards this worker. Client continues to repeat to this worker as quote: "I am not mad with you, and this has nothing to do with you. It's all about DHS and your supervisor." Client was hostile due to an Authorization of Release Form client signed for staff to contact her therapist Dr. Iris Yankelevich for a copy of the client psychosocial and psychiatrist evaluations. Client continues to report that she didn’t signed the Authorization of Release Form because the form stated that the form is for HRA 2010e and she doesn’t know how many time she need to repeat herself that she doesn’t meet the ‘CRITERAI FOR SRO”. CM tries to explain to the client that staff is requesting a copy of her psychosocial and psychiatrist evaluation, but then client continue to talk over CM and threat to contact the Coalition and DHS. Client continues to set in her way and refuses to work with staff to meet her unmet needs. CM observed that the client walk with a book bags and a small shopping cart. Client reported she walk with all her documents and letters from Adult Protective Services & Social Security Administration stating that she is capable of living independently.
On 7/11/2015, CM met with the client for Bi-Weekly ILP Review. Client arrived early for the meeting. In the meeting client appears to be well groomed with good hygiene and dressed appropriately for her age. She was calm, cooperative and well related. She discussed typical issues re: her political and activism work and automobile accident disbursement. She continues to relate her paralegal was in the neighborhood and she wanted this worker to meet her. Unfortunately, there was no parking and she left. She also mentioned the paralegal brought her documents that she need to filed with the state and the federal for her automobile entitlement.
CM inquires how the client is doing medically. Client appears to have an attitude when she replies” nothing changes, everything is the same.” She was a little hostile in providing update information.
On 7/23/2015, Client attended to her ILP meeting with her youngest son Marcos. Child was observed well dressed for the weather. Client stated that she didn’t attend to her ILP meeting because her son was sick. Client also stated that she have a boil in her waist.
I- CM inquired if client received a phone from LA Family Housing to schedule a CES assessment appointment. CM inquired if client visited the Bob Hope Patriotic Hall to fill out VA claims for service connected disabilities, and to start his “Veteran” California Driver’s License application process. CM informed client the procedures of referring him to dental. CM conducted a 90 day ISP with client. CM continued to inquire if client was interested in creating a savings plan. CM continued to assess client’s mental health and medication compliance
On 7/15/2015, CM conducted a search via Security to locate client. Client was found sitting in the patio. Security asked the client to go upstairs to the 3rd floor to see social services. CM met with the client and completed Bi-Weekly ILP Review. In the meeting the client was well groomed and dressed appropriately for the weather. She appears to be calm, cooperative and friendly. CM inquires how client is doing since the last Bi-Weekly IlP. Client replies she is doing well and she is still waiting for DHS Manifest if she was approved or disapproved for the apartment” CM reiterates to the client as per P4P meeting on 7/9/2015, client was informed to be “open minded” and also there is a possibility she may not be accepted for the housing.
Declaire informed HS that Mr. Palmer is unemployed at the moment because client is undocumented. As per Ms. Declaire this issue makes it hard for Mr. Palmer to find a job. Also, client stated that he is not looking, everyone turn him down. Ms. Declaire stated that she is employed at moment and earns about $1200 a month. Family PA case is active at this moment.
On 1/5/2016, CM met with the client to complete Bi-Weekly ILP Review and housing plan. In the meeting client was cooperative and friendly. She was dressed appropriately for the weather.
On 8/12/2016, CM met with the client to complete Bi-Weekly ILP Review. In the meeting CM observed that the client was wearing the same clothes from last session. She was hostile in the meeting. Client was wearing a yellow skirt and black blouse. Client was wearing summer hat, reading glasses and a mask covering her nose and mouth. CM also observed client had foul body odors. During the interview session client continues to be loquacious complaining that “People are out to get client and how residents are using Marijuana on her foot. Client continues to reports some of the residents here at the shelter are part of the “Russian Mafia” and when they used Marijuana on both leg it triggered the open wound ulcer and it cause it to discharge foul odor.
The purpose of this meeting is to complete case management intake forms. Client arrived fifteen minutes early to appointment. Client was in a good mood this afternoon. Client stated pampering themselves due to getting from new employment. Client was also laughing and joking around during the case management intake meeting. Client seemed very happy today. Client attire was presentable; client wore sandals due to getting a pedicure and client clothes were clean. Client stated working as a home health aide for Right at Home located Darien. Client stated to this CM working Friday at 12:00 PM to Monday at 12:00PM. This CM inquire how client is doing and client stated to this CM doing good. Client reported to this CM working in New Canaan as a Home
Client is undocumented and she is not entitled to any government entitlement and she not entitle for legalization.
Reading the case, they were unsure on what they need to do with being them undocumented. With the fear of being deported, they opted out to receive medical and financial assistance. This cause great deal of financial problems. In order to evolve from the political issues, they will have to start working on resolving the immigration issue. The family need to work with an advocate that will help them gain information to become a citizen or at least purse the immigration
