COMPLAINT On November 18, 2014, Allison Lehane submitted a complaint in reference to Adam J. Malinowski, Esq. Lehane alleges that Malinowski did not appropriately provide a bill for services, failed to disclose a “fee rate summary”, failed to provide a fee agreement, failed disclose the need for additional funds before sending a bill, and failed to communicate the status of her retainer. Malinowski has allegedly violated Mass.R.Prof.C.1.4(a)&(b), 1.5(b), 1.15(d2),and 1.16. FACTS In 2009 Lehane initially retained Malinowski to assist in a guardianship matter for a minor child; additionally she received assistance from Malinowski in 2012 when the child was having issues at school. More recently on or around January 6, 2014, Lehane retained Malinowski and paid a retainer fee of $2500, to assist in an investigation, which involved a security breach of her computer and cell phone. Lehane’s personal computer and phone were allegedly “hacked, ‘and she hoped to discover who was responsible. Malinowski corresponded with Newbury Police Detective Wotjtkowski, in an attempt to initiate a criminal investigation. Mr. Byrne's is African American 5'10,160lbs with short black hair, wearing unwashed clothing and he has a dog named "Floyd".Mr. Byrne's joined the Army in 2002 to 2006, After the released from the Army he has been since he homeless. Mr. Byrne stay with a girl friend for a short period and some friends however neither situations worked out. Julie Gurnick is a broadcasting major at Point Park University from Jeannette, commuting an hour to campus each day. She was interested in Point Park University because it offered a good program in communications. Gurnick also holds an associates degree in business communications form Westmoreland County Community College. After graduating from Point Park University Gurnick aspires to become a sports caster. Gurnick is currently a free-lance employee for the Tribune Review as well as a dietary aid at St. Anne Home.
Shadale Gillespie is a Direct Care Provider for our agency since February 2015. Work is not conistant due to the amount of Respite hours authorized by the AZ DDD department for the child she services. The authorization avaialble hours have ran out and she has not been able to work with the child since July 9, 2017. Last paycheck on July 25, 2017 for gross amount of $400.00. Bi-monthly pay periods, rate of pay $10.00/hour on average of 10 to 20 hours per week when we have authrizations available to deliver services.

Latasha Willis, an intern at River Valley Area Agency on Aging, is currently at Troy University to obtain a Masters in Social Work (MSW).Latasha will be interning with the agency until July 2016 which is when she graduates with her Masters degree. She has earned her undergrad in Social Work (BSW) as well from Troy University in 2014. After graduating with her BSW, she worked with the Division of Family and Children Services as a CPS investigator in Butler, Ga. A1. The Nature of the incident was that an employee was able to hack into the computer system and gain access to the financial payroll system, human resources and even email system. This employee used several methods in order to gain access into the system: IP spoofing, Data modification, Man in the middle attack and compromised-key attack. As a result the employee was able to tamper with payroll system. An auditor discovered the discrepancies and tried to make upper management aware of the situation through email, but the email was intercepted by the hacker. A couple of culture-specific protective factors that may have helped Andrea cope with the racism that she faced church attendance, community involvement, and family. Church attendance would have helped Andrea by helping her recognize that racism exist and helping her forgive. If Andrea isn’t religious, her community might also have helped her cope with the racism she faced. She could've created a support group with other individuals that have also faced racism. Her family could of helped her with social support such as caring, love and affection.
