CCIB received correspondence from reporting party (RP) stated that he has been investing the owner of this Independent Living facility, Elvira Ferreria, who currently has a hospice resident who requires 24 hour care. Ms. Ferreria has billed the resident's family for her services (2 checks have been written on the resident's account by Ms. Ferreria). When interviewed last week by RP, Ms. Ferreria acknowledge she was acting as an unlicensed facility back in 2015. Ms. Ferreria also currently has another hospice resident in this home (no names provided for either resident). RP stated that Ms. Ferreria stated that she regularly receives resident from UCSD.
CCIB Intake received e-mail from John Merrill regarding the initial complaint. The information is the same as the LIC802. LA City Fire Department of the location being problematic due to the number of request for medical assistance. There is a resident who is diabetic and a victim of strokes and kidney failure. He is reported to be in need of care and supervision. He was ordered by the Court to be placed at the location and is assigned to a Probation
Mental health treatment for the child was discussed with Ms. Cardiello. The mother indicated that she has considered mental health treatment for the child however, it is not covered under the insurance.
CCIB Intake LPA McGaskey received a call from resident Todd Sabin who engaged in a conversation with caregiver Martha (last name unknown). According to Todd the caregiver contacted Todd's social worker Javier Serna (626) 471-6402) and left a message on his phone stating Todd threatened to kill her and that she is afraid of him. Todd's social worker discussed the telephone message. The social worker reported he spoke with Martha and asked why didn't contact the police department if she was afraid for her life. According to the social worker Martha had no response to the question. Todd stated Martha informed the social worker he does not sleep at night and wanders through the facility. Subsequently she informed the cook to do whatever Todd wanted
CCIB Intake received a formal complaint letter from California Advocates For Nursing Home Reform (CANHR) Long Term Care Justice and Advocacy stating the facility failed to maintain the required liability insurance to cover injury to resident or guest. Cecelia contacted SC via phone Cecelia. Cecelia informed SC that she was concerned about Audrey’s weight. She noted over 30 pounds in the last year and half. SC asked if she address the concern with the provider. She notes yes. She noted the provider took Audrey to her PCP to have blood work done. She noted the blood work show there was no issue with her thyroid. She noted the provider had the PCP to check for Side-effects from medication. The reporting party (RP) stated resident Gladys Olson age 87 DOB: 1/9/30 needs are not being met in a timely manner. The RP did not specify exact what those needs where. However, the RP did state the caregiver named Delphine Shanley provides unhealthy meals to the residents. Furthermore, the RP stated the facility contains mold and rat feces. Additionally the RP stated the caregiver is not properly cleaning the facility (no specifics regarding the lack of cleanliness). Overall the RP is reporting neglect of resident Gladys Olson.
The reporting party (RP) stated her father Charles L. Clemons DOB: 2/22/31 was admitted to the facility on hospice. The hospice was provided by Kaiser Permanente who provided all the care for the resident. The RP stated after 5 days in the facility her father passed away. The RP paid $4200 for rent and request to have the pro-rate amount of the rent refunded. The administrator Susan Kendakur informed the RP that she would not refund any portion of the rent due to her father being a hospice resident. The RP disclosed that the facility did not provide the hospice care for the resident and should be entitled to a refund. TC: On August 16, 2016, Ms. Tierra Brown (victim) contacted this Agent via telephone, in reference to the Subject. It was inquired if the Subject was still in contact with Ms. Brown and if she needed any resources from our agency due to the Protection Order that was filed against the Subject by her. Ms. Brown advised that the Subject and she have not been in contact with each other and was unware of any Protection Order that was filed against him. On 3/31/17, Elsie (Michelle) Miller made a falsified documentation on a patient. Elsie wrote bed notes on a patient stating that, the patient was sound alseep during 15 minutes check, while the patient was actually not on CRU 2 at the time. The patient was actually discharge from the unit after been transferred to the hospital for medical treatment.
