On this date worker received a call from Ms. Val Wilson, RN at Alacare HH. Ms. Wilson has been Mrs. Brown’s nurse and been in the home multiple times. During Ms. Tara Brown time as caregiver the home was clean, food cooked, and Ms. Brown received her medication. According to Ms. Wilson Tara moved in during the summer after Larry Brown, son of Ms. Brown locked Ms. Brown out of her home. Tara told Ms. Wilson that her father, Larry Brown was MR and suffered from Schizophrenia. She also reported seeing bruises on Mrs. Brown’s arm where Mr. Brown would grab her. On 3/28/16 A Case conference has been conducted with Client Alexandra Almonte #119, along with HS Supervisor Ms. Garland, RS Supervisor Mr. Mitchell and Sr Case Manager Ms. Arias. The reason of this conference was as per client request due to a previous incident that occur on 3/15/16 that a smell of marijuana was coming out from Ms. Almonte unit, which she denied , stating that she did not some her unit and she refused to open her door at moment because she was in the shower. Ms. Almonte received a writer warning from her Case Manager Ms. Arias and that upset her therefore she request a conference with Ms. Arias supervisor. Drinking Amount: 2-3 standard drinks (beer) per night, up to 6 on a Saturday night. Problems: Becomes nervous if stops drinking for more than a day and has insomnia. Suzy presented to counseling due to symptoms associated with increasing anxiety over the past several years. She is a 27-year-old female who was referred by her primary care physician over anxiety concerns, and has admitted that she drinks in excess 3 days per week. Furthermore, Suzy also reveals that there are biological and genetic predispositions to substance abuse within her family, citing both her mother and father’s struggles with substance abuse. During the very first session, Suzy expressed concerns related to her ability to even come to counseling without sedating medication (Laureate Education, Inc., 2012).

Alice Williams mother seems to be diagnosed quite healthy for her age, yet she can’t see to stop calling her daughter throughout the day and night hours. This causes Alice to mistreat her husband and kids yet they also put aside their social activities and time in order to help out with Beth. The situation makes Alice feel guilty and anxious due to the fact that her loved ones might be right. It seems like Alice’s mother isn’t used to being independent. She wants her daughter to focus all of her attention on her after the death of her husband. In Sara's case, her priority was trying to beat her cancer at all cost and live a long life for her family and Vivian her newborn daughter. Until the end Sara had the mentality that she would beat her disease, therefore dying was not an option for her even though it was inevitable in her cause there for hospice would not be good for her. In order to qualify for hospice, you need to be aware that you are terminally ill and dying, and have a doctor certify that you have less than 6 months left to live. Once someone comes to the conclusion that there is not much time left, they soon prioritize the memories and time that they have left and leave behind treatment and focus on palliative care instead so they can be pain-free as
