There is a growing trend where physicians are choosing different specialties instead of choosing primary care, primarily due to the low reimbursement rate in primary care. According to Iglehart (2014),” the Association of American Medical Colleges (AAMC) still projects a shortage of 130,000 physicians by 2025, split almost equally between primary and specialty care” (para. 1). One of the suggestions is for licensed practical nurses and registered nurses to incorporate health care education and counseling, using evidenced based clinical guidelines, to patients with chronic conditions through continued care offered via home visits and telephone follow-up calls (Institute of Medicine, 2010). It is also recommended to utilize APRNs as primary care providers in both outpatient and inpatient settings as this decreases the provider to patio ratio thus potentially enhancing the quality of care, and it allows the APRNs to practice to their fullest abilities (Institute of Medicine, 2010). APRNs participating as primary care providers widen the opportunity for health promotion, disease prevention, and limiting disabilities via early diagnosis and treatment. The Kentucky Coalition of Nurse Practitioners and Nurse Midwives (KCNPNM) have concerns with three major medical and political issues. One of those issues is a concern about health care to the poor and uninsured. According to the KCNPNM (2014), Kentucky alone has 20% of the population at or below 100% poverty and 41% under 200% poverty in 2007. Their focus on this is driven by the fact that those uninsured suffer negatively, including premature death and illness, due to lack of medical care (KCNPNM, 2014). As a group, they make an effort to provide the underserved community with information regarding available healthcare programs (KCNPNM, 2014). They also work to encourage legislation to aid those without health insurance by reaching out and informing local and regional lawmakers of our due diligence to promote health in all populations (KCNPNM, 2014).

While reflecting back on the previous weeks, I have to say that this was the foundational class for the FNP students. Before this class, I used to think how I am going to fulfill the role of a Nurse Practitioner (NP). This class helped me to better understand my role and gave me the confidence that I can fulfill the role of an NP. Today, when I look at myself, I know I am in the stage of advanced beginner in Patricia Benner’s Novice to expert theory. In order to practice as a Nurse Practitioner (NP) in Indiana you must hold a state registered nursing license complete a master 's program with certain course requirements. According to NursingLicensure.com (n.d.) there are two educational options leading to Advanced Practice Nurse (APN) recognition in Indiana: obtain a master 's degree or higher in nursing, or obtain a bachelor 's degree in nursing plus national certification. There is not a specific application to become a NP unless you are also pursuing prescriptive authority. Most laws in Indiana focus on regulating practice of nurse practitioners center around prescribing.

Disease prevention is crucial to the future because there are so many diseases that are preventable, some patients just need the information to understand. I feel that there are so many people that need primary care providers and there are not enough physicians available for the growing number of patients. Nurse Practitioners are able to help bridge the gap for the shortage of health care providers. I am eager to start the Family Nurse Practitioner Program to help with this shortage.

The healthcare system in America, is a system that the nation is working hard to improve and perfect. This means the rules and regulations in healthcare continue to change, and often times change rapidly. In the years to come, nurse practitioners (NPs) are predicted to hold a prominent role as healthcare providers. Currently, there are many rules and regulations that prevent NPs from practicing to the full extent of their educations. While the NP certification is standardized nationwide, regulations vary from state to state and many NP are still fighting to be recognized as highly qualified care providers. These current issues make uniting the voices of NPs everywhere important for the progression of the NP career.

The underserved populations have increased exponentially. The Rural Policy Research Institute (2009) defines the medically underserved as, “the ratio of primary care physicians per 1,000 population, the infant mortality rate, the percent of the population with incomes below the poverty level, and the percent of the population age 65 and over.” By that definition, over half the state is considered to be an underserved population. With the demand and needs of the state, it is no surprise that the FNP has become a more utilized provider of healthcare is many settings. There has also been a recent emergence of nurse-managed health centers or (NMHCs), as a form of primary care delivery. According to Espirat and Debisette (2012), NMHCs reduce Medicaid costs, are a great Segway to community outreach and provide quality primary prevention.

It looks at the goals of nationalized healthcare 's use of nurse practitioners and discusses to what extent this policy meets these goals. This paper begins with a detailing of the problem addressed by the policy and the goals and objectives of nationalized healthcare 's increased use of nurse practitioners. Next, alternatives to meeting the increased needs of the health industry, other than through the use of nurse practitioners, is presented, along with an evaluation of the impact of these alternatives.
