Anne’s case study Anne is a 10 year old girl who has admitted to the emergency department (ED) with the right iliac fossa pain. Further assessment has done for her by the emergency registrar and appendicitis suspected. The surgical team decides that she needs an appendectomy. Anne returns to the ward with an IV therapy and triple IV antibiotics, morphine PCA, a nasogastric tube in situ on free drainage. Anne is an older of the five children and her parents has a restaurant in the city. This essay will discuss about the appendicitis pathophysiology, the growth and development theories. The large intestine is about 1.5 long and includes the appendix, cecum, colon, rectum and anal canal. Vermiform appendix is a small tail which attached to the cecum and located in the lower right of the abdomen (Kathryn & Sue, 2014). The vermiform appendix has not a physiologic function in the human body. The large intestine and the appendix’s wall have the similarity layer. These layers include a mucosa, a submuocsa, a muscularis, a shiny thin outer layer, and a serosa (Sam, 2007). The appendix receives the blood from “the appendiceal artery a branch of the ileocolic artery”, which provides blood to the end of the small intestine and the beginning of the large intestine (Donna, 2011). The appendix is inflamed, which is called appendicitis. The appendix becomes inflamed when the appendix obstructs by faeces, foreign bodies or foods which may be located in the narrowing of the appendix, thereby increasing the appendiceal pressure, stopping provide blood for the appendix and causing infection (Donna, 2011). If the appendicitis is untreated, it will increase the risk of perforation. If the appendix breaks down, the bacterial will spread to the abdomen. The bacterial can cause an infection where it is localized, known as peritonitis (Donna, 2011). Pain is the main symptom of the appendicitis. This model can help reduce the impact of illness for children and families by encouraging all family members as well as health care workers to work together to achieve the best outcomes for their child (Arango,2011). According to Kuo et al. (2012), the main point of this model is respect and dignity, information sharing, and participation. Firstly, a nurse must listen and respect the patient and caregiver for their background, culture, values, and beliefs. Providing the welcoming, assessable and responsive care to Anne and her family. Protecting privacy during consultation and treatment time for Anne and her family. The nurse should not make any assumption about Anne or her family’s beliefs (Wilfred et al., 2012). The nurse need to find out what Anne and her family know about health problem and treatment. This is because planning and delivery of care will be affected by the patient and caregiver’s perspective, therefore, a nurse should be understanding Anne and her family’s concerns and put them into treatment and care with appropriate support for her and her family members (Wilfred et al., 2012). By doing this the nurse will have the benefit such as gaining the greater trust and respect from Anne and her family.
