I called the constituent one more time and she said that is struggling with CW Roseburg since last 11 months. She said that her CW case got three different workers assigned and no one did their job. Last month, July 11th, they had a court hearing and the judge was not happy with the CW caseworker, Erin Summers, giving excuses only (that the worker is new to the case and is not familiar) when this new worker had this case for over a month ago. The judge also said to the worker that is not acceptable he is losing paperwork. Heather mentioned that she had done all that the caseworker asked, but still nothing happen. She said that finally, yesterday the CW worker and she were able to have a meeting to talk about the safety plan and the services
SC received a telephone call on 10/16/2015 stared 9:34 and end at 9:41 am from Tricia Crooks at Liberty Resources Home Choices (LRHC) Community Outreach and Enrollment Leader. Stating that she spoke Pa and he wants to resume his service order with LRHC for PAS service. SC informed SC that this information will first need to verify with Pa. SC expressed concerns about LRHC being able to fulfill service since they had the case unstaffed for over two weeks (09/25/15-10/15/2015). Tricia apologized on behalf of LRHC, and stated that they have someone assigned and is ready to go all is needed is the resumed service order ASAP. CPSW did a home visit to meet with Ms. Berner and to discuss about her safety plan since the children are moving with her on10/29/16 from the foster provider. Ms. Berner was late for her appointment and CPSW waited a 40 minutes for Ms. Berner. Ms. Berner apologized for being late. CPSW explained Ms. About safety plan. Ms. Berner understood and she signed them. Ms. Berner stated that she will be doing a house arrest for two weeks and the recommendation of her criminal court is to obey law and continue taking her medication on time and seeing her therapist. Also, cooperating with her PO and CPSW. CPSW consulted with the supervisor and she has approved both children to move back with Ms. Berner. Goal 1-2
I called the constituent, she asked how can she file a grievance against a CW caseworker that is no longer working for the DHS. She said that Karl Kikman, who was the CW CPS caseworker had told her back in June, that if she cooperates with CW and let him get in her house, this CW case will have a UNFOUNDED disposition, but he lied, after she fully cooperated with him, the case has been coded as a FOUNDED.
CCIB received a Corrective Plan of Action (CAP) detailing the visit to the home on 10/18/17 by Service Coordinator, Sally Cano. Ms. Cano arrived at the home, but no one was there. Ms. Cano proceeded to check Delta Home 3 (located next door) and found 2 of Delta Home Care IV residents along with residents from Delta Home Care 3 at the home. As Ms. Cano was leaving, she observed a bus arrive at the Delta Home Care 2. The bus driver reported that there was no staff at the home and that the residents were waiting outside. It was observed the Delta Home Care 2, 3, and IV to be lacking staff. On 10/19/17, RP and Quality Assurance specialist (QAS) Jesus Ozeda went to the facility to observe the staff to resident ratios and to collect the staff schedules
Housing Update: client NY NY I, II was approved. Client is waiting for DHS manifest to tour apartment. Another alternative housing is MRT once client SSI is approved. Client also mentioned she signed up with Brightpoint Health Home Health Services since 5/9/2015, Client report she will like to sign up with CAMBA/Home Health and she provided BrightPoint Home Health approval letter for CM to submit to CAMBA/Home Health Coordinator. Client is waiting for her coordinator at Brightpoint to return from vacation to close her case, so that she can sign up with CAMBA/Home Health
RESOURCE UPDATE: Client continues to report last week she went to Center 37 and she was told that her Center is now 066. Client was upset that she was transfer to another shelter. As per the client she will lose her Linc V if she is transferred to Center 66. CM tries to explain to the client HRA transfer many clients because Center 37 is overcrowded. Client reported she requested for her case to be closed. Client continues to report she spoke with HRA Rep Ms. Perez who requested for the client to put her request in writing. Client stated she did put her request in writing to have her case closed from center 66. Then the client provided this worker with copy
Mr Steiner originally contacted the GAO on 02/03/2017 to file a complaint against the CW Salem Caseworker and Supervisor because they were not letting him to have a quality visits with her daughter, who is in DHS custody.
Ms. Pollard wanted to know when Javon'Tay would receive the DD waiver. ISC explained the process, than she asked about receiving a case manager. She said she was told by the other lady he would receive a case manager, which would take him out to his appointments and take him places.
She wanted to file a formal complaint against a CW case worker who is her son's CW caseworker. The constituent stated that the caseworker does everything that the mother of the child (who is her grandson) and she believes that the CW caseworker is violating her and her son's civil rights. Her son's attorney had told her if her son wants to have her child back, they needs to gain the CW caseworker and make the CW caseworker her best friend.

Ms Miller has concerns that CW DHS is not acting fairly regarding her child custody issued. She stated that her child was removed from her custody and placed in her ex-husband home. She stated does not have a caseworker assigned and in the court room the CW caseworker did not talked to her, only to her ex-husband. Her child was removed per her request and placed in the father’s home. Now the both parents are fighting the custody of the child in court.
